Epithelial ICAM-1, in its dual role as the major rhinovirus receptor and as adhesion molecule for inflammatory cells may be involved in the pathogenesis of rhinovirus-induced exacerbations of asthma.〈section xml:id="abs1-2"〉〈title type="main"〉ObjectiveWe aimed to investigate the effect of experimental rhinovirus 16 (RV16) infection on ICAM-1 expression in bronchial mucosal biopsies in asthma. In addition, the effect of 2 weeks pretreatment with inhaled budesonide (800 μg b.d.) on RV16-associated changes in ICAM-1 expression was studied.〈section xml:id="abs1-3"〉〈title type="main"〉MethodsThe study had a parallel, placebo-controlled design in 25 steroid-naive nonsmoking atopic asthmatic subjects. After 2 weeks budesonide (BUD) or placebo (PLAC) pretreatment bronchoscopy was performed 2 days before (day −2) and 6 days after (day 6) RV16 inoculation (on days 0 and 1). Immunohistochemical staining for ICAM-1 was performed on snap-frozen bronchial biopsies. ICAM-1 staining intensity on the basal epithelial cells was scored semiquantitatively from 1 (weak) to 3 (intense). Similarly, epithelial intactness was noted (1 = basal cells only, 2 = basal and parabasal cells, 3 = intact epithelium).〈section xml:id="abs1-4"〉〈title type="main"〉ResultsICAM-1 scores were not significantly different between the groups at day −2 (P ≥ 0.08). Subsequent RV16 infection was associated with a trend towards an increase in ICAM-1 expression in the BUD-group (P = 0.07), whereas the increase was significant in the PLAC-group (P = 0.03). However, the increase was not significantly different between the groups (P = 0.74). Epithelial intactness score was not different between the groups before RV16 infection (P ≥ 0.07), and no significant changes were observed in either group (P ≥ 0.59). Moreover, ICAM-1 score did not correlate significantly with epithelium score in either group, at any time-point (P ≥ 0.27).〈section xml:id="abs1-5"〉〈title type="main"〉ConclusionWe conclude that an RV16 common cold in atopic asthmatic subjects is associated with increased ICAM-1 expression in the bronchial epithelium, which is not related to epithelial intactness. Glucocorticoid treatment does not appear to prevent the RV16-associated increased ICAM-1 expression. Lebensjahr wurden mittels Läppchentests auf ihre Reagibilität gegenüber sechs Substanzen geprüft, die bei Erwachsenen häufig kontaktallergische Ekzeme verursachen (Kaliumbichromat 0,5%, Sublimat 0,1%, Formalin 1,2%, Nickelsulfat 2%, Terpentin 5%, Benzocain 5%). 2. Die Häufigkeit der beobachteten Reaktionen war abhängig a) von der Testsubstanz und b) von Lebensalter. a) In absteigender Reihenfolge erzeugten Reaktionen: Kaliumbichromat (29,7%), Sublimat (13,2%), Formalin (9,5%), Nickelsulfat (2,5%), Terpentin (2,2%) und Benzocain (0,8%). b) Säuglinge im 2.–4. Trimenon reagieren am häufigsten (72,8%), im 1. Trimenon am seltensten (15,4%); vom 2. Lebensjahr an sinkt die Reagibilität: 1-3-jährige: 56,4%; 4–9 jährige: 32,6%; 10–14 jährige: 19,7%. 3. Die makroskopisch-klinisch und histologisch ekzematösen Reaktionen sind toxischer Natur. 4. Erwachsenengerechte Testkonzentrationen eignen sich nicht, um bei Kindern eine Kontaktallergie aufzudecken.Notes: Summary 1. 357, mostly healthy children in the age of 1 month to 14 years were tested with 6 substances often causing allergic eczemas in adults (potassium dichromate 0.5%, bichloride of mercury 0.1%, formalin 1.2%, nickel sulfate 2%, turpentine 5%, benzocaine 5%). 2. The percentage of positive reactions depends a) on the testsubstance and b) on the age of the children. a) Reactions were produced by potassium dichromate in 29.7%, by bichloride of mercury in 13.2%, by formalin in 9.5%, by nickel sulfate in 2.5%, by turpentine in 2% and by benzocaine in 0.8% of the children. b) Most frequently reactions were observed in infants 4–12 month old (72.8%), seldom in newborns till to the third month of life (15.4%). From the second year onwards the reagibility decreases: 56.4% in 1–3 year, 32.6% in 4–9 year and 19.7% in 10–14 year old children. 3. The macroscopic-clinically and histologically eczematous test reactions are toxie (=primary irritant) reactions. 4. Specificity was 90% in the whole patient group. Sensitivity and specificity of WBS were 50% and 99%, respectively. When the results of FDG-PET and WBS were considered in combination, tumour tissue was missed in only 7%. Sensitivity and specificity of MIBI/Tl were 53% and 92%, respectively (n=117). We conclude that FDG-PET is a sensitive method in the follow-up of thyroid cancer which should be considered in all patients suffering from differentiated thyroid cancer with suspected recurrence and/or metastases, and particularly in those with elevated thyroglobulin values and negative WBS.Type of Medium: Electronic ResourceURL:
