    Sixty adult females of ASA grade 1 or 2 scheduled to undergo diagnostic laparoscopy were allocated randomly to one of two groups. In group A, laparoscopy was performed with a standardised general anaesthetic technique alone. In group B, the same general anaesthetic technique was supplemented with bilateral rectus sheath block. Postoperative analgesia was assessed at 1, 6 and 10 hours after operation. Visual analogue pain scores in group B were significantly lower than in group A despite a greater use of intramuscular analgesic injections in group A (p 〈 0.005 in each case).

    Sciatic nerve block was performed in two groups of patients using a low power peripheral nerve stimulator to aid nerve location. In group A 1% prilocaine with felypressin was used as the local anaesthetic agent in a volume of 0.25 ml/kg body weight. In group B 3% prilocaine with felypressin was used in a volume of 0.08 ml/kg body weight (i.e. equal total drug dosages). Use of the 3% solution resulted in highly significant reductions in the mean latency for analgesia of the nerve block and in the latency and degree of motor block achieved (p 〈 0.005 in each case). The clinical value of high concentration, low volume nerve block is discussed.

    The distribution of paraesthesiae evoked by a low power nerve stimulator during performance of supraclavicular brachial plexus block was studied in 120 patients. Evoked paraesthesiae in the distribution of the median nerve was associated with a significant improvement in effectiveness of the block when compared with evoked paraesthesiae in the ulnar and radial distributions. A possible explanation for this funding is proposed.

    A technique of continuous sciatic nerve block is described. The method was used to relieve pain from ischaemic gangrene of the foot for 2 days before below-knee amputation and, combined with a continuous inguinal paravascular block, to provide regional anaesthesia both for the surgery and for the first 2 postoperative days.

    Eighty patients who presented for surgery of the forearm or hand were allocated randomly to one of two groups. In Group A, surgery was performed under supraclavicular brachial plexus block only; a mixture of equal parts of prilocaine 1% and bupivacaine 0.5% without adrenaline was used. In Group B, supraclavicular brachial plexus block was performed using prilocaine 1% alone, but in addition discrete nerve blocks were performed at elbow level using 0.5% bupivacaine without adrenaline. Patients in Group B had a significantly shorter duration of unwanted postoperative motor blockade and a significantly longer duration of postoperative analgesia (p 〈 0.005).

    High frequency jet ventilation has been used clinically in this unit for over 2 years. During this time we have treated 63 intensive care patients for whom the mean duration of ventilatory support was 3.4 days, which represents more than 5000 patient hours in total, with the Penlon Bromsgrove humidified jet ventilator. The series comprises a wide variety of general intensive care patients; a review of these cases is presented.

    The use of a low-power nerve stimulator to aid nerve location during the performance of sciatic nerve block was compared with a conventional anatomically based technique. The success rate, both in terms of the production of an effective block and the duration of postoperative analgesia, was significantly higher in the stimulator groups than in the non-stimulator groups. The implications of the study for both clinical and teaching practice are discussed.

    The Penlon Bromsgrove is a new high frequency jet ventilator, suitable for use in both adults and children. The jet stream is humidified by means of an integral in-line Bernoulli nebuliser. The self-recharging nebuliser can also convey drugs directly to the airways in the form of an aerosol. Alarms and fail-safe systems are incorporated. A pressure gauge continuously displays the jet drive pressure. There are two digital LED displays; one shows jet frequency in breaths per minute; the other, the jet drive pressure, minimum, maximum and mean patient airway pressures. Two fail-safe systems ensure that these pressures do not become excessive. Both audible and visual alarms are provided. The prototype has proved to be quiet and completely reliable over more than 4000 hours use, with no bronchoscopic or histological evidence of ineffective humidification.

    Using directed mutagenesis, amino acid substitutions have been made in the α- and β-subunits of the Klebsiella pneumoniae nitrogenase component 1 at positions normally occupied by conserves cysteine or tyrosine residues. Nif', Nif and intermediate pheno-types have been obtained. To extend our earlier biochemical characterization (Kent et al, 1989) the electrophoretic mobiliy of component 1 of the mutant and wild-type nitrogenases has been analysed by non-denaturing gel electrophoresis. The major and minor forms of component 1 separated by this methodology have been probed for by using both polyclonal and monoclonal antibodies. All Nif mutants exhibited a distribution of electrophoretic forms of component 1 comparable to the wild type, and the abundance of the major from found in purified nitrogennase correlated approximately with the specific activity of the extract. In contrast, after electrophoresis, component 1 from Nif mutants exhibited either a major low-mobility from or a fast-moving from. Analysis of co-factor (FeMoco) allowed us to conclude that changing cysteine 275 to alanine in the α-submit produces component 1 defective in its interaction with FeMoco. Substitution of other con-served cysteine residues by alanine appears to prevent early steps in nitrogebnase assembly or to promote degradation. Two single mutations (cysteine89 to alanine in the β-subunit) which are tightly Nif can be combined to produce a weakly active nitrogenase, indicating regions involved in the interaction between subunits.

    Zusammenfassung Die Nachweis-Empfindlichkeit der Atomarabsorption für 39 organische Siliziumverbindungen und 8 organische Zinnverbindungen wurde ermittelt, um so den Zusammenhang zwischen Struktur und Empfindlichkeit zu untersuchen. Das scheinbar anormale Ergebnis, daß die Empfindlichkeit abnimmt, je geringer der Energieinhalt der Alkyl-Silizium- bzw. Alkyl-Zinn-Bindung ist, wird zu erklären versucht.

    Summary Sensitivities for 39 organosilicon and 8 tin compounds in atomic-absorption analysis have been determined with the object of studying the relation between structure and sensitivity. A theory has been advanced in order to explain the seemingly anomalous result, that for alkylsilanes and alkyltins the sensitivity decreases as the energy of the alkyl—silicon (tin) bond decreases.

    Abstract Molybdenum black solar selective coatings have been produced on cobalt by immersion in a solution of ammonium paramolybdate and nickel sulphate. The cobalt was electroplated on nickel-plated copper prior to immersion in the paramolybdate solution. The maximum solar absorptance of the resulting molybdenum black coating was about 0.91. The minimum emittance was about 0.1 for coatings on cobalt deposited with an addition agent and about 0.23 for coatings on cobalt deposited without addition agent in the 60 °C plating solution. These differences have been related to the coating morphology determined by scanning electron microscopy. The emittance of coatings on cobalt deposited without addition agent decreases (or remains unchanged) during short-term heat treatment while that of coatings deposited on cobalt plated with addition agent increases somewhat. Reduction of the cobalt plating bath temperature to 45 °C can also lead to good initial coating properties but without the requirement for an addition agent in the cobalt plating bath. XPS studies show that the oxidation state of molybdenum in the coatings is approximately + 5 corresponding to Mo4O11. This reduces to + 4 after argon ion bombardment. Some cobalt may be present in the coatings in the form of CoO.

    Abstract Optical and microstructural properties of electrodeposited molybdenum-copper (Mo-Cu) black coatings have been studied with reference to their selectivity in absorption of solar radiation. Such coatings were found to have a solar absorptance, α, about 0.87 and low thermal emittance, ɛ, such that the selectivity, α/ɛ, was 3.6. Electrodeposited molybdenum-black coatings generally have selectivity α/ɛ∼3. The oxidation state of molybdenum in (Mo-Cu) black coatings as determined by X-ray photoelectron spectroscopy is about + 5 (which is fairly close to that of Mo4O11). Large numbers of irregular particles were found on the surface of molybdenum-copper black coatings. There is evidence that the particles contain copper oxide.

    Abstract Molybdenum black solar selective coatings have been prepared by a simple chemical conversion method on chemically etched zinc substrates. These coatings have been characterized using X-ray diffraction, electron diffraction, XPS and VIS-IR reflectance spectroscopy. Maximum solar absorptance obtained from these coatings was about 0.87 with an emittance of 0.13–0.17. Selectivity of the coatings, mainly attributed to the coating surface morphology, was studied by SEM. Structural studies show that the coating consists of a non-stoichiometric oxide of Mo, namely Mo4O11. XPS depth profiling studies show that the oxidation state of Mo reduces from +5 to +4 after argon ion bombardment.

    Abstract An unusual, siphonaceous, green algal thallus grows on the support floats of marinas in the Solent area of southern England. Comparative studies of British species of the order Codiales collected from the marinas, particularly relating to vegetative structure, ecology, betaine and tertiary sulphonium compounds and lectin activity of extracts of the algae, led to the conclusion that the thallus was a juvenile stage ofCodium fragile ssp.tomentosoides.

    Abstract An IBM AT monitoring system is the current version of a series of monitoring system developments at Vanderbilt University. This system and its predecessors have been used in over 800 cases to date. Within the past year, this system has been upgraded with a new serial communications board and with touchscreen input. The new communications board has allowed improved communication with serial devices in the operating room, and will allow for additional devices to be added. The most dramatic improvement in the system has come from the addition of touchscreen input. Our present use of this feature has dramatically improved a casual users ability to input data into the computer system, extensions are possible to entirely get rid of the necessity of any keyboard input of data to the system under normal operations.
