    Abstract Background: Adjuvant chemotherapy and endoprosthetic replacement for bone sarcomas of the lower extremity is well established. The specific long-term consequences of these endoprosthetic reconstructions for the patient's affected limb are unknown. Method: The oncologic results and the survival of the endoprostheses were reviewed in 32 patients with primary bone sarcoma of the femur or proximal tibia. There were 26 high-grade sarcomas, and 6 low-grade sarcomas. A proximal femoral endoprosthesis was used for reconstruction in 4 patients, a total or push-through femoral endoprosthesis in 11 patients, a distal femoral endoprosthesis in 15 patients, and a proximal tibial endoprosthesis in two patients. Results: Median survival was 10 years (range, 1.1 to 18.9 years) for patients with high-grade sarcoma, and 8.1 years (range, 7.1 to 10 years) for patients with low-grade sarcomas. Distant metastases developed in seven patients (22%), all with stage IIB sarcoma, with concomitant local recurrence in 3 patients (9%). Five-year overall and disease-free survival rates for high-grade sarcomas were 81% and 73%, respectively. The overall endoprosthetic survival rate was 87% at 5 years, 80% at 10 years, and 56% at 15 years. Median follow-up of the original endoprostheses was 8.3 years (range, 0.6 to 18.7 years). Endoprosthesis-related complications occurred in 13 patients (41%); most complications were mechanical failures. The highest complication rate was found in distal femoral replacements (60%); amputation was necessary in both patients treated with a proximal tibial endoprosthesis. Five endoprostheses (16%) were revised. An amputation of the involved limb was performed in four patients (13%): in two patients because of local recurrence and in the other two patients because of infection. For patients alive at follow-up, the median functional Enneking evaluation score was 22 points (range, 12 to 28 points), with the highest functional scores in patients with a distal femoral endoprosthesis, and the lowest functional scores in patients with total or push-through femoral replacements. Conclusion: Endoprosthetic reconstructions gave satisfying functional results in most patients after long-term survival. However, the proximal tibial and distal femoral endoprosthesis are particularly at risk for long-term endoprosthetic complications requiring additional surgical procedures.

    Abstract In order to gain insight into the management of patients with vascular malformations (VM) in the University Hospital Nijmegen in the past 10 years, 151 cases managed by different specialists were reviewed. To avoid the usual confusion in terminology, all recorded diagnoses were reclassified according to the biological classification of Mulliken. The sex distribution was equal; 79% of the malformations were diagnosed at birth or in the 1st year of life. The median time between presentation and consultation was 3 years. Sixty-two lymphatic, 26 venous, 24 capillary, 1 arterial, and 38 combined malformations (8 arteriovenous, 30 others) were found. The head and neck region was most frequently involved, followed by the lower and upper limbs and trunk. The pediatric surgeon was the most frequently consulted specialist. Confusing, mutually incompatible terminology and a wide variety of different diagnostic techniques and treatments had been used by the different specialists. To improve the management of patients with vascular malformations, the use of a uniform classification, an increase in basic investigations, and the development and evaluation of protocols for diagnosis and treatment by multidisciplinary teams are necessary.

    Summary Ice-edge-related phytoplankton blooms following the retreating sea-ice in the marginal ice zone are frequently observed phenomena. Such blooms are generally short-lived and are followed by a strong decrease in the chlorophyll concentration towards the open ocean, generally explained by the degradation of the vertical stability. Solar heating and ice melting, which control the stability of the surface water of the north-western Weddell Sea during sea-ice retreat in spring were analysed in order to relate the spatial features of the phytoplankton ice-edge bloom in 1988 to the density field. Solar heating has little effect on the density of seawater in ice-covered areas because the thermal expansion coefficient is very low close to the freezing temperature. Outside the marginal ice zone, the temperature effect on stratification increases and the combined contribution of melting and heating on buoyancy input to the surface layer is roughly constant. As a consequence, the low phytoplankton stocks in the open ocean adjacent to the marginal ice zone, after an initial bloom peak following the retreating ice in spring, are not necessarily caused by deep turbulent mixing, in contrast to what is often assumed.

    Abstract The factors controlling phytoplankton bloom development in the marginal ice zone of the northwestern Weddell Sea were investigated during the EPOS (Leg 2) expedition (1988). Measurements were made of physical and chemical processes and biological activities associated with the process of ice-melting and their controlling variables particularly light limitation mediated by vertical stability and ice-cover, trace metal deficiency and grazing pressure. The combined observations and process studies show that the initiation of the phytoplankton bloom, dominated by nanoplanktonic species, was determined by the physical processes operating in the marginal ice zone at the time of ice melting. The additional effects of grazing pressure by protozoa and deep mixing appeared responsible for a rather moderate phytoplankton biomass (4 mg Chla m−3) with a relatively narrow geographical extent (100–150 km). The rôle of trace constituents, in particular iron, was minor. The importance of each factor during the seasonal development of the ice-edge phytoplankton bloom was studied through modelling of reasonable scenarios of meteorological and biological forcing, making use of a one-dimensional coupled physicalbiological model. The analysis of simulations clearly shows that wind mixing events — their duration, strength and frequency — determines both the distance from the iceedge of the sea ice associated phytoplankton bloom and the occurrence in the ice-free area of secondary phytoplankton blooms during the summer period. The magnitude and extent of the ice-edge bloom is determined by the combined action of meteorological conditions and grazing pressure. In the absence of grazers, a maximum ice-edge bloom of 7.5 mg Chla m−3 is predicted under averaged wind conditions of 8 m s−1. Extreme constant wind scenarios (4–14 m s−1) combined with realistic grazing pressure predict maximum ice-edge phytoplankton concentrations varying from 11.5 to 2 mg Chla m−3. Persistent violent wind conditions (≥ 14 m s−1) are shown to prevent blooms from developing even during the brightest period of the year.

    Zusammenfassung Osteosarkome des Femur werden heute, wenn, möglich, mit Chemotherapie, Tumorresektion und Rekonstruktion mit Hilfe von Endoprothesen behandelt. Dabei besteht als Folge der Chemotherapie nicht nur das Risiko einer Epiphysenfraktur der langen Röhrenknochen, sondern auch einer Fraktur im Rest des behandelten Femur. Wegen des Risikos von Infektion und Heilungsstörung bei operativer Therapie sollten solche Frakturen konservativ behandelt werden.

    Summary Osteosarcomas of the femur are treated nowadays, if possible, by chemotherapy, resection of the tumor and reconstruction with an endoprosthesis. There is not only a risk of epiphyseal fractures in the long bones due to chemotherapy but also of a fracture in the remnant of the treated femur. These fractures should be treated conservatively because of the risk of infection and non-union if treated operatively.

    Abstract Of 91 limb-salvage procedures using prosthetic reconstructions because of primary or metastatic bone and soft-tissue tumors 26 revisions were performed in 16 patients. Revision was due to polyethylene wear (9 cases), aseptic loosening (8 cases), recurrent hip dislocation (3 cases), prosthetic stem fracture (2 cases), infection (2 cases), leg length discrepancy (1 case), and traumatic dislocation of a saddle prosthesis (1 case). The follow-up period for tumor control varied from 1.5 to 22 years with a median of 13.5 years. The follow-up period after the last revision operation varied from 0.5 to 12 years with a median of 3 years. At the last follow-up, the functional results had deteriorated compared with after the primary operation in 5 patients and had improved in 2 patients. In the remaining patients, the results did not change.

    Abstract We performed a retrospective study of 38 patients with pigmented villonodular synovitis (PVNS) to evaluate the treatment and functional results. The mean age of the patients was 32 (range 12–72) years at the time of treatment. Three types of PVNS have been identified: localized nodules in 9 patients, diffuse PVNS of the entire synovial membrane in 26, a combination of a diffuse involvement of the synovial membrane and an extra-articular presentation in 1, and extra-articular lesions in 2. The location of the lesions was knee (n = 31), hip (n = 3), ankle (n = 2), femoral triangle (n = 1), and gluteal region ¶(n = 1). The procedures performed were surgery alone, surgery combined with radiosynovectomy and radiosynovectomy only. A follow-up was done after a mean of ¶4 (range 1–19) years in 34 patients. A functional evaluation according to the Musculoskeletal Tumor Society was performed in 31 patients. The mean functional evaluation score of 34 patients was 24 (range 15–30). Most of the ratings were excellent or good, in 6 cases fair, and in ¶2 cases poor. This study demonstrates that the functional results are good despite residual or recurrent disease; in addition, we showed that functional evaluation after treatment gives an optimal view of the impact and results of the operation.

    Summary A rupture of the anterior cruciate ligament will give rise to the signs and symptoms of anterolateral instability after secondary insufficiency of the lateral capsule and the iliotibial tract have developed. Untreated knee instability predispose to meniscal lesions, chondromalacia, and osteoarthritis of the knee. Other reasons for reconstruction are the patient's wish to return the knee to near-normal function and rejection of life-long utilization of a brace. A combination of an intra-articular Eriksson-Drez procedure and an extra-articular Ellison procedure was carried out in 27 patients with anterolateral instability. Follow-up evaluations were performed, with an average follow-up of 4 years and 5 months. The subjective and objective results are encouraging and similar to those obtained with other combined techniques.

    Abstract Cryosurgery is an established adjuvant treatment of bone tumors which reduces the local recurrence rate. In this study, cryosurgical experiments were carried out in rabbits to study the temperature field, the extent of necrosis, and the revitalization process in order to optimize treatment. Intramedullary freezing of long bones with a closed liquid nitrogen cryoprobe and three consecutive sessions induces osteonecrosis down to the –10 °C isotherm without compromising the soft tissues. The application of a tourniquet does not influence the thermodynamics. The revitalization process is distinguished into an osteogenic and a remodelling phase. In rabbits, there is an obvious periosteal osteogenesis starting from 1 week after operation and overlapping the remodelling phase, which starts between 3 and 5 weeks after operation. Two out of eight rabbits sustained a pathologic fracture within 3 weeks of cryosurgery. No pathologic fractures were encountered during the remodelling phase, probably because of the profuse periosteal bone apposition that added mechanical strength. In clinical practice, no profound periosteal bone apposition and a high risk for pathologic fractures during the remodelling phase were noted. Future research should focus on bone strength during the remodelling phase of cryosurgically treated long bones, to decide on the role of preventive osteosynthesis or postoperative restrictions. This animal model is not advised for these biomechanical experiments because of itss profuse periosteal bone apposition.

    Abstract In 86 patients 102 consecutive cemented Kinematic total knee arthroplasties were reviewed 10–15 years after surgery to determine the clinical and radiographic results and to assess the survival rate. The average age of the 65 female and 21 male patients at the time of surgery was 63 years. Forty-six knees were affected by rheumatoid arthritis (RA), 46 by osteoarthritis (OA), 7 by haemophilic arthropathy and 3 by osteonecrosis. One patient ¶(1 knee) was lost to follow-up, and 31 patients (38 knees) died. Eleven knees had been revised for deep infection (4), wear (4), malposition (2) or persistent pain (1). Fifty-two knees were examined at an average follow-up period of 12 years. The mean Knee Society Score of 89 points was the same for RA and AO knees. Also, 92% of the knees caused no pain or only occasional mild pain. There were no cases of aseptic loosening of any component. Progressive radiolucent lines were not seen on the follow-up radiographs (43 knees, mean follow-up 12 years) The 10- and 14-year survival rates with revision as the endpoint were 90% (confidence interval, CI: 81%–95%) and 82% (CI: 67%–92%), respectively. In the worst case scenario, with knees lost to follow-up and knees with moderate pain considered as failures, the 10- and 14-year survival rates were 80% (CI: 69%–88%) and 62% (CI: 46%–77%), respectively. The Kinematic total knee arthroplasty yields equally good long-term results in patients with RA and those with OA. Deep infection and wear were the main reasons for revision.

    Zusammenfassung Die Autoren beschreiben zwei Patienten mit Kniegelenk-Symptomen. In beiden Fällen wurde die genaue Diagnose spät gestellt. Ein Patient hatte eine große Zyste des lateralen Meniskus, während bei dem anderen ein Synovial-Sarkom vorlag. Ätiologie, Symptomatologie, Untersuchungsmethoden, Therapie, Prognose und DifferentialDiagnose werden diskutiert. Es wird unterstrichen, daß eine Meniskuszyste nicht angemessen behandelt ist, solange der Meniskus nicht vollständig entfernt wurde. Die rätselhaften Befunde und Symptome eines Synovial-Sarkoms des Kniegelenkes werden betont. Bei ungewohnlichen Schwellungen des Kniegelenkes ist es unverzichtbar, eine Biopsie vorzunehmen, um einen bösartigen Prozeß, insbesondere das heimtuckische Synovial-Sarkom, auszuschließen.

    Summary The authors describe two patients with knee symptoms. In both cases the correct diagnosis was established late. One patient had a large cyst of the lateral meniscus, while the other had a synovial sarcoma. Aetiology, symptomatology, methods of investigation, therapy and prognosis are discussed, as well as the differential diagnosis. It is stressed, that a meniscal cyst is not adequately treated until the entire meniscus is removed. The puzzling signs and symptoms of a synovial sarcoma of the knee are mentioned. It is obligatory to perform a biopsy in all patients with an unfamiliar swelling of the knee, in order to rule out a malignancy, especially the treacherous synovial sarcoma.
