    Zusammenfassung In einer prospektiven Langzeitanalyse über 3–5 Jahre wurde die Effektivität einer konservativen Behandlung und elektiven Operation bei der Diverticulitis im Stadium II überprüft. Es zeigte sich, daß die konservative Behandlung auf Dauer in 70 % ineffektiv war, währenddessen durch eine elektive Colonresektion in 901/o langfristig eine deutliche Besserung des klinischen Befindens eintrat. Kein Patient starb an den Folgen der Operation. Unsere Ergebnisse sprechen damit für die operative Therapie als Behandlung der Wahl bei der Diverticulitis im Stadium II.

    Summary In a prospective study lasting 3–5 years, we observed the results of conservative therapy and elective surgery in patients with grade II diverticulitis. In 70 % of cases conservative therapy did not improve the clinical situation, whereas the elective operation improved the clinical picture in 90%. No deaths occurred postoperatively. Surgical intervention seems to be better than conservative treatment for grade II diverticulitis.
