Published 1995Staff ViewISSN: 1365-2230Source: Blackwell Publishing Journal Backfiles 1879-2005Topics: MedicineNotes: We report a case of acute intoxication due to a massive overdose of isotrerinoin.A 29-year-old male patient ingested 900 mg of isotretinoin corresponding to 12·5mg (kg/day) or 30 times the prescribed dosage and 1 day later the patient experienced mild headache. Forty-eight hours later, cheilitis, diffuse cutaneous xerosis and desquamation of the forehead and of the external auditory meatus occurred; cutaneous xerosis and cheilitis resolved spontaneously. We determined the serum level of isotretinoin and of 4-oxo-isotretinoin, its natural metabolite in sera taken 4, 5, 6 and 11 days following ingestion.The side-effects were mild and represented only exacerbations of some common isotretinoin side-effects. To date, three other cases of isotretinoin overdosage have been reported. There was a low toxicity of isotreitnoin overdose.Type of Medium: Electronic ResourceURL: -

We present a technique which allows the dimension of a point contact to "shrink'' down to a single atom. Furthermore, this technique enables us to adjust a tunnel barrier between two inherently stable electrodes. Instead of a feedback system it relies on a large reduction factor assuring an inherently stable device. Stability measurements in the tunnel regime infer an electrode stability within 3 pm in a 1 kHz bandwidth. In the contact regime the conductance takes on a discrete number of values when the constriction is reduced atom by atom. Both 7.5-cm-diam MgF2 and CaF2 windows were found to be He leak tight to temperatures exceeding 330 °C. Smaller 4-cm-diam windows seal at temperatures exceeding 400 °C. The seals do not appear to be adversely affected by temperature cycling and they do not distort the optical quality of the window. The sealing technique has been used without failures in a Hg vapor four-wave-mixing cell that is used to produce high-power 130-nm coherent radiation.Type of Medium: Electronic ResourceURL: -

A consecutive series of 54 patients with suspected clinical photosensitivity were assessed. All had the same standardized photobiological investigation from January 1989 to December 1990, including patch tests and photo patch tests with 6 sunscreen agents. Oxybenzone was found to cause 4 cases of allergic contact dermatitis (with photo aggravation in 2), and 3 cases of photocontact dermatitis (13% of patients). This is probably due to the wide distribution of oxybenzone in sunscreens and other cosmetics, 2 patients with polymorphic light eruption and persistent light reactivity, respectively, were regular sunscreen users. Photo biological investigation is necessary to ensure accurate diagnosis, since sunscreen contact or photo contact allergy may simulate other photosensitivity eruptions.Type of Medium: Electronic ResourceURL: -

Arterial levels of glucose and oxygen consumption were determined in 100 consecutive patients during orthotopic liver transplantation performed without anhepatic veno-venous bypass. Patients were divided into survivors with no obvious problems related to graft function and those with primary nonfunction of the graft. The neohepatic increase in V̇o2 was significantly higher in survivors (112 + 4 vs 88 + 11 ml.min−1.m2; p 〈 0.05), whereas blood glucose levels after reperfusion were higher (352 + 18 vs. 287+36 mg dl−1) in those with primary non-function of the graft. The calculated metabolic index was also higher (4.02 + 0.93 vs 2.67+ 0.45, p 〈 0.05) in patients with primary nonfunction of the graft. Our principal conclusion was that 92% of normal functioning liver grafts could be classified correctly by the metabolic index immediately after reperfusion, whereas glucose levels and V̇o2 alone classified only 67% and 70% of normal functioning liver grafts correctly.Type of Medium: Electronic ResourceURL:
