    A scanning near-field optical microscope is combined with a modified Sagnac interferometer to generate high-resolution magnetic contrast images of a magneto-optical disk (MOD) in reflection. The interferometer acts as a magnetic sensor to determine the Kerr rotation of reflected circularly polarized light. A separate shear-force sample topography is obtained simultaneously with the optical. This technique allows successful magnetic imaging of samples with complicated surface topographies, as demonstrated with the MOD. For these initial measurements, the resolution with nonmetal-coated fiber tips is 300 nm, and the magnetic phase sensitivity is 1 mrad. © 1998 American Institute of Physics.

    The specificity pattern of IgE from non-treated tree pollen allergic patients (n= 38) were evaluated by solid phase absorption of serum samples followed by CRIE on alder, birch and hazel CIE precipitation profiles. The majority of the serum samples seemed to contain IgE antibodies with the following characteristics: specific towards Bet v I alone and common between Aln g I, Bet v I and/or Cor a I, ‘II’. The IgE specificity profiles observed for 95% of the sera tested are compatible with birch pollen allergens being the only sensitizing allergens, indicating that the patients react to allergens from other tree pollens of the Fagales order due to IgE cross-reaction with the major allergens of birch and alder and/or hazel pollens.

    A 27-year-old female, after a fracture of the mandible treated with osteosynthesis and after a difficult extraction, developed osteomyelitis with fistulation and sequestration. Sequestrectomy and extraction of the teeth in the osleomyelitic area did not clear the patient of symptoms. A patch test to nickel turned out to be positive. After removal of the stainless steel cerclage in the mandible, clinical and radiographic changes returned to normal within 2 and 8 weeks, respectively. It is concluded that, whenever acute treatment docs not lake priority, patch tests comprising nickel, cobalt and chromium should he performed in patterns who are candidates for metal osteosynthesis.

    Abstract. In a sample of 450 patients with oral leukoplakias, 32 used snuff, which was a Danish wet variety, Göteborg snus ® (Gothenburg Snuff). The group of snuff users consisted of males only, with a mean age of 58.4 years. The mean life exposure time lo snuff was 197,000 hours, the equivalent of 22 years of constant use. Patients with leukoplakia due lo the use of snuff experienced fewer symptoms than did patients with leukoplakia due to other unknown causes. The snuff users also tended to be older at first examination, and to have a higher alcohol intake. Histological changes characteristic of snuff-users' leukoplakias were: (1) vacuolated cells in the upper part of the spinous cell layers of the epithelium with intervening streaks of parakeratinized cells, (2) epithelial hyperplasia, and (3) hyalinized, eosinophilic deposits around salivary glands. However, statistically significant differences were not found between snuff-induced leukoplakias and other leukoplakias either as lo dysplasia observed histologically or as to malignant development.

    Patients allergic to pollen from alder, birch and hazel were hyposensitized during a 3-year period with either birch pollen extract alone (n= 24) or a mixture of one or more of alder, birch and hazel pollen extracts (n= 27). The effect of the treatment was evaluated by RAST and tandem crossed-radioimmunoelectrophoresis (tandem-CRIE). The patient' specific IgE response to the major allergens of alder (Aln g I), birch (Bet v I) and hazel (Cor a I and Cor a II), as measured by tandem-CRIE, and the total specific IgE response, measured by RAST, decreased significantly (Pc 〈 0.05) during immunotherapy, irrespective of the extract used during the treatment. There was no significant difference (Pc 〈 0.05) between the two treatment groups. The results obtained indicate either that birch pollen extract alone is adequate in the treatment of the studied patient group or the patients had been sensitized towards birch pollen alone.

    Tissue extracts from 18 oral carcinomas were tested in the leukocytes migration capillary technique(LMCT) against autologous leukocytes and against leukocytes of healthy matched controls. The normal range was defined as the mean migration index (MI) of the leukocytes of the normal control ±2SD. Three carcinoma patients showed significant inhibition and one was on the lower borderline on the normal range. None showed stimulation. When leukocytes of leukoplakia patients were tested against normal intesttine and two homologous oral carcinoma extracts, significantly different MI's were found between carcinoma and normal intensive. The finding are compatible with a state of tissue-specific, cellular hypersensitivity against antigenic components present in oral leukoplakia and in oral carcinoma, and is possibly common between the two.

    Histamine release induced by Staphylococcus aureus was examined in cells obtained by bronchoalveolar lavage (BAL) in non-topic individuals. Approximately half of the individuals responded with mediatior release to the bacterium, and the release was found to be time- and concentration dependent. No difference was found between the patients obtained by who responded and those who did not respond in regard to age, sex, smoker/nonsmoker % recovery of BAL-fluid, total cell count, differential cell counts, histamine content per mast cell, or diagnoses. Also stimulation of the BAL-cells with the calcium-ionophore A23187 resulted in histamine release, S. aureus-induced histamine release from basophils was examined in leukocyte suspensions obtained from the same individuals, and in all experiments release was found. The dose-response curves were similar to those obtained with BAL cells- The bacteria-induced mediator release from superficially lying cells in the airways epithelium might be of importance for the precipitation or exacerbation of bronchial asthma in respiratory tract infections.

    Fifty-four adult patients with tree pollen-induced rhinitis (28), asthma (1), or rhinitis and asthma (25) were selected for immunotherapy with standardized and partly purified tree pollen extracts using a double blind protocol. The selection was based on clinical history, results of nasal or bronchial challenge, skin prick tests and RAST. Further, based on crossed radio-immunoelectrophoresis, sex, age and severity of symptoms, the patients were allocated in matched pairs and the treatment alternatives were randomly distributed within the pairs. Twenty-three patients treated with extracts composed of any combination of alder, birch and hazel pollen which matched their IgE response in CRIE (Group 1 (ABC)) and 22 patients receiving birch pollen extracts (Group 2 (B)) completed ail 3 years of treatment. The in vivo results comprising symptom and medicine consumption scores are given here. Changes in specific skin and nasal reactivity as well as in immunological parameters arc presented separately. No significant differences were demonstrated between the treatment groups in the two parameters. Both extracts were effective and reduced in general the symptom scores to one tenth of the starting level. Expressed another way, at the end of the study, the patients tolerated 30 times more pollen until symptoms of the same severity were elicited, compared to before. In the Nordic countries, spring-time asthma and rhino-conjunctivitis caused by pollen from deciduous trees can be effectively treated with an extract of birch pollen alone.

    Dialysed extracts of alder (Alnus glutinosa) and hazel (Corylus avellana) pollens were characterized by immunochemical methods. The alder pollen extract contained at least 33 distinct antigens of which one, antigen 5, was classified as a major allergen, three, antigens 4, 10, and 17, were classified as intermediate allergens and two, antigens 19 and 23, were classified as minor allergens. The molecular weight and pI of the major allergen were determined to be 19,000 D and 5.2, respectively. The hazel pollen extract contained at least 40 distinct antigens, two of which were classified as major allergens, antigens 8 and 13, three as intermediate allergens, antigens 12, 17, and 26, and seven as minor allergens, antigens 6, 11, 16, 18, 29, 39, and 40. The molecular weights and pI's of the major allergens of hazel pollen were determined to be: antigen 8; Mw = 12,000 D, pI = 5.1 and 〈 3.5, antigen 13; Mw = 26,000 D, pI = 5.1. Further, crossed line immunoelectrophoresis and tandem crossed immunoelectrophoresis of alder, birch and hazel strongly indicate that there exists an immunochemical partial identity between the major allergens (antigens 5 (alder), 23 (birch), and 13 (hazel)) from these tree pollens.

    Aims:  Langerhans cell histiocytosis is a rare disease with clonal proliferation of dendritic histiocytes, occurring most frequently in infancy and early childhood. In the localized form (single system), the disease is self-limiting, but in the cases of multisystem disease a third of the patients develop organ dysfunction. In these cases the prognosis is poor. Our objective has been to study the immunohistochemical expression of Fas and Fas-ligand (Fas-L) in order to determine whether the level of expression of these proteins could predict the outcome of the disease. We also wanted to determine the number of apoptotic cells to compare with the expression of Fas and Fas-L.Methods and results:  We analysed the expression of Fas and Fas-L in 76 infiltrates from 49 paediatric patients with Langerhans cell histiocytosis. We also compared the results with the expression of the tumour suppressor protein p53 and the number of cells in apoptosis detected with TUNEL. Langerhans cell histiocytosis cells showed strong expression of p53 and in some cases co-expression of Fas and Fas-L. The expression of Fas-L was significantly higher in infiltrates from patients with single-system disease. The actual number of pathological Langerhans cells in apoptosis as estimated by TUNEL was low.Conclusions:  The low number of TUNEL-reactive cells can be explained by the rapid turnover of apoptotic cells in the tissue, not leaving the apoptotic cells long enough in the tissue to be detected. The co-expression of Fas and Fas-L in some Langerhans cells can lead to an autocrine apoptotic shortcut, mediating the death of the double-positive cells. Our findings suggest that apoptosis mediated through the Fas/Fas-L pathway may contribute to the spontaneous regression of lesions in single-system disease. A delicate balance between autocrine death and survival of Langerhans cells may have been disturbed in patients with multisystem lesions.

Routine intraoperative transesophageal echocardiography (TEE) revealed a previously undiscovered ventricular positioning of an infected ventricular lead left in place for 10 years. This case report describes successful removal of this lead from the left ventricle by means of excimer laser and discusses some important aspects to be considered.
