    Summary A female patient of 32 with a primary hyperaldosteronism has been observed; the case demonstrated some clinical and biochemical specialities. Characteristic symptoms were stated: hypertension, hypokalemia, alkalosis, hyperkaluria and hyperaldosteronuria, but there were no serious muscular troubles or nephropathy of potassium depletion. The intensity of mineral disturbances and hyperaldosteronuria was inconstant. Histological examination showed a typical Conn adenoma formed by fasciculata-like cell formations, obviously still growing. Biochemical examination of the tumorous tissue revealed a specific production of aldosterone, a production of cortisol, a great deal of 11-desoxycortisol and androgens and little corticosterone. The preoperative finding of a slightly increased urinary excretion of neutral 17-ketosteroids, dehydroepiandrosterone and androsterone corresponded to the steroid spectrum of the adenoma. Problems resulting from the diagnostic terminology of adrenocortical hyperfunction were discussed.

    Zusammenfassung Es wird über die Beobachtung eines primären Hyperaldosteronismus bei einer 32jährigen Frau berichtet, der einige klinische und biochemische Besonderheiten aufwies. Während die führenden Symptome des Krankheitsbildes: Hochdruck, hypokaliämische Alkalose, Hyperkaliurie und Hyperaldosteronurie eindeutig gegeben waren, fehlten schwerere muskuläre Störungen und kaliopenische Nierenveränderungen. Die Intensität der Elektrolytstoffwechselstörung und der Hyperaldosteronurie schwankte. Histologisch fand sich ein typisches, aus Fasciculata-Strukturen aufgebautes Conn-Adenom, das offenbar noch im Wachstum begriffen war. Bei der biochemischen Untersuchung des Tumorgewebes wurde neben einer adenomspezifischen Aldosteronbildung eine Produktion von Cortisol, großen Mengen von 11-Desoxycortisol, sowie Androgenen und wenig Corticosteron festgestellt. Diesem Steroidspektrum des Adenoms entsprach der präoperative Nachweis einer leicht erhöhten Ausscheidung von neutralen 17-Ketosteroiden, Dehydroepiandrosteron und Androsteron im Harn. Die sich für die diagnostische Terminologie adrenocorticaler Überfunktionszustände ergebenden Fragen werden besprochen.

    Summary We report our experience with hematin in the treatment of 57 patients in porphyric attacks. The ratio of acute intermittent porphyria: variegate porphyria: hereditary coproporphyria was 43:11:3. More than 90% of the patients showed not only a decline in their porphyrin precursors, but also a favorable clinical response. Hematin was well tolerated and should be considered the treatment of choice in porphyric relapse.

    Zusammenfassung Wir berichten über unsere Erfahrung mit Hämatin in der Behandlung von 57 Patienten in einer porphyrischen Attacke. Das Verhältnis von akut intermittierender Porphyrie zu Porphyria variegata zu hereditärer Koproporphyrie betrug 43:11:3. Über 90% der Patienten zeigten nicht nur eine Verminderung ihrer Porphyrin-Präkursoren sondern auch eine klinische Besserung. Hämatin wurde gut vertragen und kann als Behandlung der Wahl bei porphyrischen Attacken angesehen werden.
