    In developing countries like Nepal, anaesthetic compressed gases, especially nitrous oxide, are expensive and in short supply and anaesthetic techniques must equally use oxygen and volatile anaesthetics sparingly. We have designed a non-rebreathing anaesthetic system which meets these requirements. An Ambu-E anaesthetic valve and self-inflating Ambu bag connected to a Bain system form a non-rebreathing system which uses ambient air to supplement a mixture of low flow oxygen and halothane. Over 100 patients have been anaesthetised with this system using a balanced anaesthetic technique. The oxygen flow was 2 l.min-1 and the average halothane consumption was 8 ml.h-1. The average inspired oxygen concentration was 34%, and the air: oxygen dilution ratio was 5:1. A graphical analysis of gas flow predicts that the system is almost 100% efficient, in that almost all of the oxygen and halothane will enter the alveoli. Our experience confirms that this is a safe, simple and economical method for inhalation anaesthesia. We recommend it for locations where anaesthetic machines and mechanical ventilators are lacking, and where medical oxygen is in short supply.

    Zusammenfassung Die Augen von 12 Cerco-Affen und 16 Rhesus-Affen wurden mit Herpes simplex-Virus Typ l bzw. Vaccinia-Virus inokuliert. Die rechten Augen wurden zu verschiedenen Zeiten mit Human-Leukocyten-Interferon (HIF) behandelt, während die linken Augen der Kontrolle dienten und den typischen Infektionsablauf zeigten. Bei 7 von 8 „Herpes-Tieren“ und bei allen 9 „Vaccinia-Tieren“, die HIF prophylaktisch oder simultan mit der Inokulation bekamen, konnte die Keratitis völlig verhindert werden. Späteres Einsetzen der HIF-Therapie milderte lediglich den Verlauf der Keratitiden oder hatte keinen Einfluß mehr. Die Gründe hierfür werden diskutiert.

    Summary 12 african green monkeys were inoculated in both eyes with herpes simplex virus typ 1 and 16 rhesus monkeys with vaccinia virus. The right eyes were treated with human leukocyte interferon (HIF) while the left eyes served as controls and showed the typical keratitis. 7 out of 8 herpes eyes and all 9 vaccinia eyes which were treated prophylactically or simultaneously with HIF showed no signs of disease. When HIF was given later in the course of the infection the keratitis was either little influenced or not modified at all. The reasons are discussed.

    Abstract Prophylactic use of the monoclonal antibody OKT3 has been studied for the prevention of rejection in sensitised renal transplant recipients. Patients receiving a full dose (FD) regimen were compared to a subsequent consecutive group of patients receiving a reduced dose (RD) regimen. The characteristics of the two groups were not significantly different with regard to age, HLA mismatch and panel-reactive antibody (PRA) status. The number of days that OKT3 was given was 12.9±1.8 for the FD regimen and 11.3±2.8 for the RD regimen. The total dose of OKT3 given was 64.4±9 mg (FD) and 38.3±8.5 mg (RD). Patient survival at 12 months was 8/8 for FD and 17/17 for RD. Graft survival at 12 months was 7/8 for FD and 17/17 for RD. Creatinine at 24 months was 185±68 and 201±81 μmol/l for FD and RD, respectively. A reduced dose regimen of OKT3 produced excellent and comparable results to the standard recommended fulldose regimen. The cost per patient was reduced 40% from £5676 for FD to £3344 for RD.

    Résumé Des essais de digestibilité et de bilan ont été effectués sur six moutons pour étudier l’influence d’un complément de calcium sur la consommation et la digestibilité de paille d’orge avec une proportion faible ou élevée de tourteau d’arachide. Une augmentation de la digestibilité de la cellulose brute de ce dernier a été attribuée à l’addition de calcium. L’adaptation des moutons aux rations a été progressive mais une grande variation de la consommation et de la digestibilité de quelques constituants alimentaires a été constatée entre les animaux. La rétention d’azote était plus élevée chez les animaux ayant reçu une proportion élevée de tourteau d’arachide, par suite d’une excrétion plus faible d’urée.

    Sumario Los ensayos de digestibilidad y pruebas de balance fueron llevadas a cabo en 6 ovinos castrados con el fin de estudiar el efecto del suplemento de calcio sobre la ingestión y digestibilidad dela paja dela cebada con dietas de harina de maní altas y bajas. El incremento en la digestibilidad de la fibra cruda con la harina de maní en dosis altas se atribuyó al calcio suplementado. La adaptación de los ovinos a las dietas fue notada conforme progresaba el experimento junto con una amplia variación entre los animales en ingestión y digestibilidad de algunos constituyentes de la dieta. La retención de nitrógeno fue mas alta en aquellos animales alimentados con niveles altos de harina de maní, y debido a una mas pequeña excreción de urea.

    Summary Six wether sheep were used in a 6×6 latin square experiment to study the effect of calcium intake on intake and digestibility of barley straw fedad libitum with low and high oil groundnut meal. The level of added calcium ranged from 1.6 to 3.0 g/day for the low oil and 6.4 to 10.4 g/day for the high oil diet. Each experimental period was of 21 days duration which comprised 14 days for adaption to the diet followed by a sevenday collection. The amount of added calcium had a significant effect only with the high oil groundnut meal. The digestibility of crude fibre in diets which contained this meal was significantly increased (p〈0.05) when the amount of added calcium was raised from 6.4 to 8.0 g/day. The sheep showed a marked adaptation to the diets which was manifested as a highly significant increase (p〈0.001) in straw intake as the trial progressed. There was a significant difference (p〈0.05) between animals in intake and digestibility of some dietary constituents. This was shown particularly by one animal whose intake was substantially greater but whose digestive ability was appreciably less than the other five. The nitrogen retention by the sheep on the six diets differed considerably between the two types of groundnut meal. It was higher when the high oil meal was fed and this difference was mainly due to a smaller excretion of urea.
