Abstract  The detection of malignant cells in serous effusions obtained from patients diagnosed with cancer marks the presence of metastatic disease and is associated with a poor outcome. The purpose of this study was to evaluate the role of CD44s and CD44v isoforms in the distinction between mesothelial cells and malignant epithelial cells in effusions. Fifty-nine fresh pleural and peritoneal effusions were studied. These consisted of 41 specimens from patients with known gynecological neoplasms, 9 from patients diagnosed with breast adenocarcinoma, and 9 effusions from patients with various nongynecological malignancies or tumors of unknown origin. Forty-three effusions contained malignant/atypical epithelial cells, and 16 effusions were diagnosed as reactive. Three effusions contained exclusively malignant cells. Specimens were stained with anti-CD44s, v3, v5, v6, v7 and v3-10. The presence of staining in cancer cells, benign mesothelial cells and lymphocytes was evaluated. CD44s immunoreactivity was seen in 10 of 43 (23%) cases in malignant/atypical epithelial cells and in 53 of 56 (94%) cases in benign cells. In contrast, CD44v3-10 was seen in 23 of 43 (55%) cases in malignant/atypical epithelial cells and in 3 of 56 (6%) cases in benign cells. We advocate the use of CD44s and CD44v3-10 immunostaining in diagnostic evaluation of difficult serous effusions.

Abstract CD44 is a family of cell adhesion molecules involved in a variety of cellular functions. The present study analysed the expression of two CD44 isoforms in serous effusions of patients diagnosed with ovarian carcinoma and corresponding primary and metastatic lesions. Fifty-eight effusions, 23 primary ovarian tumours, and 44 metastatic lesions were studied for protein expression of CD44s and v3-10 using immunohistochemistry. Results were correlated with clinical parameters. CD44v3-10 was seen in carcinoma cells in the majority of cases at all sites. Malignant effusions showed an up-regulation of CD44s compared to both primary tumours and metastatic solid lesions. Mesothelial cells frequently expressed CD44s, but were rarely immunoreactive for v3-10. CD44s immunoreactivity in cancer cells in effusions was significantly more often observed in patients with FIGO stage 3 than in stage 4 patients (P = 0.045). Staining results did not correlate with age, effusion site, metastatic site, tumour grade or residual tumour mass after initial surgery. Likewise, comparison of overall and disease-free survival with expression of the CD44 isoforms studied did not reveal any statistically significant associations. The up-regulation in CD44 levels in effusions, primarily in stage 3 disease, suggests that adhesion of ovarian carcinoma cells to mesothelium may be regulated at the level of CD44s expression, and provides further evidence of phenotypic alteration in the transition from primary tumour cell clones to effusions. The similar expression profile of CD44 in carcinoma cells in peritoneal and pleural effusions supports our previous observations and the hypothesis that carcinoma cells in peritoneal effusions are truly metastatic.

Als Ausgangsmaterial der Synthese diente das 20-Oxo-pregnan-Derivat 2, das von uns bereits früher [3] aus dem 18,20-Lacton 1 [2] dargestellt worden ist.

Der Beitrag beschäftigt sich mit dem Armutsproblem in philippinischen Großstädten und den Auswirkungen städtischer Siedlungspolitik und Sozialwohnungsprojekte als Teil einer Politik der gezielten Armutsminderung. Am Beispiel des "Community Mortgage Program" (CMP) wird aufgezeigt, wie zwar die Legalisierung unrechtmäßiger, von Armut betroffener Ansiedler durch die Möglichkeit rechtmäßigen Erwerbens von Land zur Errichtung von Häusern zwar weite Teile der "squatters" zu Grundstücksbesitz verhilft, gleichzeitig jedoch die wirklich Armen unter ihnen ausschließt. Die Gleichsetzung von "squatters" und "urban poor" und die Tatsache, daß viele der unrechtmäßigen Ansiedler der bürgerlichen Schicht entstammen und lediglich in Opposition zu den steigenden Boden- und Mietpreisen stehen, haben dazu geführt, daß das CMP durch die undifferenzierte Annahme einer homogenen Zielgruppe allenfalls einer elitären Mittelschicht der "squatters" dient, während die Ärmsten unter den Armen und Obdachlosen in Slums verbleiben. Das Arbeitspapier diskutiert und reformuliert den "Bielefelder Ansatz" der Analyse strategischer Gruppen/Eliten in der Entwicklungssoziologie. Nach dem Scheitern der "großen Entwürfe" plädiert der Autor für eine Theorie mittlerer Reichweite, die es erlaubt, die vielen Fallstudien zum unterschiedlichen Entwicklungsstand in der "Weltgesellschaft" aufeinander zu beziehen und vergleichbar zu machen. Der Autor hält den Bielefelder Ansatz mit seiner Frage nach kollektivem Handeln und der Gruppenbildung in komplexen Umwelten - trotz aller Kritik - nach wie vor für diesen Zweck brauchbar. Einsichten in diese Machtkämpfe sind auch für die Entwicklungspolitik unmittelbar relevant. Erforderlich sind allerdings einige begriffliche Präzisierungen von "Strategie", "strategisches Handeln", "Gruppe" und die "Appropriation von Surplus". (pmb)

Seit den 70er Jahren ist das Bewußtsein über die globalen Verflechtungen der sozialen, ökonomischen, politischen und insbesondere ökologischen Probleme zunehmend gewachsen. Durch diesen Prozeß der Globalisierung verschwimmen die Grenzen der nationalen Kulturen und Gesellschaften in fortschreitendem Maße. Die Folgen dieses Trends zeigen sich besonders deutlich in den großstädtischen Ballungsräumen (Megapolis). Das Arbeitspapier zeigt am Beispiel des südostasiatischen Raums, daß es sich bei diesen Entwicklungen um einen dualen Prozess von Globalisierung bei gleichzeitiger Regionalisierung bzw. Lokalisierung handelt. Die letzteren Tendenzen kompensieren die unbeabsichtigten Konsequenzen der Globalisierung. Die Autoren diskutieren weiterhin, welche Konsequenzen aus dieser Einsicht für die Soziologie, Kulturanthropologie und Entwicklungspolitik gezogen werden sollen. (pmb)

"Normally, when pension schemes are described, classified and compared, the categories of "public" and "private" are of primary importance. However, the distinction between "public" and "private" in the context of old-age security has recently been questioned for two reasons. First, most pension schemes can not unambiguously be classified as either "public" or "private". Consequently, the idea of an institutional welfare mix and the well-known pillar-metaphor with its clear-cut division between "public pillars" and "private pillars" are obviously too simplistic to convey the empirical diversity in old-age security. And second, recent pension reforms in Western-European welfare states have intensified the state regulation of what is conventionally called private pensions. The pension reform of 2001 in Germany is a good example of such public policy on "private" pensions and its consequences: Tax-financed subsidies for and regulation of occupational pensions and personal saving blur both the distinction between occupational and personal pensions as well as the distinction between "public" and "private". The emerging pension schemes are not only public-private mixes, but are genuinely hybrid. Applying the distinction between ‘public’ and ‘private’ to these schemes would be a serious misinterpretation of their essence and an underestimation of their complexity. Arguments about whether a particular scheme is predominantly "public" or "private" are ultimately political conflicts about the realm and extension of the state. This paper therefore pleads to leave the distinction between "public" and "private" to the political debate. New concepts and terminology are needed for the social scientific analysis of old-age security." (author's abstract)

'Ausgangspunkt der Untersuchung ist die Unterscheidung zwischen Diskursen kommunaler Kriminalprävention und ihrer Institutionalisierung in kriminalpräventiven Räten. Die Analyse kriminalpräventiver Diskurse zeigt, dass die Idee der kommunalen Kriminalprävention besonders im Hinblick auf die Entwicklung konkreter Maßnahmen deutlich unterbestimmt ist. Vielmehr handelt es sich um eine allgemeine Wertidee gemeinschaftsbezogener Sicherheit, die allenfalls einen allgemeinen Rahmen der Konstruktion von Problemlagen in der Stadt abgibt. Aus diesem Grund erweist sich auch die Institutionalisierung als kriminalpräventiver Rat ohne klare Zuständigkeit und Kompetenzen als problematisch, insofern die Themenfindung und Profilbestimmung eher dem 'garbage can'-Modell folgen und von Zufällen bestimmt werden. Dazu wird herausgearbeitet, dass die Bedeutung kriminalpräventiver Räte eher in ihrer symbolischen Funktion und dass eine zentrale Aufgabe eher in der Schaffung von sozialem Kapital innerhalb der Verwaltung zu suchen ist.' 'The starting point of this article is the distinction between discourses of local crime prevention and its institutionalisation as multi-agency partnership. The analyses of discourses of crime preventions shows that the idea of local crime prevention is underdetermined with regard to the organisation of concrete policies. Rather it is an abstract value-loaded idea of community security, which only could function as general frame for the construction of urban social problems. For this reason the institutionalisation as multi-agency partnership without clear powers and responsibilities is problematic, in so far as the decisions and the development of a collective identity follows the garbage can model of organisations and depend largely on change. The article concludes that the importance of multi-agency partnerships on crime prevention is rather symbolic and one of the central functions is the creation of social capital within the administration.' In den großen Städten der Dritten Welt hat nur eine privilegierte Minderheit der Bevölkerung über den kommerziellen Markt Zugang zu Land und Wohnung. Daher schießen "irreguläre" Siedlungen wie Pilze aus dem Boden. Doch sie sind geprägt durch Mangel und Unsicherheit. Der Artikel zeigt, dass das Versagen des formellen Marktes systematisch und strukturell bedingt ist. Daher greifen Versuche, ihn für die Armen zu öffnen, zu kurz und können seine inhärenten Begrenzungen nicht überwinden. Um die sich vergrößernde Kluft zwischen abgegrenzten Gemeinschaften und Ghettos zu verringern, müssen die Interventionen der Verwaltungen den enabling approach wiederentdecken und klüger als die gegenwärtige Mischung aus fahrlässiger Toleranz, brutaler Vertreibung, marktradikaler Privatisierung und populistischer Vergabe von Rechtstiteln werden.

In large cities in the developing world, only a privileged minority of the population has access to land and housing through the commercial market. Mushrooming 'irregular' settlements are filling the gap but are beset by deficiencies and insecurity. This article argues that the failure of formal markets is systemic and structural, and that attempts to open them up to the poor have fallen short of overcoming these inherent limitations. In order to mitigate the widening gap between gated communities and ghettos, government intervention needs to rediscover the 'enabling approach' and become much smarter than the present mix of negligent tolerance, brutal eviction, market-fundamentalist privatisation, and populist titling.
