    Abstract: In population-based studies of osteoporosis, ascertainment of fractures is typically based on self-report, with subsequent verification by medical records. The aim of this analysis was to assess the validity of self-report of incident nonspine fractures using a postal questionnaire. The degree of overreporting of fracture (false positives) was assessed by comparing self-reports of new fracture from respondents in the multicenter European Prospective Osteoporosis Study with data from other sources including radiographs and medical records. In the analysis, 563 subjects reported nonspine fractures. Verification of the presence of fracture was possible in 510 subjects. Of these, fractures were not confirmed in 11% (false positives). The percentage of false positives was greater in men than in women (15% vs 9%, p=0.04), and less for fractures of the distal forearm and hip than for fractures at other sites. In a separate study, the degree of underreporting (false negatives) was assessed by follow-up of 251 individuals with confirmed fracture ascertained from the records of fracture clinics in three European centers (Lubeck, Oviedo, Warsaw). Questionnaire responses were received from 174 (69%) subjects. Of these, 12 (7%) did not recall sustaining a fracture (false negatives). The percentage of false negatives was lower for hip and distal forearm fractures with only 3 of 90 (3%) such fractures not recalled. Using the combined data from both studies, of those who reported a ‘date’ of fracture on the questionnaire, 91% of subjects were correct to within 1 month of the actual date of the fracture. A postal questionnaire is a relatively simple and accurate method for obtaining information about the occurrence of hip and distal forearm fractures, including their timing. Accuracy of ascertainment of fractures at other sites is less good and where possible self-reported fractures at such sites should be verified from other sources.

    Summary Within the European vertebral osteoporosis study four stratified systematic samples of German residents aged 50–79 in Lübeck, Heidelberg, Jena and East Berlin have been drawn. Overall 4628 subjects were included, of whom 4385 were contactable and 3106 (71%) responded to the postal questionnaire, enquiring about rheumatic complaints “today” 2348 (54%) followed an invitation to a medical examination in which a specific gynaecological history was taken The prevalence of current back-, neck-and joint-pain is consistently higher in females than in males in all age groups. Females show a peak prevalence in the age group 55–64 104/1134 women were pre-or perimenopausal 1030 women had not had their period for at least 13 months and were classified as “postmenopausal”. Neither the menopausal status nor the duration of the postmenopause were statistically significantly associated with the frequency of the three rheumatic complaints.

    Résumé Dans le cadre d'une étude européenne sur l'ostéopòrose de la colonne vertébrále 4628 citoyens allemands agés entre 50–79 ans, résidant à Lubeck, Heidelberg, Jena et Berlin-Est ont été sélectionnés pour répondre à un questionnaire portant entre autre sur les symptomes «actuels» rhumatismaux. Parmi ces 4628 sujets, 4385 ont pu être contactés et 3106 (soit 71%) ont répondu aux questionnaires expédieés par voie postale. 2348 personnes (54%) ont accepté l'invitation à un examen médical permettant une anamnese gynécologique spécifique. La prévalence des douleurs dans le dos, la nuque et les articulations est systématiquement plus élevée chez les femmes indépendamment de la catégorie d'âge. Un maximum dans l'intensité des douleurs est observé chez les femmes entre 55 et 64 ans Parmis 1134 femmes 104 étaient pré-ou périménopausales. 1030 participants n'avaient plus de menstruation depuis au moins 13 mois, et ont étê classées dans le groupe «postménopausal». La relation entre l'état de ménopause, la durée de la postménopause et la fréquence des 3 symptomes rhumatismaux n'a pas pu être prouvée dans cette etude.

    Zusammenfassung Im Rahmen einer europäischen Studie zur vertebralen. Osteoporose wurden 1991/92 4628 deutsche Einwohner aus Lübeck, Heidelberg, Jena und Ostberlin im Alter zwischen 50 und 79 Jahren postalisch u. a. zu rheumatischen Beschwerden “heute” befragt (geschichtete systematische Stichprobe). Von 4385 erreichbaren Personen schickten 3106 (71%) einen auswertbaren Fragebogen zurück 2348 (54%) folgten der Einlàdung zu einem ärztlichen Interview, in dem auch eine gynäkologische Anamnese erhoben wurde. Die Prävalenz von aktuellen Rückenschmerzen, Nackenschmerzen und Gelenkschmerzen ist bei Frauen konsistent in allen Altersgruppen (50–54, ..., 75–79 Jahre) höher als bei Männern. Bei den Teilnehmerinnen zeigt sich durchgängig ein Beschwerdenmaximum in dér Altersgruppe 55–64 Jahre. Von 1134 Frauen wurden 104 als prä/perimenopausal eingestuft. 1030 hatten seit wenigstens 13 Monaten keine Periode mehr erlebt und galten als postmenopausal. Der Menopausenstatus und die Dauer der Postmenopause wiesen keinen statistisch zu sichernden Zusammenhang mit der Häufigkeit der drei erfragten rheumatischen Beschwerden auf.
