We followed-up 43,166 subjects for the mean 14.6 years, and 213 suicides were committed. Daily coffee drinking had a J-shaped association with the risk of suicide. Using the Cox model we controlled for potential covariates, and found that among heavy coffee drinkers (≥ 8 cups/day) the risk of suicide was 58% higher compared with more moderate drinkers.Type of Medium: Electronic ResourceURL: -

ISSN: 1434-4726Keywords: Chronic otitis media ; Cholesteatoma ; Middle ear surgery ; TympanoplastySource: Springer Online Journal Archives 1860-2000Topics: MedicineNotes: Summary A study of 277 ears with chronic otitis media undergoing one-stage tympanoplasty has been carried out. The mean follow-up period was 6.4 years. Ears with intact ossicular chains were excluded. Ossiculoplasty was performed using autologous ossicles or cortical bone. Mean hearing gain was 10.8 dB. Closure of the postoperative air-bone gap to within 20 dB was achieved in 51% of cases. The best results were obtained in ears with intact stapes, while cholesteatomatous ears showed poorer results than other chronic ears. The present findings show that autologous ossicle and cortical bone are still suitable for ossicular reconstruction in chronic ears, especially when one-stage surgery is preferred.Type of Medium: Electronic ResourceURL: -

ISSN: 1434-4726Keywords: Deafness ; Otosclerosis ; Stapedectomy ; Bone conduction thresholdsSource: Springer Online Journal Archives 1860-2000Topics: MedicineNotes: Abstract The effect of preoperative bone conduction (BC) threshold levels on the results of stapedectomy was evaluated in 378 adult patients with otosclerosis. The mean follow-up period was 8.8 years. Both short-term and long-term postoperative hearing levels were found to be significantly dependent on preoperative BC levels. Only 2 of 31 patients (7%) with preoperative BC thresholds 〉 40 dB had postoperative hearing levels of 30 dB or better. However, hearing still improved by 〉_ 21 dB as often in patients with elevated preoperative BC thresholds as in patients with normal BC thresholds. These findings support the use of stapes surgery being worthwhile even in patients with highly elevated BC thresholds.Type of Medium: Electronic ResourceURL: -

ISSN: 1434-4726Keywords: Chronic otitis media ; Cholesteatoma ; Tympanomastoid surgery ; Complications ; DeafnessSource: Springer Online Journal Archives 1860-2000Topics: MedicineNotes: Abstract Preoperative and postoperative bone conduction thresholds were compared in 181 chronic ears operated on over a 5-year period between 1990 to 1994. In the majority (92%) of cases the bone conduction thresholds remained unchanged (± 10 dB). Nine ears (5%) showed better thresholds after surgery, with improvements ranging from 11 dB to 25 dB. This improvement was especially noted in ears with severe tympanic pathology. One ear with a large labyrinthine fistula became totally deaf after surgery. In 5 ears (3%) bone-conduction thresholds deteriorated, but remained measurable at all frequencies tested. In these latter cases this impairment ranged from 11 dB to 27 dB. Cholesteatomatous ears having intact ossicular chains were found to be at the highest risk of inner ear damage when “canal wall-down” mastoidectomies were performed. Methods for prevention of sensorineural hearing loss following chronic ear surgery are discussed.Type of Medium: Electronic ResourceURL: -

ISSN: 1434-4726Keywords: Key words Audiometry ; Deafness ; Otosclerosis ; GenderSource: Springer Online Journal Archives 1860-2000Topics: MedicineNotes: Abstract The course of hearing in the non-operated ears was examined in 112 female patients and 63 male patients with bilateral otosclerosis who had operations in one ear only. All patients had a follow-up of at least 5 years. The mean annual impairment of the air conduction thresholds in the female patients (2.0–2.5 dB/year) did not differ significantly from that of the male patients (2.0–2.3 dB/ year). At the last follow-up examination, no difference was found between the sexes in the magnitude of the air-bone gaps. However, a sensorineural component developed in the women more frequently than in the men. Although at the last examination bone conduction thresholds of the female patients were significantly worse than those of the male patients, these were not matched by age.Type of Medium: Electronic ResourceURL: -

ISSN: 1434-6036Source: Springer Online Journal Archives 1860-2000Topics: PhysicsNotes: Abstract. Negative refractive index media have become a hot topic in physics due to their proposed revolutionary properties, which would have drastic consequences in design of novel optical devices. We show that Kramers-Kronig relations connecting the real and imaginary parts of the complex refractive index of absorbing media are valid even though the real refractive index may take negative value at some spectral range. In addition universal sum rules for linear optical constants of negative index media are also valid. This means that negative refractive index media are not fundamentally different from regular media. Hence, any spectrum measured from negative refractive index media can be analyzed using dispersion relations and sum rules, which have so far provided information on the optical properties of materials.Type of Medium: Electronic ResourceURL: -

ISSN: 1434-4726Keywords: Bilateral otitis media ; Cholesteatoma ; TympanoplastySource: Springer Online Journal Archives 1860-2000Topics: MedicineNotes: Summary We have compared the clinical presentations and results of operative treatment in 86 patients with bilateral chronic otitis media with findings in 641 patients with unilateral disease. Concomitant conditions such as cleft palate, rhinitis and chronic adenotonsillitis were found significantly more frequently in those patients with bilateral chronic otitis media than in the patients with unilateral disease. Additionally, cholesteatomas in the cases in which both ears were involved were larger than in those cases which only one ear was involved. The overall results of surgery were similar in the bilateral and unilateral cases. The timing of operation in cases of bilateral chronic otitis media and the special problems of operation on an only hearing ear are discussed.Type of Medium: Electronic ResourceURL:
