    Background: Atherosclerotic occlusive disease of the vertebral artery (VA) affects a significant proportion of patients with cerebrovascular disease and can cause significant morbidity and mortality despite maximal medical therapy. Careful selection of both symptomatic and asymptomatic patients for intervention based on neurological symptoms, as well as anatomical and hemodynamic considerations, is essential. Methods: To achieve proper results with angioplasty and stenting, standard endovascular techniques are modified for the unique anatomical considerations of the VA. Several technical nuances are described here to aid in the endoluminal revascularization of the VA. Results: Technical success (〈50% residual diameter stenosis) rates ranging from 94 to 98% and low rates of periprocedural complications have been achieved with VA angioplasty and stenting in select series with angiographic follow-up. The long-term success in those series is limited by the high (10–43%) rate of recurrent stenosis. Proper placement of the stent can reduce the risk of periprocedural complications and restenosis. In-stent stenosis appears most commonly during the first 6–12 months after stent placement, but does not appear to correlate with return of neurological symptoms. We believe the high rate of recurrent stenosis makes follow-up angiography essential, especially in the first 12 months after the procedure. Conclusion: Endovascular VA angioplasty with stent placement provides an innovative approach to treat a difficult clinical entity. Evolving therapies, including drug-eluting stents or bioresorbable stents, may prove to provide better long-term results.

    Abstract Long Evans rats were given atenolol doses ranging from 0.27 to 5.4 mg/kg by intraperitoneal injection. Animals were dosed once every 2 hr for a total of five doses. Atenolol concentrations 1 hr after the last dose were measured from simultaneously obtained plasma and cerebrospinal fluid (CSF) samples. CSF concentrations of atenolol were not proportional to plasma concentrations. The ratio of CSF/plasma concentrations was higher (0.33) at lower plasma atenolol levels (〈100 ng/ml) than at the higher atenolol plasma levels (0.05) (P 〈 0.001). The relationship between plasma and cerebrospinal fluid atenolol concentrations was best described by the sum of a Michaelis–Menten and linear function. Animals were also given atenolol doses and then subjected to global cerebral ischemia. The relationship of atenolol concentrations from plasma and CSF in these animals was linear, with a constant partition ratio of 0.02. Together these data show that atenolol partitioning between plasma and CSF is nonlinear and possibly an energy-dependent process.

    Objective  To determine whether choice of colposcopy or six month cytological surveillance would be beneficial to women with mildly abnormal smears when compared with the national policy of six months surveillance in terms of psychological morbidity.Design  A randomised trial based on the Zelen design.Setting  A hospital-based research clinic.Population  Four hundred and seventy-six women who had had a recurrent borderline or mildly dyskaryotic smear on routine cervical screening in primary care.Methods  Women were randomised either to six months cytological surveillance or to make a choice between that or colposcopy and were followed up for 1 year.Main outcome measures  The primary outcome measure was caseness (score ≥4) on the General Health Questionnaire at 12 months follow up. Other measures were the Spielberger State and Trait scores, default rates and cytology/colposcopy outcomes.Results  There was no significant difference between the arms for General Health Questionnaire (GHQ) scores and Spielberger State and Trait at 12 months. There was a significant reduction in psychometric morbidity between baseline and 12 months in both arms. Overall rates of default from the protocol were the same in both arms, but default that led to uncertain ascertainment of cervical pathology was greater in the no-choice arm.Conclusions  This trial indicates that having choice did not impact favourably or harmfully on anxiety or feelings of wellbeing. If a patient is anxious, allowing the patient to choose immediate colposcopy may be preferable because it will improve ascertainment of underlying disease in a group who are more likely to default.

    To date most passively mode-locked erbium-doped fiber laser systems have employed solitons for short pulse generation, and ultrashort high energy pulses have been difficult to achieve. We show that by reducing the system nonlinearity via pulse stretching and employing net positive dispersion cavities, pulses of sub-100-fs duration and (approximately-greater-than)100 pJ energy are readily generated. Data are presented for varying net dispersion in a fiber ring.

    We report an additive-pulse mode-locked (APM) thulium-doped fiber ring laser producing 350–500 fs pulses tunable from 1798 to 1902 nm. The laser operates in the soliton regime, where periodic perturbations cause predictable sidebands and modulation in the optical spectrum. © 1995 American Institute of Physics.
