Investigations were made about the alterations in the bone marrow, in the peripheral blood cells and in the blood serum after one injection of vitamine B12. It was noticed, that the first change was a decrease in the activity of thymidine kinase and dihydrofolate reductase in the bone marrow. This decrease was seen before morphological alterations of the megaloblasts were detected. The morphological alterations after B12 injection, established by common differentiation of the bone marrow, had finished when the peak of reticulocyte crisis in the peripheral blood occured. The rate of DNA synthesis simultaneously decreased in the erythropoetic system to low values, whereas the rate remained higher than normal in the granulopoetic system. Some other characteristic laboratorical symptomes of the megaloblastic anaemia became normal more slowly. The activities of some glycolytic enzymes in the red blood cells were increased and there was also a pathological pattern of haemolytic type in the LDH isoenzymes in the serum. Both biochemical lesions could be still demonstrated over weeks after the vitamine B12 injection.Notes: Zusammenfassung Es wird über vier megaloblastäre Anämien (m. A.) berichtet; dabei handelt es sich um drei echte perniziöse Anämien, von denen eine mit einem Plasmocytom kombiniert war, und um eine durch B12-Mangelernährung (Ziegenmilch ?) bedingte m. A. Es wurden Verlaufsuntersuchungen an Knochenmark, peripheren Blutzellen und Blutserum nach einer einmaligen Vitamin B12-Injektion durchgeführt. Dabei ergab sich, daß als früheste Veränderung ein Abfall der Aktivitäten der Thymidinkinase und der FH2-Reductase in den Knochenmarkzellen zu beobachten ist, und zwar in den ersten 12–36 Std, noch bevor eine morphologische Umwandlung der Megaloblasten nachgewiesen werden konnte. Die bei üblicher Knochenmarkdifferenzierung erkennbaren morphologischen Umwandlungen hatten sich im großen und ganzen bis zum Gipfel der Reticulocytenkrise im peripheren Blut vollzogen. Die DNS-Syntheserate innerhalb der Erythropoese war auf niedrige Werte abgefallen, während innerhalb der teilungsfähigen Zellen der Granulopoese noch ein gegenüber der Norm erhöhter Anteil in DNS-Synthese nachweisbar war. An den peripheren Erythrocyten lassen sich noch Wochen nach der ersten Vitamin B12-Gabe erhöhte Aktivitäten von Glykolyseenzymen messen. Im Blutserum hält sich als längste für die m. A. charakteristische Veränderung ein pathologisches LDH-Isoenzymmuster vom Hämolysetyp; es kann andere Hämolysezeichen wie auch eine Erhöhung der Gesamt-LDH-Aktivität um Wochen überdauern.Type of Medium: Electronic ResourceURL: -
