Nevertheless, a simple block model which does not include the existence of such an interphase is able to reproduce not only the dependence of the loss tangent and storage modulus with the fiber content, but also the shift of the α-relaxation and the presence of the new α′ peak in the composites.Additional Material: 12 Ill.Type of Medium: Electronic ResourceURL: -

However it is absent in the vast mayority of other T-cell malignancies and B-cell lineage lymphomas irrespective on primary tumor site.Methods: Expression of CLA was investigated on six cases of mantle cell lymphoma (MCL) which differed in their histopathological subtype (typical or blastic) and their tendency to infiltrate skin and/or central nervous system (CNS).Results: CLA immunostaining on neoplastic cells was only observed in a 61-year-old female suffering from a lymphoblastoid MCL which clinically presented with specific skin lesions and further developped CNS disease. In this patient, coexpression of CLA with MCL markers (CD20 and CD5) was confirmed by conventional immunohistochemistry and double immunofluorescence studies.Conclusions: To our knowledge, CLA immunoreactivity on B-cell lymphomas has not beeen previously reported. The expression of this skin-related adhesion molecule on malignant MCL cells could explain the clinical behavior of our case which presented and relapsed with cutaneous lesions. However, CLA seems not to be a MCL marker nor a CNS-related adhesion molecule. The authors review the clinical and histopathological characteristics of MCL-specific skin lesions and their diagnostic clues based on cell morphology, immunohistochemistry and molecular investigations.Type of Medium: Electronic ResourceURL: -

We analyse the causal treatment-possibilities of the different localisations. Fifty-five patients with non-unions of the femoral neck (average age 53 years) with or without preexistent femoral head necrosis (44 %) were treated by abduction osteotomy and followed up at regular intervalls. In 15 % of cases a second operation was necessary after an average of 9.3 years, including the early complications. At the latest control 90 % of the patients were satisfied, with an average Harris hip score (HHS) of 91. The survivorship analysis with end point total hip replacement is favourable. In the same period 22 patients were treated with a total hip replacement. The 11 survivors had a clearly worse HHS of 65. The low-risk, technically demanding valgisation osteotomy should be the first step in the treatment of femoral neck non-unions, even in the presence of femoral head necrosis; secondary operations are not compromised. Pertrochanteric non-unions are rare. The pertrochanteric fragment very often heals, leaving a lateral femoral neck non-union which can be treated with valgisation osteotomy. Depending on the type of non-union and the age of the patient, anatomical reduction, medial displacement and valgisation osteotomy can be employed. With the angulated plates of the ASIF (95 °, 120 °, 130 °) 23 of the 24 non-unions could be healed in one operation. Fourteen patients underwent total hip replacement. In the subtrochanteric area mechanical and vascular instability leads to implant failure or fatigue fracture. Rigid compression-reosteosynthesis is the therapy of choice, the 95 ° condylar plate the implant. Twenty-three of our documented 24 subtrochanteric non-unions healed, 4 in the presence of an infection. Multiple operations have been necessary in 2 of the 4 non-unions following a pathological fracture.Notes: Zusammenfassung Abhängig von der Lokalisation werden die kausalen Behandlungsmöglichkeiten von Pseudarthrosen des proximalen Femurs analysiert. Schenkelhalspseudarthrosen: 55 Schenkelhalspseudarthrosen mit oder ohne präoperativer Kopfnekrose (44%) wurden mittels Valgisierungsosteotomie behandelt (Durchschnittsalter 53 J.). Bei 15% wurde nach 9,3 Jahren ein Zweiteingriff notwendig. 90% der Patienten waren subjektiv zufrieden, der durchschnittliche „Harris-Hip-Score" HHS) betrug 91 Punkte. Die Überlebenskurve mit Endpunkt endoprothetische Versorgung ist als sehr positiv zu werten. 22 Schenkelhalspseudarthrosen bei alten Patienten wurden primär mit Totalprothese versorgt. Die 11 Überlebenden hatten einen deutlich schlechteren HHS von 65 Punkten. Die risikoarme, technisch jedoch anspruchsvolle valgisierende Osteotomie sollte der 1. Schritt in der Behandlung der Schenkelhalspseudarthrosen sein, Sekundäreingriffe werden dadurch nicht kompromitiert. Pertrochantäre Pseudarthrosen: Pertrochantäre Pseudarthrosen sind selten. In der Regel heilt der pertrochantäre Anteil, es entsteht eine laterale Schenkelhalspseudarthrose, die mittels Valgisierungsosteotomie ausheilt. Anatomische Reposition, mediale Verschiebung und Valgisierungsosteotomie kommen zur Anwendung. Unter Verwendung von AO-Winkelplatten heilten 23 der 24 Pseudarthrosen in einem Eingriff mit sehr gutem Resultat. Subtrochantäre Pseudarthrosen: Mittels rigider Kompressionsosteosynthese heilen die subtrochantären Pseudarthrosen aus. An HPLC method which achieves the separation of the bulk of PCB (0–6 mL of hexane), mono-ortho PCB (6–8 mL of hexane), non-ortho PCB (8–15 mL of hexane) and PCDD/F (15–50 mL of toluene) on a PYE column (2-(1-pyrenyl) ethyldimethylsilylated silica gel) in a single step without the use of backflush as other authors proposed was developed. The method shows a good accuracy and precision and it is linear in the range studied, e.g from 5.8 to 2420 pg injected in HPLC for TCDD/F, from 28.8 to 12100 pg for PeCDD/F, HxCDD/F, HpCDD/F and from 57.6 to 24200 pg for OCDD/F. To investigate the merits of arthrography after supination trauma of the ankle. Design and patients. In a group of 160 consecutive patients operative exploration was performed in cases where arthrography and/or a delayed physical examination showed positive findings. In all patients arthrography was performed within 48 h after trauma. To determine interobserver agreement, all arthrograms were independently evaluated by two radiologists, both ignorant of the first assessment. Results. The prevalence of an ankle ligament lesion was found to be 76%. Of the 122 patients with a rupture of one or more ankle ligaments, 52% had an isolated anterior talofibular ligament lesion, 3% had an isolated calcaneofibular ligament lesion, and 45% had combined lesions. The site of the lesion was predominantly intraligamentous. In the determination of the presence or absence of an ankle ligament lesion, the specificity and sensitivity of the ankle arthrogram were 71% and 96% respectively. Interobserver agreement on the arthrogram was very good (κ 0.9). In 1% of patients a clear diagnosis was not possible by means of arthrography. Conclusion. Arthrography provides information of high diagnostic quality with excellent interobserver agreement and therefore remains the gold standard for early diagnosis (within 48 h) of a lateral ankle ligament rupture.Type of Medium: Electronic ResourceURL: -

All patients sustained multiple (failed) knee operations or had knees with gross instability. Twenty-eight (29 knees) of the ¶52 patients (54%) in whom the Goretex prosthesis was still in situ were available at a minimum follow-up of 5 years (mean 9 years, range 5–11 years). The mean age at examination was 39 years (range 30–57 years); there were 15 men and 13 women. The results of the procedure were compared with the results of the same patients at a mean follow-up of 3 years. Eighty-one percent of the patients of the whole group complained about pain. This was 78% for the patients with an ACL reconstruction and 75% for the patients with a PCL reconstruction. The Tegner activity score and the Lysholm knee score showed a statistically significant difference over time. The anterior instability pattern improved in only 43% of the knees and the posterior instability in 41% of the knees. The Lachman test showed also a significant difference over time. In all patients X-ray showed an increase in degenerative changes. In conclusion, the PTFE prosthetic ligament in the reconstruction of the ACL and the PCL in the (chronically) unstable knee seems to deteriorate over time.Type of Medium: Electronic ResourceURL: -

Actuellement, l’arthroplastie par résection de la hanche (hanche de Girdlestone) est envisagée principalement dans le contexte d’échec d’interventions chirurgicales de la hanche. Après le traitement de la complication, le chirurgien se voit confrontéà l’alternative de la réimplantation d’une prothèse totale ou de l’acceptation de la situation de Girdlestone. Nous avons suivi deux groupes de patients: l’un vivant avec une hanche de Girdlestone depuis longtemps (32 patients), l’autre dont la hanche de Girdlestone avait été remplacée par une prothèse totale au bout de trois ans en moyenne (16 patients). L’amélioration dans le fonctionnement de la hanche suivant la réimplantation s’est avérée minime et les résultats apparaissaient comparables aux hanches de Girdlestone en bon fonctionnement. Toutefois, le niveau de satisfaction personnelle et de fonctionnement dans la vie quotidienne était plus élevé dans le groupe des réimplantations. Le Harris-hip-score du groupe ayant subi une nouvelle intervention chirurgicale s’élevait à 64, opposé au 58 du groupe Girdlestone. La méthode de Girdlestone resterait une intervention de sauvetage raisonable en cas de chirurgie de la hanche compliquée. Néanmoins, si les indications y sont favorables, la réimplantation d’une prothèse totale de la hanche est à recommander.Notes: Summary. A Girdlestone pseudarthrosis of the hip (resection arthroplasty) is nowadays mainly carried out for failed hip replacements. A decision may have to be made whether to reimplant a new hip prosthesis or to accept the result of a pseudarthrosis. We followed 2 groups of patients: 32 patients had a long standing pseudarthrosis; in the other group of 16 patients, a total hip replacement was reimplanted at an average of 3 years after a pseudarthrosis. The improvement in hip function after the reimplantation was marginal and the results were comparable to a good functioning pseudarthrosis. However, personal satisfaction and the activities of daily living were better in the reimplantation group, and their Harris hip score was 64 compared to 58 in those with a pseudarthrosis. The Girdlestone procedure still seems to be a reasonable salvage operation for some complications following hip surgery, but when there are the correct indications, reimplantation of a total hip prosthesis is recommended.Type of Medium: Electronic ResourceURL: -

Fresh cancellous autografts were rapidly revascularized and invariably induced a complete healing of the defect. Frozen and fresh cancellous allografts were largely resorbed, the latter evoking a strong antigenic response in two of the five cases. Sintered hydroxyapatite granules were largely encapsulated in fibrous tissue, neither stimulating nor inhibiting osseous ingrowth. Sowohl die isolierte Ruptur des Ligamentum fibulotalare anterius als auch die ausgedehntere Läsion des fibularen Bandapparates soll primär genäht werden. Sehr wichtig in der Nachbehandlung ist eine funktionelle Zwischenbehandlung vor Applikation eines Unterschenkelgehgipses. Vorteile der primären Bandnaht verbunden mit funktioneller Zwischenbehandlung sind optimale Gelenksstabilität, frühzeitiges, hervorragendes funktionelles Resultat und damit uneingeschränkte Arbeits- und Sportfähigkeit.Notes: Summary Diagnosis of a sprained ankle has to be refined with stressed X-rays in supination/inversion under local analgetic block of the superficial peroneal nerve. If rupture of one or several fibular collateral ligaments is evident, primary suture is adequate. Previous to the application of a below-knee walking plaster, active mobilisation should restore normal ankle joint function during a few days.
