This study describes the presence of crystalline inclusions in the endothelial cells of foreskins. They were found in 9 normal full-term newborn infants, without maternal medication or complication during pregnancy and delivery. These cytoplasmic crystalloids occurred in 10% of endothelial cells of small blood vessels. The diameter of the inclusions ranged from 0.3 to 1.5 μm and they appeared as round, oval, hexagonal or irregular polygonal in shape. These inclusions were surrounded by a triple membrane and their contents demonstrated granular, homogenous and crystalloid-like material with a regular periodicity of dense and less dense layers measuring about 20–25 nm. Similar crystalloids in the endothelial cells were observed in the normal upper lip skin of a 6-week-old girl, although they were present in much smaller numbers than in foreskin. No endothelial inclusions were found in normal skin taken from 11 body areas in 29 patients aged 2.5 to 56 years. The nature and function of these cotyplasmic crystalloids are unknown.

    Notes:
We performed standard epicutaneous patch testing on 59 community-based patients with leg ulcers of predominently venous aetiology. Allergic reactions occurred in a total of 30 of 59 (50.8%) of patients and were significantly more common in patients with coexisting eczema (62%) than in those without (38%) (P〈0.05). In 26 of 30 (86.7%) patients with reactions, the allergies were related to leg ulcer therapy and the treatment was changed accordingly. This study demonstrates that contact sensitivities are common among community-based leg ulcer patients and we suggest that identification of allergens by patch testing should reduce the morbidity, in particular in those patients with complicating eczema.

    Notes:
A retrospective review of patch test results from all new patients with venous leg ulcers was performed for the preceding 11 months. Eighty one patients referred from general practitioners and district nurses with venous stasis ulcers were included. Positive patch tests were found in 54 patients (67%), including a continued high incidence of allergy to lanolin and topical antibiotics. Multiple allergies were found in 48 patients (58%). In addition, a new problem of allergy to cetearyl alcohol, a constituent of commonly used creams and paste bandages, was identified in 13 patients. There is a continuing high incidence of contact sensitivity in patients with venous stasis ulcers which has important implications for the management of these patients.
