A case of Alternaria alternata (Fr.) Keissler infection of die knee in a 48-year-old farmer who had bilateral adrenocotical hyperplasia (Cushing's syndrome) is presented. The lesion appeared following a minor trauma sustained during the harvesting of wheat. The patient was successfully treated with intralesional infiltrations of miconazole.
