    Objective  To assess the effects of tibolone on climacteric symptoms, endometrium and serum lipid/lipoproteins in postmenopausal women receiving tamoxifen after surgery for breast cancer.Design  Double-blind, randomised, placebo-controlled, multicentre pilot study.Setting  Hospital outpatient clinic.Sample  Seventy postmenopausal women receiving tamoxifen following surgery for early breast cancer.Methods  Women received 20 mg/day oral tamoxifen plus either 2.5 mg/day oral tibolone or placebo for 12 months.Main outcome measures  Frequency and severity of hot flushes (diary cards); intensity of hot flushes and sweats (Landgren scale); interference of hot flushes and sweats with normal life; frequency and intensity of other climacteric symptoms; endometrial thickness and histology; vaginal bleeding; breast cancer recurrence and serum lipid/lipoproteins.Results  Daily card data showed no change in the daily number of hot flushes with either tibolone or placebo (P= 0.219) after three months. There was a significant reduction in the severity of flushes with tibolone compared with placebo (−0.4 vs 0.2, P= 0.031). The Landgren scale showed a mean change in the number of hot flushes of −0.6 with tibolone and +1.1 with placebo after 12 months (P= 0.022). Endometrial biopsies were normal and vaginal bleeding was similar in both groups. A significant decrease in triglycerides (−23%vs 1.4%) and HDL (−12%vs 19%) was seen with tibolone compared with placebo after 12 months.Conclusions  Tibolone prevented an increase in hot flushes in postmenopausal women given tamoxifen following surgery for breast cancer without untoward effects on the endometrium. Beneficial effects on serum lipid profile were noted.

    In 100 patients of a 5 year prospective study, primary cancer of the liver could be demonstrated in almost 100% using AFP determination in the serum and combined 67 Gallium scintigraphy. By separate evaluation of the AFP results or 67 Gallium scintigraphy an accuracy of only 80% could be demonstrated.

    In a prospective study 4,000 patients of a gastroenterologic and haematologic department were screened for AFP and LP-X. X-ray, endoscopic and scintigraphic examinations concerning liver and bile ducts were performed. Only histologically confirmed results were used for the evaluation of this prospective study. Applying combination of these methods enabled the differential diagnosis of localized liver diseases and increased additional information in cases of parenchymal damage of the liver up to 35%.

    Abstract Objective: The aim of this study is to examine whether a routine preoperative ECG is necessary in children to assess the risks in anaesthesia and surgery. Methods: We examined the records of 3030 children anaesthetized in our hospital in 1990 and compared records of complications with preoperative ECGs. We also evaluated questionnaires on this subject which we had sent to 51 large pediatric hospitals in Germany in 1991. Results: During 3030 anaesthetic procedures there were 93 complications, 67 of which were not of cardiac origin. Of the 26 cardiac complications only 4 needed therapy (AV-Block 2°, fall in blood pressure). Preoperative ECGs were recorded in 180 cases: 152 were normal, 8 showed harmless deviations from normal and 16 were recorded because of known cardiac defects. None of the 180 children developed complications during anaesthesia. Our questionnaire was returned completed carefully by 36 of the 51 hospitals. In most hospitals a preoperative ECG is only recorded when heart disease is known or suspected. Conclusion: Routine preoperative ECGs are unnecessary in children unless there are clinical symptoms of heart disease or heart disease is suspected.

    Zusammenfassung Fragestellung: Es soll geklärt werden, inwieweit ein präoperatives EKG bei Kindern für die Risikoabschätzung vor einer Narkose notwendig ist. Methode: Wir überprüften die Narkoseprotokolle von 3030 Kindern, bei denen im Jahr 1990 in unserer Klinik eine Narkose durchgeführt wurde, und stellten die Komplikationen bei diesen Narkosen den Ergebnissen der präoperativen EKGs gegenüber. Außerdem werteten wir Fragebögen zu diesem Thema aus, die wir 1991 an 51 große Kinderkliniken in Deutschland verschickt haben. Ergebnisse: Bei 3030 Narkosen traten 93 Komplikationen auf, von denen 67 nicht kardial bedingt waren. Von den 26 kardialen Komplikationen waren nur 4 behandlungsbedürftig (AV-Block 2°, Blutdruckabfall, Schock). Von 180 präoperativ geschriebenen EKG waren 152 normal; 8 zeigten harmlose Normvarianten, 16 wurden wegen bekannter Herzfehler durchgeführt. Keines dieser Kinder hatte Komplikationen bei der Narkose. Die Fragebögen wurden von 36 der 51 angeschriebenen Kinderkliniken sorgfältig ausgefüllt zurückgeschickt. In den meisten Kliniken wird nur bei Hinweisen auf eine mögliche oder bestehende Herzerkrankung ein präoperatives EKG geschrieben. Schlußfolgerung: Es gibt bei Kindern keine Indikation für ein präoperatives EKG als Routinemaßnahme. Nur wenn sich aus Anamnese und körperlicher Untersuchung Hinweise auf eine Herzerkrankung ergeben, ist ein EKG und – bei Bedarf auch ein Echokardiogramm – eine sinnvolle Untersuchung.

    Abstract Variations in the length of simple repetitive tandem repeats (microsatellite instability, MIN) between constitutive and tumour DNA, which is characteristic of tumours in patients affected with hereditary nonpolyposis colon cancer (HNPCC), have been found to be very important in the carcinogenesis of a variety of human neoplasms. Recently, MIN has been found in sebaceous and colorectal tumours as well as in keratoacanthomas of Muir-Torre syndrome. In order to elucidate the significance of both MIN and loss of heterozygosity (LOH) in the pathogenesis of sporadic keratoacanthomas, the presence of MIN and LOH at five loci [chromosome 5q21 (D5S346, APC), 9p21 (D9S171, p16), 10pter (D10S89, Mfd28), 11p (D11S904) and 17p12 (D17S520, p53)] was evaluated. MIN was found at only one locus (p53) in 1 of 12 keratoacanthomas and no evidence for the presence of LOH could be detected. Our results suggest that, in contrast to keratoacanthomas associated with Muir-Torre syndrome, neither MIN nor LOH appear to be significant in the induction of sporadic keratoacanthomas.

    Zusammenfassung Zink-Protamin-Glucagon zeigt bei s.c.-Injektion in einer Dosis von 5 mg einen deutlichen, aber protrahierten Anstieg des Blutzuckers um rund 40% innerhalb 1 Std und behält dieses erhöhte Niveau durch mehrere Stunden bei. Die Lipase-Aktivitätssteigerung nach Glucagon ist beim Menschen nur kurzfristig und unbedeutend [rund 7% ige Zunahme der freien Fettsäuren (FFS) 10 min p. inj.], sie wird von einem insulininduzierten Abfall der FFS um rund 50% im Laufe von 6 Std gefolgt. Die Zunahme der Insulinspiegel im Serum ist zwar statistisch signifikant, erreicht aber nicht das Ausmaß der von der intravenösen Verabreichung bekannten Veränderungen. Die hämodynamischen Effekte einer Einzeldosis von 5 mg Zink-Protamin-Glucagon sind nicht eindeutig und lassen lediglich eine nicht signifikante Zunahme von LVdp/dt max um 16,7% nach 15 min erkennen, während alle anderen Parameter im wesentlichen unverändert bleiben. Die mögliche klinische Bedeutung dieser Glucagoneffekte wird diskutiert.

    Summary The subcutaneous injection of 5 mg of zincprotamine-glucagon results in a 40% increase of blood glucose within 1 hour; this elevated level is maintained for several hours. The activation of lipase after the administration of glucagon in patients is insignificant and of short duration (only 7% rise of plasma FFA 10 minutes after injection) and is followed by an insulin-induced fall of plasma FFA by 50% within 6 hours. There is a concomitant, statistically significant rise in serum insulin. Haemodynamic measurements following a single dose of 5 mg of zinc-protamine-glucagon revealed only a statistically non significant 16.7% rise in LVdp/dt max, after 15 minutes, the other parameters remaining essentially unchanged. The possible elinical significance of these glucagon effects is discussed.
