ISSN: 1432-2323Source: Springer Online Journal Archives 1860-2000Topics: MedicineDescription / Table of Contents: Résumé Chez les malades atteints de sténose du tronc coronaire gauche (TCG), les accidents du cathétérisme sont plus fréquents, les résultats du traitement médical sont moins bons, la morbidité et la mortalité postopératoire sont plus élevées que chez les patients présentant d'autres lésions d'artériosclérose coronaire, simples ou multiples. La sténose du TCG se traduit par des manifestations cliniques très variées, allant du patient “asymptomatique” qui n'a que des tests d'effort perturbés jusqu'au malade gravement atteint chez qui la coronarographie et l'opération doivent être faites en urgence. Au Peter Bent Brigham Hospital, entre 1970 et 1977, nous avons opéré 141 malades atteints de sténose du TCG. La mortalité opératoire a été de 10% (5 sur 49) pour les malades en état instable, de 4% (4 sur 92) pour les malades stables, de 6% (9 sur 141) au total. La survie actuarielle jusqu'à 90 mois est meilleure chez les malades stables que chez les instables, chez les patients chez qui une revascularisation complète a pu être réalisée, chez ceux qui avaient comme seule lésion une sténose du TCG. Des études prospectives, des traitements médicaux et chirurgicaux, indiquent que les malades atteints de sténose du TCG doivent être opérés de pontage coronaire.Notes: Abstract Catheterization morbidity, results of medical treatment, and operative morbidity and mortality in patients with left main coronary artery stenosis (LCA) are higher than in patients with other coronary atherosclerotic lesions, occurring in combination or alone. Patients with LCA stenosis have a wide variety of clinical manifestations and range from “asymptomatic” patients with only positive exercise tests to critically ill, unstable patients requiring emergency angiography and operation. Experience with the surgical treatment of LCA stenosis in 141 patients at the Peter Bent Brigham Hospital from 1970–1977 is reviewed. Operative mortality rates were 10% (5/49) in unstable patients, 4% (4/92) in stable patients, and 6.4% (9/141) overall. Actuarial survival up to 90 months was better in stable than unstable patients, in patients who were completely rather than incompletely revascularized, and in patients in whom only LCA stenosis existed and was bypassed. This study describes the course of disease in eight patients with Lyme disease, aged 37–70 years, all of whom were suffering from histologically proven myositis. The Clnical, electrophysiological, and myopathological findings are reported. One patient showed signs and symptoms of myositis of all limbs. In six patients myositis was localized in the vicinity of skin lesions, arthritis or neuropathy caused byBorrelia burgdorferi. In another patient suffering from pronounced muscle weakness of the legs and cardiac arrest, inflammation of the myocardium, the conducting system and skeletal muscles was revealed at autopsy. Muscle biopsy revealed lymphoplasmocellular infiltrates combined with few fibre degenerations in three patients. The lymphoplasmocellular infiltrates were found predominantly in the vicinity of small vessels. Several spirochetes were stained in six of seven muscle biopsy samples by means of the immunogold-silver technique. Culturing ofB. Burgdorferi from the muscle biopsy samples was, however, unsuccessful. Antibiotic treatment succeeded in curing the myositis in four of six patients. In one patients signs and symptoms improved. One patient died from cardiac arrest caused by myocarditis and Guillain-Barré syndrome. The outcome is unknown in one patient. Clinical and myopathological findings indicate that Lyme myositis can be caused either by local spreading ofB. burgdorferi or an unknown antigen or toxin from adjacent tissues or haematogenously.Type of Medium: Electronic ResourceURL: -

Serial measurements of serum parathyroid hormone (PTH), calcium, and phosphate were made. Four patients had an increased serum PTH postoperatively, which was already apparent by the third postoperative day. All patients became normocalcemic. Their hyperparathyroid-like phosphate parameters indicated that we were dealing with a biologically active PTH. Using preoperative biochemical parameters it was impossible to predict which patients would have an increased PTH post-parathyroidectomy (PTX). Probably the patients with high PTH post-PTX had higher parathyroid volumes. In our opinion after PTX, a normocalcemic high PTH situation should be avoided by 3 1/2 parathyroid gland extirpation in all cases.Type of Medium: Electronic ResourceURL:
