    Abstract Anterior resection with colo-anal anastomosis was carried out in 40 patients. The main indications were rectal neoplasia (30 cases) and irradiation proctitis (8 cases). Early complications occurred in 12 patients and three died of cardiopulmonary complications. Fistula formation and stenosis were the most frequent late complications. In 23 patients with adenocarcinoma operated for cure there was one recurrence. The functional results were good, although it often took up to 1 year until satisfactory continence, stool consistency and frequency were obtained. Manometry showed a significant decrease in anal canal length and resting tone postoperatively. The method is a practicable alternative to total rectal excision.

    Abstract A description is given of a means of implementing a parallel version of the continued fraction integer factoring algorithm (CFRAC) of Morrison and Brillhart on the Massively Parallel Processor. A case study is provided for the factorization of a 60-digit composite factor of 2405 -1, and some further factorizations are also obtained.

    Summary 1. Total production of ♂♂ and ergatomorphs in one brood period of 58 colonies ofHarpagoxenus sublaevis collected in the open during hibernation (experiment a: 25 colonies, brood rearing in the beginning of hibernation, experiment b: 33 colonies, brood rearing in spring time after a compete natural hibernation) was investigated (Tab. Ia and Ib). 2. Sexually active («locksterzelnde») potential queens and sexually inactive «primary workers» can be distinguished among the workerlike ergatomorphs. 3. The sex ratio (♂♂/sexually active ergatomorphs) is near 1:1 (experiment b). 4. The rate of sexually active ergatomorphs (63,3% of all ergatomorphs produced in experiment b) is much higher than the percentage of queens (14,4%) in the female brood of the related free-livingLeptothorax acervorum. Sexually active ergatomorphs remaining unmated may return to their mother colonies as «secondary workers». 5. The production of ♂♂ and ergatomorphs as well as the rate of sexually active ergatomorphs grow with increasing numbers of slaves per colony, but by far not proportionally to it (Tab. II). 6. The production numbers vary from colony to colony in an extraordinarily wide range. Some factors presumably responsible to this variation are discussed.

    Abstract Results of a computer-aided follow-up programme for patients with colorectal cancer are analyzed. Between 1978 and 1987 1293 patients underwent this programme, the drop-out rate was 17%. 299 recurrences in 168 patients were discovered (40% local recurrence, 29% liver metastases and 31% others). Fifty-one per cent of patients with local recurrence and 47% with liver metastases were symptom free. Radical surgery could be performed in 50% of local recurrences and in 26% of liver metastases. The three year survival rate after radical surgery for recurrence was 35% for local recurrences and 33% for liver metastases, the five-year-survival rate 23% and 15%, respectively.

    Abstract The study was designed to select criteria which influence the incidence of local recurrence after radical anterior resection for rectal cancer. Local recurrence developed in 18 patients (20%) out of 90. All patients entered a prospective clinical study for the detection of local recurrence (mean observation time: 50 months). The following criteria were evaluated retrospectively: age, sex, staging, grading, gross appearance of the tumour, lymphatic reaction, invasion of lymph- and blood vessels, perineural invasion, mucus production of the tumour and width of the distal margin of clearance (measurement in cm in the specimen immediately after resection). The incidence of local recurrence (%) depended on Dukes stage (A: 7%, B: 17%, C: 40%;p≤0.03), grading (well differentiated: 5%, average: 20%, poorly differentiated: 55%;p≤0.02), gross appearance (protuberant: 15%, infiltrating: 47%;p≤0.006), lymphatic stroma reaction (yes: 10%, no: 45%;p≤0.006), invasion of veins (yes: 75%, no: 20%;p≤0.0002), perineural invasion (yes: 52%, no: 17%;p≤0.001) and the margin of clearance (〈1 cm: 52%; 1–3 cm: 10%, 〉 3 cm: 15%;p≤0.02 Mantel,p≤0.05 Breslow between 〈1 cm vs 1–3 cm and 〉3 cm, respectively). Local recurrence was not related to age, sex and mucus production of the tumour. Unfavourable morphological criteria may help to define groups with a higher risk of developing local recurrence.     Zusammenfassung Bei Studium der seit 1938 über das Ewing-Sarkom erschienenen Literatur stößt man auf die verschiedensten Angaben bezüglich der 5 Jahres-Überlebensraten. Die Ursache für die unterschiedliche Beurteilung der Prognose des Tumors ist in Diskrepanzen in der histologischen Diagnose zu suchen. Es werden die möglichen Fehlerquellen in der morphologischen Beschreibung des Ewing-Sarkoms — unter besonderer Wertlegung auf den negativen Ausfall der Gitterfaserfärbung und auf den positiven Glykogennachweis — aufgezeigt. Grundlage für die Kritik an zu hohen Überlebensraten ist das Fehlen mancher der für die Erkennung des Ewing-Sarkoms unbedingt erforderlichen histologischen Kriterien des Tumors. Nach Anlegung eines strengen Maßstabes zeigt sich, daß kaum eine Statistik oder Kasuistik alle notwendigen Charakteristika zur Diagnose ihrer Fälle berücksichtigt. Aus diesem Grunde kann zur Zeit noch keine sichere Aussage über die tatsächliche Lebenserwartung beim Ewing-Sarkom gemacht werden. Höchstwahrscheinlich ist die Prognose jedoch noch schlechter, als bisher angenommen worden ist.

    Summary Studying the literature which has been published since 1938 about Ewing's Sarcoma, one may find the most different statements concerning the 5 year survivalrates. The reason for the various judgement of the tumor's prognosis is to be seen in differences in the histological diagnosis. The possible sources for mistakes in the morphological description of Ewings Sarcoma are shown, calling special attention to the negative result of reticular fibre staining and the positive glycogen-test. The basis for the critique of too high survivalrates is the absence of some of those histological criteria which are absolutely necessary for the identification of Ewing's Sarcoma. Basing one's judgement on the severe standard, one sees that hardly one statistic or casuistic has regard to all the characteristics necessary for the diagnosis of their cases. Therefore at present no precise statement can be made about the actual prognosis of Ewing's Sarcoma. But very likely it is even worse than has been generally assumed uptill now.

    Zusammenfassung Die Sphincterfunktion nach coloanaler Anastomose wurde in 27 Fällen durch Kontinenzanamnese, Sphinctermanometrie und Retentionstest untersucht. In einem Beobachtungszeitraum von 3 bis 12 Monaten nach dem Colostomieverschluß stieg der Anteil von Patienten mit vollständiger Kontinenz von 50 auf 80%, von jenen mit fester Stuhlkonsistenz von 30 auf 90% und von jenen mit weniger als 5 Defäkationen/Tag von 30 auf 80%. Die Operation bewirkte eine signifikante Verkürzung des Analkanals von 4,0 ± 0,3 auf 1,7 ± 0,2 cm (p 〈 0,001) und eine signifikante Reduktion des Ruhedrucks von 85,5 ± 7,9 auf 30,3 ± 6,1 cm H2O (p 〈 0,001). Der Kontraktionsdruck wurde nicht wesentlich beeinträchtigt, der rectoanale Reflex ging temporär verloren. Wesentliche Voraussetzung zur Indikationsstellung der KAA ist eine durch präoperative Sphincterdiagnostik objektivierte normale Funktion des analen Schließmuskelapparates.

    Summary The effect of coloanal anastomosis (CAA) on the function of the anal sphincters was assessed in 27 cases by interview, sphincter manometry and a retention test. During an observation period of 3–12 months after closure of colostomy, the percentage of patients with perfect continence increased from 50 to 80%, of those with solid stool consistency from 30 to 90% and of those with solid stool consisteny from 30 to 90% and of those with less than 5 b.o./day from 30 to 80%. The operation resulted in a significant reduction of the length of the anal canal from 4.0 ± 0.3 to 1.7 ± 0.2 cm (P 〈 0.001) and a significant reduction of the resting tone from 85.5 ± 7.9 to 30.3 ± 6.1 cm H2O (P 〈 0.001). The contraction pressure was hardly influenced, the recto-anal inhibition reflex was temporarily lost. Minute preoperative assessment of a normal sphincter function with anal manometry is an essential prerequisite for the evaluation of the indication for a CAA.

    Zusammenfassung Zwischen 1965 und 1984 wurden 215 Patienten wegen Coloncarcinoms organerweitert operiert. Bei 158 Patienten lag ein Primärtumor, bei 57 Patienten ein Rezidiv vor. 77% der mitentfernten Organe oder Organteile entstammten dem Urogenitaltrakt, entsprechend der Häufigkeit der Lokalisation des Primärtumors im Sigma-Rectumbereich. Bei der histologischen Untersuchung waren 55% der entfernten Organe bzw. bei 71% der Patienten mindestens ein Organ tumorös infiltriert. Die Komplikationsrate lag bei 26%, die Letalität bei 12%. Im Zeitraum 1978–1984 konnte die Letalität auf 7,6% gesenkt werden. Die Prognose war von der Lokalisation des Primärtumors oder der Anzahl der mitentfernten Organe unabhängig. Patienten mit histologisch nachgewiesener Tumorinfiltration der entfernten Organe hatten jedoch eine schlechtere Prognose als Patienten, bei denen histologisch keine Tumorinfiltration nachgewiesen werden konnte. Auf Grund der niedrigen Operationsletalität und der akzeptablen Überlebensraten sollte die lokale Tumorpenetration beim colorectalen Carcinom nicht als Inoperabilitätskriterium angesehen werden.

    Summary Betweeen 1965 and 1984 in 215 patients with colorectal cancer an extended resection was performed due to invasion of adjacent organs. 158 patients were operated on primary and 57 patients on recurrent tumors. Most of the tumors were localized in the sigma and the rectum and therefore about 77% of the resected organs derived from the urogenital tract. 55% of the resected organs proved to be infiltrated histologically by the tumor. In 71% of the patients at least one organ was histologically infiltrated. Postoperative complications were observed in 26%. The mortality was 12%. In the period between 1978 and 1984 the mortality rate was decreased to 7.6%. The prognosis was independent of the localization of the primary tumor and of the number of resected organs. Infiltration of adjacent organs by the tumor was of significant prognostic value. The low perioperative mortality and the reasonable 5 year-overall survival (35%) are encouraging and tumor penetration should not be the reason for inoperability.

    Zusammenfassung Von 1982 bis 1985 wurden 40 coloanale Ànastomosen durchgeführt. Indikationen waren: Tumor 30mal, Strahlenschaden 8mal, postoperative Stenose nach vorderer Resektion 2mal. Bei den Tumoren betrug der durchschnittliche Abstand ab ano 6,4 ± 1,5 cm. Bei einer medianen Beobachtungszeit von 17 Monaten ist das funktionelle Ergebnis für alle Patienten befriedigend, allerdings besteht in den ersten drei Monaten beim Großteil der Patienten eine erhöhte Stuhlfrequenz mit bis zu 10 pro die. Im Laufe eines Jahres kommt es zu einer weitgehenden Normalisierung. Die Manometrie zeigt in den ersten Monaten postoperativ eine Reduktion von Ruheund Kontraktionsdruck sowie eine fehlende Internusrelaxation bei den meisten Patienten.

    Summary From 1982–1985 we performed 40 coloanal anastomoses. Indications were: tumor 30, radiation injury 8, postoperative rectal stricture 2. The distance ab ano was 6.4 ± 1.5 cm for the tumors. The functional result is satisfactory for all patients after a median observation time of 17 months, although the frequency of bowel movements is up to 10/day during the first months postoperatively. Within one year a significant reduction was observed. Manometry showed a reduction of the basal and contraction pressure, internus relaxation was missing in most patients.

Proteolytische AktivitÄten des menschlichen PE (Fibrinplattenmethode; 14,5 ± 8,7 μg Trypsinäquiv./ml) werden durchα2M zu 70 %, durch Aprotinin (Trasylol, Bayer AG) zu 80 % gehemmt. Aprotonin verhindert ferner bei Ratten signifikant den Übertritt von Proteinen (1317-HSA) aus dem Bauchraum in die Zirkulation.

    Summary Peritoneal proteinases during acute pancreatitis deplete active endogenousα2M in peritoneal exudates (p. e.) in patients (2.8 ± 1.2 nMol/g protein vs. 41.3 ± 6.7 in patient's plasma, despite an immunological reactivity of 63 %) and rats (15.4 ± 3.0 nMol/g protein vs. 68.8 + 27.1 in plasma). Proteolytic activities of p. e. on fibrin plates (14.5 ± 8.7 μg trypsin equivalents/ml) could be inhibited byα2M to a rate of 70 %, by aprotonin (Trasylol, Bayer AG) to 80 %. Furthermore, in rats, intraperitoneal aprotinin reduces significantly the movement of proteins (1317-HSA) from the peritoneal space to the circulation.

    Abstract PCB mixtures in surface deposits composed of core sand and ash from an aluminum foundry are enriched (3.6–9.4%) in chlorine percentage relative to deeper horizons. The deeper materials closely match (R2 = 0.902–0.962) Aroclor 1248, the aroclor used on-site and recovered from the floor inside the foundry (R2 = 0.998). This similarity suggests the more deeply buried PCBs have been stable and immobile for a 20–40 yr period. Chlorine enrichment upward is interpreted as the result of volatilization of lower and orthochlorinated congeners from surficial materials during repeated water evaporation. These results suggest surficial deposits contaminated by PCBs may be a significant source of lower ortho-chlorinated PCB congeners to the atmosphere and that buried PCBs can remain unchanged for decades. Comparison with sediments contaminated with Aroclor 1248 and known to have undergone anaerobic microbial dechlorination, indicates that Aroclor 1248 can be transformed to PCB mixtures enriched or depleted in chlorine by environmental processes.
