    A male and female linkage map of hop has been constructed using 224 DNA polymorphisms (106 amplified fragment length polymorphisms (AFLPs), three random amplified polymorphic DNAs (RAPDs), one RAPD-sequence-tagged-site (STS), and three microsatellite (STSs) segregating in an F1 population of the English cultivar ‘Wye Target’-the German male breeding line ‘85/54/15’. Linkage between these loci was estimated using JOINMAP Version 2.0. The final map for the female parent consisted of 110 loci assigned to eight linkage groups covering a distance of 346.7 cM. For the male map, 57 loci could be mapped on nine linkage groups spanning over 227.4 cM. One of these male linkage groups (Gr09-M) presumably represents the Y chromosome, since all markers assigned (10 AFLPs, three RAPDs and one STS) were closely linked to the male sex (M). Because of their sex-specific segregation, 10 doubly heterozygous AFLPs spanning a distance of 18.7 cM could be identified as markers describing the X chromosome, which is part of the male and female map. Three STMSs, which had already proved useful in hop genotyping, could be integrated as codominant locus-specific markers and thus allowed to produce reliable allelic bridges between the female and male counterparts.

    DNA fingerprinting using amplified fragment length polymorphisms (AFLPs) was successfully employed to detect genetic relationships and variability among 90 hop cultivars and breeding lines comprising a collection of the world's hop germplasm. Seven AFLP primer combinations produced a total of 347 fragments of which 151 (43.5%)) were polymorphic. One-hundred and thirty informative, highly reproducible DNA polymorphisms were used to estimate the genetic similarity (GS) which varied between 1.0 (e.g. ‘Saazer’ vs. ‘Tettnanger’) and 1.17 (‘Columbus’ vs. ‘Tettnanger’, ‘Spalter’ and ‘Saazer’). UPGMA (unweighted pair-group method with arithmetic averages) clustering revealed two main clusters, reflecting the two main sources of origin and the two main breeding objectives: one cluster of mainly European origin representing the aroma pool and a second cluster associating accessions with European germplasm infiltrated by wild American genes with less aroma quality, but a higher bittering potential. Each main branch was composed of four or three subclusters with subgroups, respectively. Assignment of almost all genotypes in the dendrogram was consistent with the pedigree data as far as they are known. Consequently, AFLPs are shown to be suitable for assessing the genetic variability in hop germplasm and are useful for describing the genetic relationships among cultivars and accessions, which allows phylogenetic questions to be addressed.

    Increased catecholamine secretion from neuroblastomas can occasionally be demonstrated, but severe hypertension is uncommon. We report the perioperative management of a 5 year old child with stage III adrenal neuroblastoma who presented with malignant hypertension and high norepinephrine and dopamine levels. Hypertensive crises occurred during anesthesia for surgical biopsy and during chemotherapy. After blood pressure control using phenoxybenzamine and enalapril, doxazosin was used successfully as the preoperative α-adrenergic receptor antagonist for surgical tumor resection.

    Summary We present a case report of a child with intussusception who underwent air reduction which was complicated by bowel perforation. Life threatening tension pneumoperitoneum developed rapidly and immediate needle decompression was life saving in this case. The pathophysiology of hyperacute abdominal compartment syndrome is discussed.

    Iatrogenic oesophageal perforation in neonates is well recognized in the medical and surgical literature with intubation injury listed as a possible contributing mechanism besides nasogastric tube placement and suctioning. Diagnosis can be difficult and sometimes confused with other entities. With early diagnosis, nonsurgical management often leads to complete resolution in neonates. We report the case of a 1-day-old premature neonate who was brought to the operating room with the preliminary diagnosis of proximal oesophageal atresia with stump perforation and distal tracheo-esophageal fistula. His intubation for respiratory distress at birth had been difficult due to Pierre–Robin sequence with micrognathia. Oesophagoscopy in the operating room revealed a patent oesophagus but perforations in the pharynx and in the proximal oesophagus with the nasogastric tube entering the pharyngeal perforation. Oesophageal perforation and the limitations of the difficult airway algorithm in small neonates are discussed.

    Background: Cephalad advancement of epidural catheters to the thoracic region via the caudal route has been shown to be feasible in neonates and small infants. This has allowed many young infants to receive thoracic level epidural analgesia with dilute local anaesthetic solutions using the simpler caudal approach. Since radiographic confirmation of the catheter tip is routine at this institution, we wished to determine how often radiographic studies led to adjustment or replacement of the epidural catheter. Methods: After institutional review board approval, we retrospectively reviewed the medical records of neonates and infants less than 6 months of age who had thoracic or lumbar epidural analgesia via the caudal route between August 1995 and January 2000. Demographic data were recorded, including age, weight and type of surgery. The epidural catheter type, tip location by radiograph and any manipulation of the catheter after the radiograph were also noted. Results: During the study period, a total of 115 infants were identified as having received caudal placement of a thoracic catheter. Radiographic studies were available for 86 of these infants. The position of 28 (32%) of the epidural catheters was considered to be inadequate after review of the confirmatory radiograph. Ten of these catheters were determined to be in the high thoracic or cervical region and were pulled back to the desired level. Seventeen of these catheters were coiled in the lumbosacral area and 15 of these were replaced at an adequate level. One catheter was found to be outside the epidural space in the presacral area. No correlation could be found between age, weight, type of catheter or type of surgery and the need for catheter manipulation. Conclusions: Even in young infants, radiographic determination of the catheter tip appears warranted when thoracic catheters are placed via the caudal route.

    Abstract. Study objective. To follow up endothelin (ET), adrenocorticotropic hormone (ACTH), and cortisol levels in patients undergoing aortocoronary bypass grafting (CABG) and to determine whether these are extracted from plasma by the pulmonary circulation. Design. Convenience sample trial. Setting. University hospital. Patients. Eight male routine CABG patients without appreciable concomitant disease. Interventions. Patients were given anaesthesia in a strictly standardised manner using etomidate, flunitrazepam, fentanyl, and pancuronium. Nitroglycerin was administered prior to cardiopulmonary bypass (CPB) at 2 mg/h and dopamine as the only catecholamine starting from CPB weaning until the end of sampling at 3.5 – 5 µg/kg⋅min. Samples were drawn in rapid sequence from cannulated radial and a distal pulmonary arteries (Swan-Ganz catheter) at eight sampling times starting after induction of anaesthesia and catheter placement and finishing 22 h after the end of operation. Measurements and Results. ET levels were determined by an ET-1, 2, 3-sensitive radioimmunoassay (RIA), ACTH and cortisol by commercially available RIA kits. There was a significant (P=0.032, linear regression analysis) correlation between ET and cortisol from pulmonary arterial samples. ET was significantly (P=0.042, two-tailed Wilcoxon test) extracted by the pulmonary circulation after induction of anaesthesia. This pulmonary-systemic arterial difference nearly disappeared intraoperatively, but tended to be restored 22 h after the end of operation at an approximately twofold increased level. Conclusions. No interrelation between ET and the hypothalamic-pituitary-adrenal axis could be established by the ET, ACTH, and cortisol plasma levels. However, the significant correlation between perioperative ET and cortisol lends further support to the hypothesis of ET release by cortisol from vascular smooth-muscle cells. There is a net pulmonary clearance of ET in patients prior to CABG that is lost intra- and early postoperatively, but tends to be restored on the 1st day thereafter at an increased level.

    Zusammenfassung. Anhand perioperativer Plasmaspiegelverläufe von Endothelin (ET), ACTH und Kortisol während und bis zu 22 h nach aortokoronaren Bypassoperationen sollten mögliche Regelungszusammenhänge zwischen ET und der hypothalamisch-hypophysär-adrenalen Achse (HHAA) verfolgt werden. Eine eventuelle Netto-Extraktion oder -Freisetzung von ET aus der pulmonalen Strombahn war ebenfalls zu erfassen. Während ET- und Kortisolplasmaspiegel intraoperativ nur wenig von ihren Ausgangswerten abwichen und erst am ersten postoperativen Tag auf doppelt bis dreifach erhöhtes Niveau anstiegen, zeigte ACTH intraoperativ einen exponentiellen Anstieg, um am ersten postoperativen Tag bereits wieder das Ausgangsniveau zu erreichen. Daraus läßt sich kein Regelungszusammenhang von ET mit der HHAA ableiten; wohl aber findet anhand einer signifikanten (p=0,032) Korrelation zwischen pulmonalarteriellen ET- und Kortisolspiegeln die Hypothese Unterstützung, daß Kortisol ET aus glatten Gefäßmuskelzellen freisetzt. Im Gegensatz zu ACTH und Kortisol, deren Plasmaspiegel durch die Passage der pulmonalen Zirkulation unverändert blieben, wurde Endothelin nach Narkoseeinleitung signifikant geklärt (p=0,042), im weiteren intraoperativen Verlauf kam es zu einem weitgehenden Verlust der pulmonalvaskulären Klärfunktion, die am ersten postoperativen Tag dem Betrage nach wiederhergestellt war.
