    Description:
    Participatory Health Research (PHR) builds on a long and diverse history drawn from a range of disciplines, each of which have informed and developed their own understanding of what participatory research practice is. However, just as with qualitative methods ten to fifteen years ago, there is an increasing interest in setting out the fundamental principles that should drive the assessment of quality within PHR. While those using participatory research methods in health have often called for a review of quality criteria, to date there has not been anything published that undertakes the same substantive systematic attempt as found in qualitative research and evaluation. This paper starts the process of agenda setting on quality criteria by describing some key notions/issues that are core to PHR and relevant for developing steps for action, drawing together some key literature. The paper arose from the work of the International Collaboration on Participatory Health Research (ICHPR). Through consolidating existing knowledge and reaching agreement on common terminology and principles, the ICHPR seeks to strengthen the role of PHR in intervention design and decision-making on health issues. This includes developing guidelines for conducting and evaluating PHR, describing which forms of theory and evidence are produced by this approach, and finding a means for conducting systematic reviews of the PHR literature in order to contribute to the body of international knowledge on community health.     Die Partizipative Gesundheitsforschung (PG) entstand über einen längeren Zeitraum aus einer Vielfalt an Disziplinen, die zum Teil unterschiedliche Auffassungen von Partizipativer Forschung vertreten. Wie bei den qualitativen Forschungsansätzen vor ca. fünfzehn Jahren befassen sich immer mehr partizipativ forschende Wissenschaftler/innen mit der Ausformulierung von Gütekriterien für diese Form von Forschung. Mehrere Autor/inn/en haben einen Bedarf an einer systematischen Betrachtung der bereits formulierten Gütekriterien festgestellt, jedoch gibt es bis heute noch keinen Beitrag, der mit den Abhandlungen zu den Gütekriterien der qualitativen Forschung vergleichbar wäre. Dieses Manuskript stellt einen Versuch dar, aufgrund einschlägiger Literatur einen konzeptuellen Rahmen für die Diskussion über Gütekriterien auf dem Gebiet der PG aufzustellen. Es ist ein Ergebnis der Arbeit der International Collaboration on Participatory Health Research (ICHPR). Die ICHPR hat als Ziel, internationale Erfahrungen und Wissensbestände zum Thema Partizipative Gesundheitsforschung zusammenzuführen und einen Konsens über Definitionen und Kernprinzipien zu erreichen, um eine breitere Anerkennung und Anwendung dieses relativ neuen Forschungsansatzes zu fördern. Dies beinhaltet u. a. zu klären, welche Formen von Theorie und Evidenz durch PG generiert werden und nach welcher Vorgehensweise die Ergebnisse aus lokal situierten, partizipativ angelegten Studien systematisch zusammengetragen werden können, um einen Betrag zur internationalen Debatte über die sozialen Determinanten von Gesundheit zu leisten.     Der Regierungswandel in Georgien nach der 'samtenen Revolution' von 2003 führte zu grundlegenden Reformen des Bildungssystems und insbesondere auch zu einer neuen Sprachen- und Bildungspolitik. Im vorliegenden Beitrag werden die Implikationen dieser Entwicklungen für die nationalen Minderheitengruppen in Georgien untersucht. Zu Beginn wird ein Überblick über das allgemeinbildende Schulwesen gegeben und die Unterrichtung der georgischen Sprache in nicht-georgischsprachigen Schulen, die nationalen Curricula, der Unterricht für ethnische Minderheiten, die Vorschulerziehung sowie das Management von Schulen beschrieben. Im Anschluss daran werden die Institutionen der Hochschulbildung dargestellt und Fragen des Hochschulzugangs für Minderheitengruppen erörtert. Darüber hinaus werden verschiedene Förderungsprogramme und Initiativen des Ministeriums für Bildung und Wissenschaft aus den Jahren 2005 bis 2008 vorgestellt und Empfehlungen für weitere Reformen des Bildungswesens gegeben, die sich z. B. auf bilinguale und multilinguale Erziehungsmodelle in der Vorschule und der Sekundarstufe beziehen. (ICI)

    Theoretical Sampling ist als Methode der Fallauswahl ein in der qualitativen Forschung etablierter Gegenentwurf zur repräsentativen Zufallsstichprobe. Im Zentrum steht die Verzahnung des Auswertungsprozesses mit der Auswahl der zu untersuchenden Fälle beziehungsweise Untersuchungseinheiten. In diesem Beitrag diskutieren wir vorliegende Verfahrensvorschläge, identifizieren Schwächen in der Reflexion und Darstellung bei der Auswahl forschungsrelevanter Untersuchungseinheiten und schlagen eine an das Memoing angelehnte Dokumentationsform vor. Zur Illustration dient ein Forschungsprojekt, anhand dessen Wege zur Präzisierung und Systematisierung der Fallauswahl mithilfe sogenannter Verlaufskarten aufgezeigt werden sollen.
    Theoretical sampling is an alternative to statistical sampling in social science that is compatible with genuinely qualitative research. It focuses on integrating the process of analyzing data with the process of selecting the cases or units to be examined. Using preliminary findings, theoretical sampling guides researchers toward the most relevant data for advancing their research project. In this paper, we discuss existing proposals of how theoretical sampling can and should be implemented, identify deterants to research that often emerge from shortcomings in reflecting on and presenting how data are selected, and propose a form of documenting sampling decisions that expands upon established memoing practices. A research project conducted in the University Quarter of Augsburg, Germany is used to demonstrate ways of clarifying and systematizing case selection using so-called progression cards.

    The term 'political will' is often conveniently used to explain the success or failure of any policy or programme. It has emerged as the "sine qua non of policy success which is never defined except by its absence" (Hammergren, 1998, p. 12). Therefore, a structured examination of the term is necessary to analyse social policy and programming. The Mahatma Gandhi National Rural Employment Guarantee Act (MGNREGA), enacted by India's United Progressive Alliance government in 2005, offers a compelling case to examine the role of ‘political will’ in the formulation of a major programme. The evolution of the MGNREGA, which has been described by the World Bank as the largest antipoverty state-run employment-generation scheme anywhere in the world (Dutta, Murgai, Ravallion, & van de Walle, 2014), depended significantly on leadership and political commitment in the legislature and the executive, as well as their coordination and substantive engagement with civil society, represented through non-governmental organisations and activists. We explore the complex power relations between the diverse range of actors involved in the MGNREGA, and gauge the role of leadership and political will in the formulation of the MGNREGA, as carried out by the United Progressive Alliance government between 2005 and 2014, in contrast to the manner in which it was reframed and retained by its successor dispensation, the National Democratic Alliance from 2014 onwards. We then examine the MGNREGA, utilising a framework which expands our understanding of political will as being contingent upon leadership at the individual, collective and societal levels (Hudson, Mcloughlin, Roche, & Marquette, 2018), thereby providing us with greater explanatory power.

    We introduce this thematic issue by exploring the role of leadership in social and political change. In current times, the importance of leadership and choice has proved as important as ever. Leadership is often the critical variable separating success or failure, legitimacy and sustainability or collapse. This thematic issue explores a range of in-depth case studies across the Asia-Pacific region that help illustrate the critical elements of leadership. Collectively they demonstrate that leadership is best understood as a collective process involving motivated agents overcoming barriers to cooperation to form coalitions that have enough power, legitimacy and influence to transform institutions. Five themes emerge from the thematic issue as a whole: leadership is political; the centrality of gender relations; the need for a more critical localism; scalar politics; and the importance of understanding informal processes of leadership and social change.

    Background: Describing the relationship between socioeconomic inequalities and cancer survival is important but methodologically challenging. We propose guidelines for addressing these challenges and illustrate their implementation on French population-based data. Methods: We analyzed 17 cancers. Socioeconomic deprivation was measured by an ecological measure, the European Deprivation Index (EDI). The Excess Mortality Hazard (EMH), ie, the mortality hazard among cancer patients after accounting for other causes of death, was modeled using a flexible parametric model, allowing for nonlinear and/or time-dependent association between the EDI and the EMH. The model included a cluster-specific random effect to deal with the hierarchical structure of the data. Results: We reported the conventional age-standardized net survival (ASNS) and described the changes of the EMH over the time since diagnosis at different levels of deprivation. We illustrated nonlinear and/or time-dependent associations between the EDI and the EMH by plotting the excess hazard ratio according to EDI values at different times after diagnosis. The median excess hazard ratio quantified the general contextual effect. Lip–oral cavity–pharynx cancer in men showed the widest deprivation gap, with 5-year ASNS at 41% and 29% for deprivation quintiles 1 and 5, respectively, and we found a nonlinear association between the EDI and the EMH. The EDI accounted for a substantial part of the general contextual effect on the EMH. The association between the EDI and the EMH was time dependent in stomach and pancreas cancers in men and in cervix cancer. Conclusion: The methodological guidelines proved efficient in describing the way socioeconomic inequalities influence cancer survival. Their use would allow comparisons between different health care systems.

    There is a growing recognition of the effectiveness of locally led processes of social change and development. However, most of the case studies that have been discussed in the literature are focused on programs run by international development agencies. This article examines three locally led processes of change in the Pacific. These include the Simbo for Change Initiative in the Solomon Islands, the Voice in Papua New Guinea and a regional process led by the Green Growth Coalition. We explore how local understandings of leadership, preferences for informal ways of working, holistic ways of thinking, the importance placed upon maintaining good relationships and collective deliberation fundamentally shaped each of the cases. We note how these preferences and ways of working are often seen, or felt, to be at odds with western modes of thought and the practice of development agencies. Finally, we conclude by exploring how these initiatives were supported by external agencies, and suggest further research of this type might provide benchmarks by which Pacific citizens can hold their governments and development agencies to account.

    ROCHE, F., et al.: Arrhythmic Risk Stratification After Myocardial Infarction Using Ambulatory Electrocardiography Signal Averaging. Ambulatory ECG had been proposed to examine the amplified high resolution signal-averaged electrocardiogram (SAECG). Clinical investigations are required to confirm the predictive value of such a high resolution technique in arrhythmic risk stratification. The prognostic value of ambulatory Holter SAECG was evaluated in 108 postinfarction patients for the purpose of predicting the occurrence of serious arrhythmic (SARR) events (sudden cardiac death [SCD], VT, or VF) in comparison with classical real-time SAECG. During the 42 ± 8 months of follow-up, the sudden cardiac death mortality was 4.6% (five deaths), six (5.6%) patients had VT, and one (0.9%) VF. QRSd was found to be the most predictive parameter using ROC curves analysis for SAAR+ outcome (W = 0.833 and W = 0.803 for 25–250 Hz and 40–250 Hz filters, respectively) followed by RMS (W = 0.766 and W = 0.721) and LAS (W = 0.759, W = 0.709) (all P 〈 0.01). Abnormal Holter SAECG for 25 and 40-Hz LP filter were significant predictors of SARR+ by log-rank test (P 〈 0.01, P 〈 0.05, respectively). This study confirms that valuable prognostic information can be obtained from the ambulatory high resolution ECG technique and that Holter SAECG may predict arrhythmic risk in a postinfarction population.

    Summary In a complex animal model in sheep, polydioxanone (PDS®) and polylactic membranes were used for the reconstruction of large orbital-wall defects. In a long-term experiment over 1 year, polylactic implants alone showed the best performance as compared with combinations involving autogenous bone grafts and titanium miniplate fixation. As soon as these polylactic implants are approved for human surgery, they will be used to solve the still challenging problem of anatomical reconstruction of large comminuted fractures of more than one orbital wall.

    Zusammenfassung In einem experimentellen Modell wurden biodegradierbare Membranen aus Polydioxanon (PDS ® ) resp. Polylaktat zur Rekonstruktion großer Orbitawanddefekte bei Schafen verwendet. Im Langzeitversuch über ein Jahr zeigten diejenigen Rekonstruktionen die besten Resultate, bei welchen ausschließlich biodegradierbare Implantate aus Polylaktat zur Anwendung kamen. Bei gleichzeitiger Verwendung von autologen Knochentransplantaten und Titan-Miniplatten-Osteosynthesen war die anatomische Form der rekonstruierten Orbita weniger zuverlässig gewährleistet. Es darf erwartet werden, dass die klinische Zulassung der Polylaktat-Implantate die chirurgische Wiederherstellung ausgedehnter Trümmerfrakturen mehrerer Orbitawände wesentlich erleichtern wird und zu vorhersehbaren, anatomischen Rekonstruktionen führt.
