    The purpose of this study was to determine the in vitro load transmission characteristics of the canine stifle joint, paying particular attention to the positioning effect of the meniscus in the coronal plane. The intact joint was first loaded, and then tested under two different loading conditions after a complete medial meniscectomy. The first set of test conditions attempted to simulate those used by previous investigators, by ignoring the spacer effect of the meniscus and not repositioning the joint after its removal. The second set of tests was carried out after the joint was repositioned in the coronal plane to allow initial contact to occur in both tibiofemoral compartments. It is presumed that this occurs subsequent to a meniscectomy in vivo, following the application of any weight-bearing load. As with previous investigators, it was found that after meniscectomy the joints produced slightly larger displacements and lower stiffnesses than when intact (no significant differences from intact). However, repositioning the meniscectomized joint produced markedly smaller displacements (35-49%, p 〈 0.01) and greater stiffnesses (47-123%, p 〈 0.05) over the range of forces analyzed, compared with the intact joint. The ratio of dissipated to input energy was 42% for the intact joint, and rose following meniscectomy to 54% (p 〈 0.05) with repositioning and 55% (p 〈 0.05) without repositioning. Measured contact area decreased by 17% (p 〈 0.05) following meniscectomy alone, and by 12% (p 〈 0.05) following meniscectomy with repositioning. Since repositioning of the joint subsequent to meniscectomy (accounting for the loss of the meniscal spacer) resulted in an increse in structural stiffness, it was concluded that the medial meniscus decreases the structural stiffness of the intact stifle joint. In addition, the meniscus has a role in elastic energy storage and increasing contact area. This study is intended to serve as a baseline comparison for future in vivo studies on meniscetomy, meniscal repair, and meniscal replacement, in addition to more fully elucidating the mechanism of load transmission. A model is presented to explain both the decrease in stiffness after meniscectomy without repositioning and the increase in stiffness after meniscectomy with repositioning, employing linear springs of unequal length and different stiffnesses. After removal of the softer meniscal element and allowing joint approximation to occur, loading of the stiffer articular element results in an initially stiffer preparation.

    Objective: This study examined differences in health and access to dental services among a nationally representative sample of patients with HIV using Andersen's Behavioral Model of Health Services Use. Methods: This investigation is a longitudinal study that used structural equation modeling to analyze data from the HIV Cost and Services Utilization Study, a probability sample of 2,864 adults under treatment for HIV infection. Key predisposing variables included sex, drug use, race/ethnicity, education, and age. Enabling factors included income, insurance, and regular source of care. Need factors included mental, physical, and oral health. Dependent variables included whether a respondent utilized dental services and number of visits. Results: More education, dental insurance, usual source of dental care, and poor oral health predicted a higher probability of having a dental visit. African Americans, Hispanics, those exposed to HIV through drug use or heterosexual contact, and those in poor physical health were less likely to have a dental visit. Of those who visited dental professionals, older persons, those with dental insurance, and those in worse oral health had more visits. African Americans and persons in poor mental health had fewer visits. Conclusions: Persons with more HIV-related symptoms and a diagnosis of AIDS have a greater need for dental care than those with fewer symptoms and without AIDS, but more pressing needs for physical and mental health services limit their access to dental services. Providers should better attend to the oral health needs of persons with HIV who are in poor physical and mental health.

    An understanding of developmental mechanisms and new cell therapies can be achieved by transplantation into the nervous system. Multipotential stem cells have been isolated from the foetal and adult central nervous system (CNS). Immortalized and primary precursor cells integrate into the developing brain generating both neurons and glia as defined by immunological and morphological criteria. Here we show for the first time that in vitro-expanded CNS precursors, upon transplantation into the brains of rats, form electrically active and functionally connected neurons. These neurons exhibit spontaneous and evoked postsynaptic events and respond to focal glutamate application. Donor cells were grafted into the foetal hippocampus, and the amplitude and frequency of spontaneous synaptic events were monitored in the grafted cells in area CA1 for the first month of postnatal life. The formation of synapses onto grafted neurons indicates that grafted CNS stem cells can be used to study synaptic development in vivo and has important implications for clinical cell replacement therapies.

    Despite the fact that more adolescents use alcohol than any other drug, studies of teenage alcohol misuse are relatively rare. Those that exist frequently fail To include high school dropouts and often focus on only part of the problem, such as how much or how often the adolescent drinks. This study examines the prevalence and demographic predictors of teenage alcohol misuse in a diverse sample of 4390 high school seniors and dropouts. It focuses on three different dimensions of misuse—high-risk drinking, alcohol-related problems and high consumption—and provides prevalence estimates by gender and race/ethnicity that are weighted to represent the original seventh grade cohort of 30 California and Oregon schools. Results show that by grade 12, nearly 70% of these teenagers have exhibited some form of alcohol misuse within the past year. Two-thirds have engaged in high-risk drinking and over 50% have experienced one or more alcohol-related problems. More stringent estimates that require variety or persistence of risky drinking and/or alcohol-belated problems still capture between 40% and 54% of this population. However, focusing solely on high consumption fails to identify as many as half of these at-risk misusers. Males and females both exhibit high rates of alcohol misuse, as do most racial ethnic groups. However, African Americans and Asians are less likely to misuse alcohol than whites and Hispanics. The results underscore the need for including different forms of alcohol misuse in prevention programs, for improving our understanding of its etiology, and for providing upper and lower bound estimates of alcohol misuse in future research.

    This paper examines sexual risk-taking within a sample of sexually active gay and bisexual men entering substance abuse treatment (n= 383), and identifies correlates of unprotected anal sex within this group. Sexual risk-taking was high, with 55% of these men engaging in anal intercourse without a condom within a 90–day period. Correlates of unprotected anal sex varied somewhat when looking at unprotected anal sex with a primary partner only and with non-primary partners; substance use variables (number of drugs used, use of inhalant nitrites or stimulant drugs with sex, length of time since use of alcohol/drugs, loss of control problems associated with alcohol/drug use) appear to play more of a role in unprotected anal sex with non-primary partners. Overall, logistic regression analyses indicated that sexual risk was greater for those who were more sexually active, enjoyed unprotected anal sex with withdrawal prior to ejaculation, did not approve of sex outside of a love relationship, and identified themselves as more risky. In addition, those who reported more social problems due to substance use had fewer expectations that substance use increased risk, had been HIV-tested, and used reappraisal/problem-solving coping strategies showed greater risk with a primary partner only. Sexual risk with non-primary partners was greater for those who used more drugs, reported more difficulty avoiding high-risk sex when aroused and were HIV + The paper discusses the implications of these findings for the design of sexual risk-reduction interventions.

    This report describes an unusual occurrence of cavitary lung lesions which may have been secondary to aspiration of mineral oil and illustrates a possible danger of aspiration pneumonia associated with administration of lipids. Lipid pneumonia should be considered in the differential diagnosis for pulmonary disease with radiographic signs ranging from increased interstitial opacity to nodules or cavitary lung lesions. This report also emphasizes the benefits of serial radiography to detect changing pathology and the occurrence of complications such as pneumothorax.

    This report describes the use of computed tomography (CT) to evaluate the tympanic bullae and angular processes of the mandible in a West Highland White Terrier with craniomandibular osteopathy. The patient presented initially for swelling of the right forelimb. The report illustrates the use of computed tomography for evaluation of craniomandibular osteopathy, initial presentation with longbone pathology, and remarkable resolution of osseous changes.
