    A multicentre, prospective study of 26 patients was undertaken for the assessment of insertion of minitracheotomy tubes by the Seldinger technique. The technique of insertion is described. There were two misplacements, three blockages of the inserting Tuohy needle with fat, and six cases of difficulty in passing the minitracheotomy tube.

    The Graseby patient-controlled analgesia system was evaluated in the laboratory and in clinical use. The problems encountered with eight examples used to treat 510 patients are reported. Laboratory performance revealed the unit to be accurate at infusion volumes of 1 and 2 ml.

    A survey of postoperative analgesia in 195 anaesthetic departments in England and Wales was undertaken. The results showed that 64% of respondents were dissatisfied or very dissatisfied with the present situation. Large differences were demonstrated between what was regarded as the safest technique and what would form the ideal management of postoperative pain.

    Twenty anaesthetic machines were examined to detect leakage of fresh gas through open Rotameter flow control valves and empty cylinder yokes. Significant leakage was demonstrated in most machines. Any leakage in machines which comply with ISO 5358, where oxygen is the last gas into the common gas manifold, is selective for nitrous oxide and, conversely, it is selective for oxygen in machines where oxygen is the first gas into the common gas manifold. Leakage is augmented by back pressure from ventilators that function as minute volume dividers. In ventilated patients the leakage described may contribute to hypoventilation and patient hypoxia or to pollution of the operating theatre and patient awareness.

    Transdermal hvoscine (Scopoderm TTS CIBA) was compared with placebo in 67 patients receiving postoperative analgesia via a patient-controlled analgesia system. All patients underwent an abdominal hysterectomy and had a standard anaesthetic. They were followed up in recovery and daily for 3 days postoperatively. Fewer patients in the hyoscine group suffered emetic sequelae in recovery and on the third postoperative day (p 〈 0.05). The hyoscine group received half the number of supplementary antiemetic doses compared to placebo. However, despite transdermal hyoscine there was still a high (78%) incidence of nausea and vomiting. The only significant (p 〈 0.05) increase in side effects attributable to hyoscine was a higher reported incidence of visual disturbance on the second day.

    Eight experienced anaesthetists performed a ‘cockpit drill’, following instructions in the Association of Anaesthetist's checklist, on an anaesthetic machine that had a significant leak (3 l.min−1 at a pressure of 16 kPa). Only one anaesthetist detected the leak and this was by audible means rather than by any of the protocol's set manoeuvres. We demonstrated that a leak of 3 l.min−1 from the flowmeter block resulted in an inspired oxygen concentration of 6% when the anaesthetic machine was used with a minute volume divider ventilator.

    Zusammenfassung Es wurde die Abhängigkeit von feuchten und trockenen Tagen vom Zustand an vorhergehenden Tagen für sechs Stationen in Ost- und West-Afrika und in Südost-Asien untersucht. Während eine Anzahl von Untersuchungen in höheren Breiten darauf hinweisen, daß häufig nur der Zustand am vorhergehenden Tag in Betracht zu ziehen ist, zeigt das Ergebnis dieser Studie an, daß in den Tropen mindestens die zwei vorhergehenden Tage von Bedeutung sind. Für manche Stationen mag es angezeigt sein, in bestimmten Zeiten des Jahres die drei vorhergehenden Tage in Betracht zu ziehen.

    Summary Dependence of the occurrence of wet and dry days on the situation on previous days was tested for six stations in East and West Africa and South East Asia. Although a number of studies in higher latitudes suggest that often only the situation on the previous day needs to be taken into account, evidence from this pilot study indicates that in the tropics, at least the previous two days are important. For some stations at certain times of the year, it may be wise to consider the previous three days.

    Abstract For over 40 years germ-cell mutagenesis experiments have generated many new mutations at the brown (b or Tyrp1) locus on mouse Chromosome (Chr) 4. These mutations, many of which are deletions, were recovered by the specific-locus mutagenesis technique. Previous analysis of a panel of brown deletions, generated at Oak Ridge, has enabled both a preliminary molecular and a functional map around the locus to be generated. We have used a panel of hybrid DNA from 25 Oak Ridge deletions, where the deleted chromosome was heterozygous with a Mus spretus chromosome, to map polymorphic markers including microclones, microsatellites, and cloned DNA markers. We have generated a fine structure map, based on 25 new markers, of an 8.5-cM region surrounding the brown locus. This map will prove useful in future mapping studies of this region and in the isolation of the genes that lie within it.

    Zusammenfassung Es wird auf die Tatsache hingewiesen, daß in verschiedenen Teilen von Tanzania sehr unterschiedliche Schwankungen der Niederschlagsmengen von Jahr zu Jahr auftreten. In dieser vorläufigen Untersuchung wird ein einfaches analytisches Verfahren angewendet, das auf Korrelationen zwischen den Jahresniederschlagsmengen der Stationen beruht, mit dem Ziel zu zeigen, ob Regionen mit ähnlichen Schwankungen der Niederschlagsmengen abgegrenzt werden können. Dabei zeigte sich eine regionale Unterteilung des Staatsgebietes. Die Zonen zeigen eine Tendenz zu einer Erstreckung in Richtung Nordwest-Südost mit Modifizierung durch den Einfluß der Reliefformen. Es wird auf zwei ausführlichere Analysen der Niederschlagsschwankung hingewiesen, die in Vorbereitung sind.

    Summary The fact that various parts of Tanzania have very different fluctuations in rainfall from year to year is pointed out. In this preliminary study, a simple analytical technique involving correlations between rainfall stations on an annual basis was adopted, the aim being to see whether regions showing similar fluctuations in rainfall could be defined. A regional subdivision of the country, not based on proximity, was apparent. The zones tended to have a northwest-southeast alignment, modified by the influence of relief features. Two more detailed analyses of rainfall fluctuations which are under way are described.

    Summary The paper proposes a new classification of raindays and drydays incorporating soil moisture status. This is of particular value for agricultural purposes and also allows the adoption of a low rainfall threshold to distinguish between raindays and drydays. This is important since, as indicated in the paper, small falls may be important and yet in the tropics, for agricultural purposes, a high threshold is often adopted to take account of the high evaporative demand of the atmosphere. Soil moisture is expressed as a percentage of available soil moisture storage capacity (SMSC) and conditions are described as deficit, limiting, adequate, and surplus, if soil moisture levels are 0–29%, 30–59%, 60–100% and 〉 100% of SMSC respectively. Combining this with rainday and dryday occurrence, three types of drydays and four types of raindays are identified. This rain-soil moisture index classification more nearly approaches a universal definition suitable for different tropical regions than previous ones. Application of the concept is illustrated with data from Sierra Leone, using a simple water balance model to estimate soil moisture.

    Summary Using 14 variables combining rainfall occurrence/non-occurrence and soil moisture, 8 relatively homogeneous regions relevant to rainfed agriculture in Sierra Leone are defined. The regions do not simply reflect patterns of either mean annual rainfall or numbers of raindays. For each region, the basic characteristics are described and representative stations are chosen.
