    Zusammenfassung Von 1962–1975 wurden in der Chirurgischen Universitätsklinik Heidelberg 815 Herzklappenprothesen implantiert. 474 Aortenklappen und 341 Mitralklappen, einschließlich 70 Doppelklappen wurden ersetzt. 22 Patienten (3%) mußten wegen einer Funktionsstörung der Prothese reoperiert werden. 19 Aortenklappen (3 Cross-Jones-, 16 Starr-Edwards-Prothesen) und 3 Mitralklappen (Cross-Jones) mußten wegen ausgerissener Fäden refixiert, wegen Thrombosierung oder defekter Scheiben ausgewechselt werden. Die häufigsten Komplikationen der gestörten Klappenfunktion waren: Herzinsuffizienz, Hämolyse und periphere Embolien. 7 Patienten sind verstorben. 4mal an Herzinsuffizienz, 2mal am Nierenversagen und 1mal an einer Hirnembolie.

    Summary In the Department of Surgery at the University of Heidelberg, 815 prosthetic heart-valve replacements were carried out from 1962 to 1975, 474 of aortic valves and 341 of mitral valves, including 70 double valve replacements. Twenty-two patients (3%) needed second operations due to insufficiency of the prothetic valve. In these 22 patients, 19 aortic valves (3 Cross-Jones and 16 Starr-Edwards prostheses) and 3 mitral valves (Cross-Jones) had to be refixed or changed due to thrombosis or a defect of the valve. The most common clinical complications of these valve malfunctions were: Cardiac insufficiency, hemolysis, and peripheral embolism. There were 7 deaths: 4 due to cardiac insufficiency, 2 due to renal failure and 1 due to brain embolism.

    Abstract Ciclotropium is a recently developed parasympathicolytic agent. Plasma concentration and heart rate increase (the most prominent anticholinergic effects) were measured in 12 healthy subjects before, during and after a 15-min intravenous infusion of 10 mg ciclotropium. The effect was described by using either a linear or a nonlinear (E max) effect model linked to a linear three-compartment kinetic model via an effect compartment. Maximum heart rate increase was 33 (10) beats·min−1, and half-value duration of effect was 41 (9) min. Total plasma clearance was 0.51 (0.13) l·min−1, and mean terminal elimination half-life was 12 (4) h, whereas the equilibration half-lives of drug removal from the effect compartment ranged from 2 to 14 min.

    Summary Chicken embryos were treated with partially paralysing doses ofd-tubocurarine (dtc) from embryonic (E) days 6 to 10. The pattern of innervation of the lateral gastrocnemius (GL) muscle was examined both morphologically and physiologically just before hatching on day E20 or E21. There was a 70% increase in number of surviving motor neurons in the lateral motor column and a 50% increase in the number of myelinated axons in the nerve to GL. The GL muscle was significantly atrophic, with an average weight of 40% of normal. The atrophy was largely due to the reduced size of the muscle fibres. The mean size of the motor units was essentially unchanged or perhaps slightly increased. There was a striking increase in the level of polyneuronal innervation of the muscle fibres, both in terms of number of synaptic sites per fibre and number of axons innervating each site. Spontaneous miniature endplate potentials (mepps) indicated focal innervation of the fibres in the normal muscle. Most fibres in the dtc-treated muscles had mepps of widely varying time courses, and there was no simple relation between amplitude and rise time. Many of the slow mepps were not represented in the endplate potentials evoked by nerve stimulation. The quantal content of the endplate potential (epp) was generally increased in the dtc-treated muscles. The findings are discussed in terms of a retrograde signal from muscle to nerve and its dependence on muscle activity.

    Abstract The Schizosaccharomyces pombe rec16-125 mutation reduces meiotic recombination, delays pre-meiotic DNA synthesis, and reduces the accumulation of some but not other rec gene transcripts. To elucidate the function of the Rec16 global meiotic regulator, we cloned and sequenced rec16. The data revealed that rec16 is identical to rep1, which was previously shown to encode a protein with a zinc-finger motif required for pre-meiotic DNA synthesis. Transcripts of rec16 (rep1) were strongly induced and subsequently degraded during meiosis. In a rec16 (rep1) deletion mutant, meiotic induction of the seven rec genes tested, which appear to be directly involved in meiotic recombination, was significantly reduced or essentially abolished. Deletion of 80% of the gene essentially abolished meiotic recombination, whereas strains deleted for approximately one-quarter of the gene, from either end, retained partial activity. The rec16-125 mutation strongly reduced recombination in the intervals tested on chromosomes I and III, a phenotype characteristic of mutations in rec genes, such as rec7, whose expression requires Rec16 (Rep1). These results show that Rec16 (Rep1) does not have the regional specificity of Rec10. We infer that Rec16 (Rep1) is a transcriptional activator that is required for meiotic replication and recombination because it plays a role in the transcriptional induction of the rec and other meiosis-specific genes.

    Zusammenfassung Es wird über unerwünschte Wirkungen (UW) berichtet, die im Zusammenhang mit der Gabe von Co-trimoxazol aufgetreten sind und von denen die meldenden Ärzte vermuten, daß sie durch dieses Arzneimittel verursacht sind. Die Übersicht beschreibt die Meldungen aus den Jahren 1981 bis 1984 von 260 im Raum Nordbaden und Rheinland-Pfalz niedergelassenen Ärzten sowie die Meldungen der Stationsärzte der Medizinischen Univ.-Klinik Heidelberg aus den Jahren 1980 bis 1983, die sich auf 33 300 stationäre Aufenthalte beziehen.Bereich niedergelassener Ärzte: Von den in 3 1/2 Jahren gemeldeten 3739 UW bezogen sich 180 auf Co-trimoxazol-haltige Präparate. Die „forte“-Do-sierung war 3,3mal so häufig betroffen wie die 80 mg Trimethoprim/400 mg Sulfamethoxazol-haltigen Zubereitungen. Am häufigsten genannt wurden UW an der Haut (n=63; darunter 2mal Quincke-Ödem und 3mal urtikarielle Reaktionen) und am Gastrointestinaltrakt (n=52) sowie Befindlichkeitsstörungen (n=30). Gastrointestinale Störungen scheinen nach der hohen Dosierung häufiger vorzukommen als nach der niedrigen.Stationärer Bereich: Co-trimoxazol-haltige Präparate wurden im Beobachtungszeitraum im Durchschnitt bei 12,6% aller stationären Aufenthalte angewandt. Die Gesamtzahl der auf Co-trimoxazol zurückgeführten UW betrug 255. An gravierenden Reaktionen wurden gemeldet: zwei anaphylaktische Reaktionen, zweimal Thrombocytopenien unter 80 000/mm3 und zwei Lyell-Fälle (von denen einer nicht zweifelsfrei belegbar war). UW wurden öfter nach i.v. als nach oraler Gabe berichtet. Während Übelkeit und Erbrechen häufiger nach oraler Applikation vorkamen, trat Durchfall gehäuft nach i.v. Verabreichung auf. Bezogen auf die stationären Aufenthalte, bei denen Co-trimoxazol verordnet worden war, betrug die Quote an UW nach dem am häufigsten verordneten Co-trimoxazol-haltigen Präparat 3,8% und deckt sich somit mit den in der Literatur zu findenden Angaben. Die mitgeteilten Zahlen beziehen sich auf eine Population im niedergelassenen Bereich, die ca. 170 000 Patientenjahre (bei unbekannter Co-trimoxazol-Exposition), im stationären Bereich ca. 1200 Patientenjahre (hier unter einer Co-trimoxazol-Exposition von ca. 12%) umfaßt. Auf Co-trimoxazol zurückgeführte Todesfälle wurden nicht berichtet.

    Summary This study concerns the incidence of side effects occurring in connection with the prescription of co-trimoxazole which, according to the observations of medical practitioners, were suspected of being drug related. It is based on reports from 260 doctors in the Nordbaden/Rheinlandpfalz area between 1981 and 1984, as well as on those from hospital doctors concerning 33,300 in-patients at the Department of Internal Medicine at the University Hospital of Heidelberg from 1980 to 1983. General practitioners' reports: Of 3,739 side effects reported over a three and a half year period, 180 were related to drugs containing co-trimoxazole. Side effects were 3.3 times as frequent with the “forte” dosage as compared to the 80 mg trimethoprim/400 mg sulfamethoxazole preparation. The most frequently cited unwanted reactions concerned the skin (n=63, of which two were Quincke oedema and three were urticarious reactions) and the gastrointestinal tract (n=52), as well as disturbance of well being (n=30). Gastro-intestinal disturbances appeared to occur more frequently after a higher than after a lower dosage. Clinicians' reports: During the period of observation an average of 12.6% of all in-patients were treated with drugs containing co-trimoxazole. The total number of cases of side effects due to this drug amounted to 255. Serious reactions included: two anaphylactic reactions; two thrombocytopenias below 80,000/mm3; and two cases of Lyell's syndrome (one of which could not be confirmed beyond all doubt). Side effects occurred more often after i.v. than after oral application. Whilst nausea and vomiting were more frequent after oral treatment, diarrhoea occurred to a large extent after i.v. administration. In-patients for whom co-trimoxazole was prescribed suffered on average 3.8% of side effects following treatment with the most frequently prescribed co-trimoxazole preparation. This corresponds to the figures given in the existing literature. These figures cover approximately 170,000 patient years for the general population within the survey (prescription of co-trimoxazole not known) and 1,200 patient years for the in-patients (12% of whom received treatment with co-trimoxazole). No incidence of death caused by co-trimoxazole was reported.

    Vulcanization kinetics for a natural rubber compound were studied by a kinetic approach using the cure meter and DSC methods. A simplified but realistic model reaction scheme was used to simulate induction, curing, and overcure periods continuously. Physically significant parameters of the model were extracted from isothermal experimental data using a cure meter. The calculation demonstrated a good correspondence with isothermal cure meter data over the temperature range studied. The length of induction time, variation of maximum modulus with temperature, and the reversion phenomena observed from cure curves can be predicted. DSC data were found to be incompatible with the cure meter test, because the complex vulcanization reaction system is multiexothermal and it is difficult to isolate the heat due to crosslinking. Hence, the cure meter technique is suggested for the study of crosslink formation. The kinetic approach provides a way to incorporate vulcanization kinetics into simulation of reactive processing operations. © 1996 John Wiley & Sons, Inc.
