More recently radical trachelectomy and laparoscopic lymphadenectomy have been introduced to allow preservation of fertility. We present a new approach to fertility-sparing surgery, namely abdominal radical trachelectomy. The technique is similar to a standard radical hysterectomy and lymphadenectomy. In our technique the ovarian vessels are not ligated and, following lymphadenectomy and skeletonisation of the uterine arteries, the cervix, parametrium and vaginal cuff are excised. The residuum of the cervix is then sutured to the vagina and the uterine ateries re-anastomosed.Type of Medium: Electronic ResourceURL: -

ISSN: 1433-7339Keywords: Key words Breast conservation ; Mastectomy ; Quality of lifeSource: Springer Online Journal Archives 1860-2000Topics: MedicineNotes: Abstract Sixty-five patients who underwent modified radical mastectomy were compared with 55 breast conservation patients, all of whom were in stage I, II or III of breast cancer and were under treatment at an oncology clinic in northern Israel. The main findings indicate no differences between the groups in physical, psychosocial, marital or medical interaction, according to the CARE scale. These findings reflect the results of similar comparative studies conducted during the 1980s and 1990s as documented in the literature. ISSN: 1573-7691Keywords: Solidification Microstructure ; finite element method ; moving boundary problemSource: Springer Online Journal Archives 1860-2000Topics: Computer ScienceNotes: Abstract Galerkin finite element methods are presented for calculation of the dynamic transitions between planar and deep two-dimensional cellular interface morphologies in directional solidification of a binary alloy from models that include solute transport, the phase diagram, and the interfacial free energy between melt and crystals. The unknown melt-solid interface shape is accounted for in the finite element formulation by mapping the equations to a fixed domain. Novel nonorthogonal transformations are introduced combining cylindrical and Cartesian coordinate interface representations for approximating the deep cellular interfaces that evolve from a planar solidification front. The algorithm for time integration combines a fully implicit Adams-Moulton algorithm with the Isotherm-Newton method for solving the nonlinear set of differential-algebraic equations that result from the spatial discretization of the moving-boundary problem. The fully implicit scheme is found to be more accurate and efficient than an explicit predictor-corrector algorithm. Sample calculations show the connectivity between families of shapes with resonant spatial wavelengths.Type of Medium: Electronic ResourceURL: -

Weld (1987, 1988a,b) has developed a series of comparative techniques that analyze the effect of perturbations to the parameters of a given qualitative unit model. In this article we demonstrate a system, based on the qualitative process theory of Forbus (1984), which extends comparative analysis in two ways. First, it predicts the effects of changes in the qualitative equations and in parameter values.
