ISSN: 1471-0528Source: Blackwell Publishing Journal Backfiles 1879-2005Topics: MedicineNotes: The management of abruptio placentae at the Glasgow Royal Maternity Hospital over the five years 1966 to 1970 has been reviewed. There were 379 cases in 29,456 deliveries representing an incidence of 1.3 per cent or 1 in 78 deliveries. The overall perinatal mortality was 38 per cent and there was one maternal death. The perinatal loss related to prematurity and clot size was examined as well as the loss of fetuses which were alive on admission to hospital. Nineteen babies were probably saved by Caesarean section but 13 babies, weighing over 2000 g. and with clinical evidence of fetal distress died in uteroand might have been saved by abdominal delivery. A 4.4 per cent incidence of fetal abnormality was noted. The role of fetal monitoring and Caesarean section are briefly discussed.Type of Medium: Electronic ResourceURL: -

Published 2001Staff ViewISSN: 1471-0528Source: Blackwell Publishing Journal Backfiles 1879-2005Topics: MedicineNotes: Objective To determine whether a combined therapy with isosorbide mononitrate (40mg) and misoprostol (400μg) for pre-operative cervical ripening in the first trimester would result in improved clinical effectiveness, and fewer side effects compared with each agent used alone.Design Randomised controlled trial.Setting Glasgow Royal Infirmary.Population Sixty-six primigravid women scheduled for suction termination of pregnancy.Methods Women were randomly assigned to receive before surgery, per vaginam, isosorbide mononitrate 40 mg (n=22), misoprostol 400 μg (n=22) or both agents together [isosorbide mononitrate 40 mg and misoprostol 400 μg] (n=22).Main outcome measures 1. To assess the cumulative force required to dilate the cervix to 8 mm; 2. the onset of new symptoms before termination of pregnancy.Results The cervical resistance following combination therapy with isosorbide mononitrate and misoprostol was not significantly different than following misoprostol alone [24.5N vs 18.5N; median difference (95% CI) 19N (-22 to 49)]. Pre-treatment with misoprostol used alone resulted in a lower cervical resistance than isosorbide mononitrate alone (18.5Nvs 39N, P=0.04, Mann-Whitney U test). There was no difference in the number of women remaining asymptomatic following either isosorbide mononitrate or misoprostol or combination therapy [14/22 (64%) vs 11/21 (52%) vs 11/22 (50%), Fisher's exact test].Conclusions We have not shown any advantage of combining misoprostol with the nitric oxide donor isosorbide mononitrate compared with misoprostol alone for pre-operative cervical ripening in the first trimester.Type of Medium: Electronic ResourceURL: -

Published 1997Staff ViewISSN: 1471-0528Source: Blackwell Publishing Journal Backfiles 1879-2005Topics: MedicineNotes: Objective To determine whether nitric oxide donors can induce cervical ripening before surgical termination of pregnancy in the first trimester.Design Prospective, randomised controlled trial.Setting Department of Obstetrics and Gynaecology, Royal Infirmary, Glasgow.Participants Forty-eight primigravid women undergoing surgical termination of pregnancy before 12 weeks of gestation.Methods The women were randomised to receive per vaginam before surgery either the nitric oxide donor isosorbide mononitrate, the nitric oxide donor glyceryl trinitrate, the prostaglandin analogue gemeprost, or no treatment.Main outcome measures The cumulative force required to dilate the cervix to 8 mm was measured objectively and the cervical diameter before surgical dilatation was recorded.Results Following isosorbide mononitrate or gemeprost, a lower cumulative force was required to dilate the cervix to 8 mm and a higher cervical diameter before dilatation was recorded. Pretreatment with glyceryl trinitrate reduced the cumulative force required to dilate the cervix but had no effect on cervical diameter.Conclusions Like the prostaglandin analogue gemeprost, the nitric oxide donors isosorbide mononitrate and glyceryl trinitrate can effect cervical ripening. Nitric oxide donors may provide an alternative to prostaglandins for cervical ripening before surgical procedures in the first trimester.Type of Medium: Electronic ResourceURL: -

The purpose of the study was to determine the prevalence of burning mouth (BM) in a population of Parkinson's Disease (PD) sufferers and also to assess the use of pain profiles in identifying the type of burning sensation experienced. Design. Subjects were surveyed by means of a one shot postal questionnaire for which ethical approval had previously been granted. Anonymity was guaranteed and therefore no attempt was made to follow up non-respondents. Main outcome. BM was reported by 24% of respondents. The pain profiles were completed by 17 BM sufferers. Conclusion. Burning mouth is reported to occur in 24% of PD sufferers which is 5 times greater than that of the general population. The reason for this is uncertain but the result has implications for the future care of PD patients and indicates the need for increased dental input at PD outpatient clinics.Type of Medium: Electronic ResourceURL: -

They were classified into a toothwear (largely of attritional origin) group (n=30) and a non-toothwear, or normal, group (n=60). The mean CRP deduced for the groups was similar in all three dimensions and found to be just less than 2 mm open from and 0·6 mm posterior to intercuspal position (IP) and approximately in the midline. The CSP, in two dimensions, of the toothwear group was significantly closer to IP than that of the normal group. The vertical component of CRP and CSP was similar for the normal group but CSP was significantly reduced in comparison to CRP for the toothwear group. The most forward component of CSP was significantly more anteriorly placed than was that of CRP. Although there was a pronounced inter-individual variation the results suggest toothwear, of long acting aetiology, has less affect on CRP than on CSP.Type of Medium: Electronic ResourceURL: -
