    Abstract We report on a 49-year-old woman with relapsing ovarian cancer who developed a hypersensitivity reaction (HSR) to carboplatin and, subsequently, to cisplatin. This patient was known to be allergic to Co-Amoxiclav and talc, both giving rise to a transient macular skin rash, but had no other history of atopy. Similar cases, including some of life-threatening severity, have been reported in the literature. These severe reactions may prevent a small population of young patients from receiving effective therapy with cisplatin or its analogues, treatment known to be associated with a significant improvement in survival in germ-cell tumours, ovarian cancer and osteogenic sarcoma.

    Résumé Les auteurs rapportent un cas de méningiome de la convexité cérébrale, qui, deux ans après une exérèse chirurgicale quasi complète, récidiva localement et à distance au niveau de la duremère et du cortex. Les lésions avaient les mêmes caractéristiques de bénignité que la tumeur primitive. Puisque les nodules n'étaient pas visibles lors de la première artériographie pré-opératoire ni lors de la première intervention, les auteurs émettent l'hypothèse que la tumeur s'est développée par dissémination des cellules par le liquide céphalorachidien soit lors de l'intervention soit plus tard. La transmission rétrograde des cellules tumorales pu également avoir lieu par voie veineuse rétrograde dans un système veineux partiellement obstrué.
    Abstract:
    Zusammenfassung Es wird über einen Fall eines Meningiomas der Hirnkonvexität berichtet. Innerhalb von 2 Jahren nach der ersten fast totalen chirurgischen Entfernung des Tumors entstanden ein Rezidiv des Primärtumors sowie multiple Implantationen in der Dura und auf der Gehirnoberfläche. Die Primärläsion und die Streuherde blieben jedoch gutartig. Da die Knötchen weder beim ersten preoperative Aortogramm noch bei der Operation beobachtet worden waren, könnten sie sich durch Aussaat über den Liquor entweder bei der Operation oder später entwickelt haben. Eine hämatogene, retrograde Ausbildung der Tumorzellen in einem teilweise verlegten Venensystem der Gehirnkonvexität wäre auch möglich.

    Summary A case is reported of convexity meningioma, which, during a period of two years after the initial, almost complete surgical removal of the tumor, exhibited prominent recurrence of primary tumor as well as multiple dural and brain surface implants. The lesion remained benign, as did the associated tumor nodules. Since the nodules were not observed in the first preoperative arteriogram nor at the initial operation, the lesions may have developed due to CSF migration, either at the time of operation or later. Retrograde transmission of tumor cells in a partially obstructed convexity venous system could also have occurred.

    Abstract A 31-year-old man with malignant melanoma of his right popliteal fossa wastreated in 1987 with surgical excision followed by local radiotherapy. Eightmonths later, he presented with recurrence in the right inguinal lymph nodes,which were resected and followed by radiotherapy to the groin. Ten monthslater, he developed liver metastases and was treated with vindesine (12months) and interferon-α-2a (30 months) resulting in complete remissionwhich has been maintained for over 10 years. This interesting case report,with brief review of literature, is presented here.

    Summary Background: Drug selection for the treatment of advanced breast cancer is based on both efficacy and toxicity. Combination chemotherapy produces higher response rates than single agents, of which doxorubicin is the most active. This study compares efficacy and toxicity of the drugs doxorubicin and mitoxantrone when used as part of a 3 drug combination. Doxorubicin is the most active agent, but also one of the most toxic, and in this study was compared, in a 3-drug combination, with mitoxantrone with the aim of achieving comparable efficancy with reduced toxicity. Patients and methods: 110 patients with advanced breast cancer previously untreated by chemotherapy were randomized to receive cyclophosphamide and vincristine, together with either doxorubicin 50 mg/m2 IV (VAC) or mitoxantrone 10 mg/m2 (VNC) for up to 6 cycles. Results: Of 53 eligible patients randomized to VAC, the overall response rate was 55% (CR rate 17%), while of 55 patients randomized to VNC the overall response rate was 42% (CR rate 7%). The difference is not statistically significant (p = 0.07), but there was a trend towards a higher response rate to VAC in patients aged less than 60, those with nodal and soft tissue disease, and those with 2 or more sites of disease. The principal difference in toxicity was reduced alopecia in favour of VNC. However there was also an increased number of deaths within the first cycle in patients randomized to VAC. There were no differences in survival, relapse free interval, or freedom from progression between the two arms. Conclusions: Both VAC and VNC are effective regimens in advanced breast cancer. While the confidence limits in this study mean the response rate advantages of VAC could have arisen by chance, younger patients with adverse prognostic factors may warrant consideration of the VAC regimen.
