    Dendritic spines in the hippocampus are sources of synaptic contact that may be involved in processes of learning and memory [Moser (1999) Cell. Mol. Life Sci., 55, 593–600]. These structures are sensitive to sex differences as females in proestrus possess a greater density than males and females in other stages of the estrous cycle [Woolley et al.. (1990) J. Neurosci., 10, 4035–4039]. Moreover, exposure to an acute stressful event increases spine density in the male hippocampus but decreases spine density in the female hippocampus [Shors et al.. (2001) J. Neurosci., 21, 6292–6297]. Here we demonstrate that antagonism of N-methyl-d-aspartate (NMDA) receptors prevents the increase in spine density as females transition from diestrus 2 to proestrus, when estrogen levels are rising. Antagonism of NMDA receptors during exposure to the stressful event also prevented the changes in spine density in males and females, despite differences in the direction of these effects. Thus, the stress-induced increase in spine density was prevented in the male hippocampus as was the stress-induced decrease in spine density in the female hippocampus. NMDA receptor antagonism during exposure to the stressful event did not alter corticosterone levels or the corticosterone response to stress. These data suggest that both increases and decreases in spine density can be dependent on NMDA receptor activation.

    A retrospective chart review was undertaken to collect and report data on the prevalence and characteristics of pain in persons with terminal-stage AIDS A convenience sample consisting of 50 patient charts at a residential AIDS hospice was utilized Data were collected from the McCaffery Initial Pain Assessment Tool (IPAT) (1983) which was completed at the time of each hospice admission The IPAT is composed of 10 sections that collectively describe the patient's pain experience Demographic and AIDS-related pathology data were also collected Data were analysed using descriptive statistics to examine the incidence and characteristics of pain Pain was experienced by a majority of the subjects in the sample Pain levels for‘present pain,‘worst pain gets,‘best pain gets' and‘acceptable level of pain’are reported Characteristics of pain including location, causal factors and verbal descriptors are reported Implications for nursing practice and suggestions for further research are discussed

    Introduction: Transcoronary ablation of septal hypertrophy (TASH) is safe and effectively reduces the intraventricular gradient in patients with hypertrophic obstructive cardiomyopathy (HOCM). To analyze the potential of anti- and proarrhythmic effects of TASH, we studied the discharge rates of implanted cardioverter defibrillators (ICD) in patients with HOCM who are at a high risk for sudden cardiac death. Methods: ICD and TASH were performed in 15 patients. Indications for ICD-implantation were secondary prevention in nine patients after resuscitation from cardiac arrest with documented ventricular fibrillation (n = 7) or sustained ventricular tachycardia (n = 2) and primary prevention in 6 patients with a family history of sudden deaths, nonsustained ventricular tachycardia, and/or syncope. All the patients had severe symptoms due to HOCM (NYHA functional class = 2.9). Results: During a mean follow-up time of 41 ± 22.7 months following the TASH procedure, 4 patients had episodes of appropriate discharges (8% per year). The discharge rate in the secondary prevention group was 10% per year and 5% in the group with primary prophylactic implants. Three patients died during follow-up (one each of pulmonary embolism, stroke, and sudden death). Conclusion: In conclusion, on the basis of ICD-discharge rates in HOCM-patients at high risk for sudden death, there is no evidence for an unfavorable arrhythmogenic effect of TASH. The efficacy of ICD treatment for the prevention of sudden cardiac death in HOCM could be confirmed, however, mortality is high in this cohort of hypertrophic cardiomyopathy patients.

    Abstract  The detection of dysplastic features of hematopoiesis in de novo acute myeloid leukemia (AML) by light microscopy is defined as AML with trilineage myelodysplasia (AML/TLMD). The prognostic relevance of these dysplastic features for patients with de novo AML remains unclear. In order to evaluate the role of dysplasia in de novo AML, bone marrow aspirates from 69 patients were analyzed prospectively and investigated separately for erythropoiesis, granulopoiesis and megakaryopoiesis by three independent investigators. The overall complete remission (CR) rate was 48.8% and partial remission (PR) or nonresponders consituted 52.2% of the patients investigated. The median overall survival time was 5 months with a disease-free interval of 3.5 months for all patients. Dysgranulopoiesis (DysG) was observed in 30.4%, dysmegakaryopoiesis (DysM) in 50.7%, and dyserythropoiesis (DysE) in 43.5%. Of all patients, 26.0% showed trilineage dysplastic features and were thus classified as AML/TLMD. A significantly worse prognosis (Kaplan-Meyer plot, Student's t–test) was calculated for those patients with detection of only DysG (p=0.002), DysM (p=0.02), DysE (p=0.04) as compared with patients without any dysplastic signs. An unfavorable karyotype was correlated with patients showing DysG (P=0.02) and DysM (P=0.04). For these patients with an unfavorable karyotype, the occurrence of any dysplastic features had no additional prognostic impact. Dysplastic features (DysG, DysM, DysE) seem to be an important prognostic factor in de novo AML correlating with short overall survival. DysG and DysM correlated well with the appearance of unfavorable chromosomal abnormalities. It may be reasonable to assume that patients with dysplastic features should be considered for more aggressive treatment schedules at the time of diagnosis.

    Zusammenfassung Statistische Vergleichsuntersuchungen an 700 Kindern haben ergeben, daß eine eindeutige Korrelation zwischen der allgemeinen Entwicklungsbeschleunigung der Jugend und der Anfälligkeit gegenüber neurotischen Erkrankungen (abnormen Erlebnisreaktionen) besteht. Im Untersuchungsalter zwischen dem sechsten und vierzehnten Lebensjahr handelt es sich dabei neben psychoneurotischen Verhaltungsweisen nervöser Kinder mit Erziehungsschwierigkeiten infolge Angst-, Aggressions-und Schüchternheitsreaktionen um typische “Kinderfehler” wie Bettnässen, Tics, Masturbation, Pavor nocturnus, Sprachstörungen und auch um psychogene Lähmungen. Als pathogenetische Faktoren der Acceleration müssen die Entwicklungsdisharmonie der Persönlichkeit und die parallellaufende, accelerativ bedingte, neuropathische (vegetative) Stigmatisation gelten, die zugleich die kausalen Zwischenglieder zwischen Acceleration und Neurose darstellen. Neben den bisher bekannten Einflüssen der Entwicklungsbeschleunigung auf das Krankheitsgeschehen — einer Vorverlegung des Manifestationsgipfels der Chorea minor und Polyomyelitis sowie einer erhöhten Disposition zu neurozirkulatorisch-dystonen Krankheitsbildern (Hochrein) — muß demnach auch die gesteigerte Anfälligkeit gegenüber Neurosen als für den accelerierten — ganz besonders aber für den überdurchschnittlich accelerierten — Menschen typische und zugleich tief im Biologischen verankerte Erkrankungsdisposition gegenüber widrigen Umwelteinflüssen gelten.
