As a typical sign of stretching of the aortic wall, patients complain of pain during the angioplasty procedure. In the literature, aortic wall rupture and ventricular fibrillation during the procedure are reported. Additional sudden deaths can occur within 40 hours after the procedure. Mortality ranges from 0.1% to 2.5%. By transesophageal echocardiography, monitoring of balloon angioplasty, control of the positioning of the balloon, and control of the results and detection of complications are possible. Intimal as well as medial dissections can be detected with observed healing for intimal but also medial dissections. In order to avoid the patient's discomfort, intraaortic ultrasound will be used in the future, when major methodological improvements are done. Computed tomography demonstrates medial dissections but is not able to visualize intimal dissections. Using computed tomography and magnetic resonance after angioplasty of the coarctation of the aorta, irregularities are described in up to 17% of the patients. For angiography, a low detection rate of medial dissections has to be expected, when not biplane angiographies of the whole thoracic aorta are performend. Medial dissections can be seen, but intimal dissections are missed. In conclusion, a review of the literature demonstrates a high incidence of intimal and medial dissections after angioplasty of the coarctation of the aorta with spontaneous healing in most patients. As is the way with coronary angioplasty, aortic wall ruptures are rare, but stand-by surgery is necessary.Type of Medium: Electronic ResourceURL: -

At λdiss=248 nm we determined the quantum yield of the dominant decay channel leading to O2(1Δg)+O(1D) and the fragment vibrational state distribution. The fragment recoil anisotropy (β=1.25±0.15) was found to be independent of the fragment vibrational states. Between λdiss=275 and 295 nm β assumes a value of 1.6±0.2, which exceeds the value expected for a simple impulsive process. Photofragment yield measurements carried out by PTS between 272 and 286 nm revealed a strong fluctuation of the vibrational state distribution with λdiss. Based on the small but distinct structure superimposed on the broad continuum of the Hartley band and the findings of recent 3D wave packet calculations, we propose this fluctuation, a manifestation of wavelength-dependent partial cross sections, to arise predominantly from an interference effect. The latter occurs between the part of the initially prepared wavepacket which propagates directly into the exit channel and a (minor) part which is temporarily trapped by the motion of the bending and symmetric stretching modes in the excited O3 molecule. © 1995 American Institute of Physics.Type of Medium: Electronic ResourceURL: -

Bei 7 Patienten bestand ein Zustand nach apoplektischem Insult: bei 13 Patienten bestand die Vertebralisabgangsstenose als einzige supraaortische Gefäßveränderung. Pathophysiologisch besteht eine Insuffizienz des Circulus Willisii zur Kompensation der basilären Minderdurchblutung. Die Durchführung einer Elektronsystagmographie ist aus diagnostischen Gründen and zur Ergebniskontrolle wichtig. 92% der operierten Patienten waren postoperativ beschwerdefrei.Notes: Summary Twenty-five patients were operated upon for 28 orificial stenoses of the vertebral artery; the main symptom was vertigo. Seven of them had suffered from stroke.In 13 patients, the vertebral artery stenosis was the only lesion of the supraaortic branches. The cause of cerebrovascular insufficiency is an asymmetric circle of Willisii, which cannot compensate the lowered basilar blood flow. Für 67 Patienten ergab sich hieraus eine Operationsindikation; 39 Kranke unterzogen sich einer supraaortischen vor der peripheren Gefäßrekonstruktion. Die Bedeutung der Diagnose einer cerebro-vasculären Erkrankung bei diesen Patienten ergibt sich aus der Op.-Risikominderung für die periphere Rekonstruktion und aus der Entdeckung asymptomatischer Carotisstenosen sowie deren Beseitigung.Notes: Summary Supra-aortic angiography in 109 patients with occlusive arterial disease of the legs demonstrated significant stenoses of the supra-aortic branches in 63.3 % and ulcerated carotid plaques in 11.9 %. Supra-aortic reconstruction was indicated in 67 patients: 39 were operated on the supra-aortic branches prior to the peripheral vessels. Our histotomographic studies have shown direct radiogenic effects on the dentoenamel junction (atrophy of the odontoblast processes). Whether there are direct effects of radiation on dental enamel is not yet known. Material and Method: Sound teeth (group 1, n = 10) were compared either with enamel specimens after high dose in vitro radiation (500– 2500 Gy; group 2; n = 10) and after experimental enoral in situ irradiation (60 Gy; group 3; n = 20), or with teeth from patients after cancer radiotherapy (36 Gy; group 4; n = 20) and with teeth which had obviously decayed due to cancer radiotherapy (60 Gy; group 5; n = 20). Vestibular enamel surfaces of the teeth were exposed to identical in vitro demineralizations (lactic acid gel; pH = 5.0). Each of the samples was prospectively studied histologically by confocal laser scanning microscopy (CLSM) after 90, 180 and 270 min of acid interaction. Data were interpreted micromorphometrically (width of the demineralized area) and micromorphologically. Results: The width of the demineralized area varies between the two in vivo irradiated groups of specimens (group 4: P 〈 0.01; group 5: P 〈 0.001) and the high dose in vitro irradiated teeth (P 〈 0.01) and is significantly different from the sound controls. The histological image of the demineralization in subsurface areas in the teeth of radiotherapy patients is characterized by a total loss of the prismatic structure (homogeneously, amorphous substance). Conclusions: Direct radiogenic effects at the dentoenamel junction have been described earlier. The additional information offered by this study is that there are significant micromorphometric differences in the demineralizing behaviour of irradiated enamel. Obviously, enamel is less resistant to acid attack after irradiation.Notes: Fragestellung: Das umfangreiche Schrifttum zur Strahlenkaries zeigt den kontroversen Forschungsstand. Eigene Untersuchungen haben direkt radiogene Veränderungen an der Schmelz-Dentin-Grenze histotomographisch nachweisen können (Atrophie der Odontoblastenfortsätze). Offen ist die Frage nach direkten Strahlenfolgen im Schmelz. Material und Methode: Gesunde Zähne (Gr. 1, n = 10) wurden mit Proben nach hochdosierter In-vitro- (500–2500 Gy, Gr. 2, n = 20) und nach experimentell-enoraler In-situ-Bestrahlung (60 Gy, Gr. 3, n = 20) sowie mit Zähnen von Patienten nach präoperativer Strahlentherapie (36 Gy, Gr. 4, n = 20) und strahlenkariösen Zähnen nach tumortherapeutischer Strahlendosis (60 Gy, Gr. 5, n = 20) verglichen. Im direkten Parallelgruppenvergleich erfolgte eine standardisierte In-vitro-Demineralisierung (Milchsäuregel pH 5,0) an den vestibulären Schmelzoberflächen. Jede Probe wurde prospektiv histologisch durch konfokale Laser-Scanning-Mikroskopie (CLSM) nach 90, 180 und 270 min Entmineralisierung untersucht. Die Auswertung erfolgte mikromorphometrisch (Breite des Demineralisationssaums) und mikromorphologisch. Ergebnisse: Die Breite des Demineralisationssaums unterschied sich sowohl bei beiden in vivo bestrahlten Probengruppen (Gr. 4: p 〈 0,01; Gr. 5: p 〈 0,001) als auch bei den hochdosiert in vitro bestrahlten Zähnen (p 〈 0,01) signifikant von den Befunden der gesunden Kontrollen. Das histologische Bild der Demineralisation an den Zahnoberflächen von Radiotherapiepatienten zeigte dabei einen völligen Verlust der Prismenstruktur (homogen amorphe Substanz). Schlußfolgerungen: Direkte Strahlenfolgen an der Schmelz-Dentin-Grenze wurden in früheren Untersuchungen dargestellt. Zusätzlich konnten jetzt signifikante mikromorphometrische Unterschiede im Demineralisierungsverhalten des bestrahlten Schmelzes gezeigt werden. Beyond this it has to be calculated with the possibility of changing of cell kinetics in the therapeutic course against the pretherapeutic values. Regarding the variety of open questions in cell kinetics the accordance with cell kinetic parameter in tumor management seems to be necessary. Tethered cord (89%) and associated malformations (syrinx, lipoma etc.) (33%) were the most important findings. Additional MRI scans of the head (44 patients) revealed numerous further anomalies. Arnold Chiari malformation was found in 76% of the patients (ACM I: 32%, ACM II: 44%). In the ACM II group compression of lower cranial nerves, brain stem, and cerebellum can lead to considerable neurologic symptoms. Therefore in patients with progressive neurologic dysfunction a complete investigation of the whole spine and brain is necessary.
