    Summary. An integrated, automatic electrical stimulator has been developed to treat urinary incontinence. A group of 121 women with either stress-, motor urge- or mixed incontinence was selected for treatment, but 23 discontinued the treatment within 3 months and were excluded from the study. In the study group of 98 women, anal stimulation was applied for an average of 9 months (range 3–36 months). Between 9 and 36 months after the treatment was completed, 91% were either continent or had improved significantly, but in 9% the stimulation had no effect. Significant improvement occurred on clinical and urodynamic testing. There were no serious side-effects. Intra-anal electrostimulation therapy provides an alternative to surgery in patients with stress incontinence, and could be the first choice of treatment in women with combined stress and motor urge incontinence.

    Zusammenfassung Es wird über 3 überlebte und 4 tödlich verlaufene akzidentelle Intoxikationen, sechs mit Methadon, eine mit Dihydrocodein, berichtet. Die Intoxikationen betrafen nicht die Substituierten selbst, sondern deren Familienangehörige, Bekannte oder Mit-Patienten. In allen Fällen wurden die Substitutionsmittel weitgehend ungesichert und teilweise vermutlich auch unbeschriftet stehengelassen. Den Substituierten selbst war dabei die Gefahr, die von ihrem Substitutionsmittel ausging, offenbar nicht ausreichend bewußt. Diese Fälle zeigen deutlich, daß qualitätssichernde Maßnahmen bei der immer großzügigeren Substitutionspraxis, insbesondere in Hinblick auf „take-home“-Dosen, notwendig sind.

    Abstract A total of seven cases of accidental ingestion of methadone or dihydrocodeine by four children and three adults are reported of which four were fatal. In each case, someone in the environment was taking methadone or dihydrocodeine as a substitute drug for heroin addiction who obviously did not realize the dangers of methadone for non-addicts. Possible preventive measures are the usage of child-proof containers with adequate labels for take-home medications. Furthermore substituted addicts have to be thoroughly indoctrinated concerning the toxicity and hazards of methadone.

    Abstract A 47-year-old woman unwittingly ingested an unknown substance together with her breakfast coffee. She suffered effects such as strong headache, generalized cyanosis, and a burning sensation of the lips and collapsed some minutes later. After admission into hospital a methemoglobin level of 35% was determined in the blood. Treatment by administration of tolonium chloride (toluidine blue) resulted in complete recovery of the patient. The toxic agent was identified as aniline by GC with mass selective detection after organic solvent extraction and 11 h after ingestion the plasma aniline level was 0.13 mg/l. Acetanilide (0.79 mg/ml) and acetaminophen (2.3 mg/ml) were identified in plasma as metabolites of aniline. It was assumed that a high metabolic capacity for acetylation protected the victim from more severe reactions. Her husband confessed later that he had tried to poison her.

    Abstract Human papillomavirus (HPV)16-specific nucleic acid sequences were analysed in separate biopsies taken from a patient with a poorly differentiated squamous cell carcinoma of the uterine cervix. Biopsies were obtained from histopathologically normal epithelium adjacent to the carcinomatous epithelium, the primary carcinoma and a metastatic lymph node. Signals characterizing viral DNA and oncogene transcription were obviously differentiation dependent as shown by in situ hybridization of viral nucleic acids and immunofluorescence of epithelial differentiation specific proteins. In histologically normal parts of the epithelium viral DNA was amplified at the transition from basal to maturing cells, whereas E6/E7 genes were actively transcribed mainly in maturing epithelial cells following the basal cell layer. Some of the cells in the primary carcinoma and in the metastatic lymph node expressed involucrin at increased levels. Signals for viral DNA and HPV16-specific E6/E7 transcripts decreased in intensity during differentiation in an inverse relationship to the observed involucrin increase in those cells. The absence of Ki67 in cells expressing large amounts of involucrin as revealed by immunostaining, support the inverse correlation between differentiation of cancer cells, HPV16 replication and E6/E7 transcription. The changes in cytokine expression may indicate an HPV16 associated disruption of normal cytokine expression pattern in the carcinoma.

    Summary A 73-year-old woman was diagnosed with seropositive destructive rheumatoid arthritis in 1981. She was treated with cortisone, chloroquine, and cyclophosphamide (Sendoxan) in 1982 and 1984 and contracted severe neutropenia. After that she only received cortisone. During 1991, again low neutrophilic counts were registered, especially granulocytopenia. At first, B-cell lymphoma was suspected, but later Felty's syndrome was established. The patient was treated with high-dose cortisone with some success and had a few minor septic episodes. In May 1992 she contracted a traumatic wound on the back of the lower leg. Conservative treatment resulted in a worsening of the condition and an increased wound area, most likely related to the neutropenic condition. In mid July the patient was hospitalized. Bacterial isolates yielded mixed gramnegative enteric bacteria from the wound. Parenteral antibiotic treatment was started, followed by oral drugs. rhG-CSF (filgrastim) was given subcutaneously once a day, starting 3 days after admission. This resulted in increased numbers of peripheral granulocytes. The ulcer started to heal and by mid August the patient received a transplant with autologous skin grafting. In mid September the wound was completely healed. It is concluded that the combination of antibiotics, skin transplantion, and G-CSF was necessary for the successful result. Actually, the bacterial growth did not call for antibiotics, but it was considered necessary to cover for staphylococci. No worsening of the underlying arthritis was observed.

    Abstract Trend analyses based on WHO statistics for average life expectancy, age-standardized cardiovascular (CVD) morbidity and mortality show significant differences between the former German Democratic Republic (GDR) and the former Federal Republic of Germany (FRG). To investigate whether this is due to a different prevalence of cardiovascular risk factors, the Dresden Cardiovascular Risk and Nutrition (DRECAN) study was conducted using the complete methodology of the Prospective Cardiovascular Münster (PROCAM) study, i.e., the same methods and strict quality controls, with an exchange of specimens between both laboratories. The results were compared with those of an adjusted subpopulation of the PROCAM study. Even before unification there were only small differences in lipoprotein profiles between West and East Germany, 10 months after unification these differences were minimal. The survey does not sufficiently explain the differences in CHD morbidity and mortality between Western and Eastern Germany. Further analyses of the nutritional aspects will show whether the change, in available foodstuffs after unification has led to substantially changed nutritional habits, and whether this might explain some of the results.
