    Objective To explore maternal and child perspectives on children’s adjustment in the context of paediatric renal disease, and maternal psychological variables that may account for variance in child and maternal ratings.Methods Forty-three children with end stage renal disease and their maternal caregivers completed the Strengths and Difficulties Questionnaire (SDQ). Mothers also reported on their own mental health, and the strategies they used to cope with their child’s illness. The severity of the child’s condition was rated independently by a renal clinician.Results Compared with normative data for the SDQ, mothers reported their children to be at increased risk of psychological problems. However, the children themselves reported no more problems than a normative sample. Mothers’ coping and mental health explained some of the variance in their ratings of the child’s adjustment but were not predictive of the children’s self-ratings.Conclusions The results suggest that maternal factors may not explain the variability in children’s adjustment to chronic illness, perhaps especially within the age range studied here. Practical implications of the data are also discussed. In particular, a systemic approach to paediatric liaison by psychologists is emphasized.

    The bombardment of silica by low-energy electrons induces compaction and an increase in refractive index. We have used this phenomenon, combined with photolithographic processing, to make channel waveguides in silica, most of which are monomode, with a depth of 2–7 μm, and peak index variation between 10−3 and 10−2. Channel waveguide widths down to 2 μm have been fabricated on fused silica plates and on silica layers grown on silicon by plasma-enhanced chemical vapor deposition (PECVD) methods. The lowest measured linear losses at 633 nm were 0.3 db/cm in PECVD silica and 1.9 db/cm in the fused silica plates, although much lower losses are anticipated in the latter material. Using this method we have fabricated 3 dB directional couplers in both materials.

    Abstract It is shown that buried channel waveguides may be formed from a bilayer planar guide (made by plasma-enhanced chemical vapour deposition of silicon oxynitride) by bombardment with low-energy electrons. This results in an expansion of the material, and hence a decrease in the refractive index. Consequently, irradiation around a narrow stripe will induce lateral confinement. Data are presented for the electron-induced refractive index and volume changes, together with preliminary results for single-mode channel guides operating at 1.52μm wavelength.

    Abstract .A sedation regimen using sequential oral trinepazine, intravenous Pethco (pethidine, chlorpromazine and promethazine) and diazemuls was evaluated in children having native kidney (n = 17) and transplant kidney (n = 17) biopsies. Biopsy was successful in all cases, with no serious side effects. A self-reported scale of memory recall and pain perception showed the optimal time for biopsy to be between 30 and 90 min after the intravenous Pethco. The child’s level of distress was measured by a self-reported scale, a parent-reported scale and an observational scale for doctors and nurses; 45% of children rated themselves highly distressed prior to the procedure, their parents being the best assessors of this distress. Younger children and those undergoing native kidney biopsy had less understanding of the procedure. Children’s worries could be clearly categorised into procedural and outcome issues: those undergoing transplant biopsy were more worried about outcome, whereas those undergoing native kidney biopsy were more worried about the procedure.
