    Background : Pancreatic cancer is an aggressive cancer with a low survival time. So far, there have been no studies assessing direct and indirect costs in individual patients.Aim : To assess prospectively the cost of illness in patients with pancreatic cancer.Methods : Patients were consecutively included at first admission to hospital. Sociodemographic factors, medical resource use and employment status were assessed by patient interviews and from medical records in a standardized way. Costs were calculated from the perspectives of the hospital, social insurance and society. Linear regression analyses were used to determine factors associated with costs.Results : A total of 57 patients were admitted with suspected pancreatic cancer. Of these patients, 45 (79%) had pancreatic cancer as final diagnosis, 11 (19%) pancreatitis and one patient cystadenoma. The median survival was 10.9 months in patients with pancreatic cancer. Per month of observation from societal perspective, total costs were €4075 for patients. Costs of hospitalizations were responsible for 75% of total costs. In multivariable analyses, surgery, a lower educational level, younger age, and the prevalence of metastases were significantly associated with higher total costs.Conclusions : Costs are considerable in patients with pancreatic cancer. Our results may facilitate further economic evaluations and aid health policy-makers in resource allocation.

    Zusammenfassung Bericht über 215 transduodenale Sphincterotomien (156 Erstoperationen, 59 Rezidivoperationen). Absolute Indikationen zur Sphincterotomie bildeten der eingeklemmte Papillenstein und die Papillenstenose, relative Indikationen Fälle von Choledocholithiasis ohne Beteiligung der Papille. Die Operationssterblichkeit betrug 3,2% (2,5% bei Erstoperationen, 5% bei Rezidivoperatioen). 15 Pat. starben in den folgenden Jahren. 183 Pat. (=94%) wurden 2 bis 7 Jahre postoperativ nachuntersucht: 162 Pat. (88,5%) waren vollkommen beschwerdefrei; 21 Pat. (11,5%) gaben Beschwerden an. Die Ursache der Beschwerden konnten bei 11 Pat. nicht eindeutig geklärt werden, bei 4 Pat. fand sich eine chronische Pankreatitis, 6 Pat. wurden reoperiert (2mal Choledocholithiasis, 1mal Narbenstenose des Choledochus, 1mal Hepatitis, 1mal Ileitis terminalis, 1mal Ulcus duodeni). Bei 3 Pat. wurden anläßlich der Reoperation 2, 4 und 5 Jahre nach Sphincterotomie der Choledochusresidualdruck und der Papillendurchfluß kontrolliert. Bei 123 Pat. wurde 2 bis 7 Jahre nach Sphincterotomie die Ventilfunktion der Papille untersucht. Ein duodenobiliärer Reflux wurde in 43% der Fälle beobachtet. Ein Zusammenhang zwischen Reflux und postoperativen beschwerden ließ sich nicht nachweisen.
    Notes:
    Summary Report on 215 transduodenal sphincterotomies (156 primary operations, 59 reoperations). The absolute indications for transduodenal sphincterotomy were impacted papillary stone and papillary stenosis. The relative indications were cases of simple lithiasis of the common duct without involvement of the papilla. The overall mortality rate was 3,2% (2,5% in primary operations, 5% in reoperations), 15 patients died during the subsequent years. 183 patients (94.8%) were followed up 2 to 7 years after operation: 162 (88.5%) patients were completely free of complaints, 21 patients (11,5%) reported complaints. The cause of complaints could not be established unequivocally in 11 patients, 4 patients suffered from chronic pancreatitis, 6 patients were reoperated. The operative findings in the reoperated patients were: common duct stones (2 patients), cicatriceal stenosis of the common duct (1 patient), hepatitis (1 patient), regional enteritis (1 patient), duodenal ulcer (1 patient). In 3 patients, on the occasion of the reoperation, 2, 4 and 5 years after sphincterotomy, the common duct residual pressure and the papillary flow could be measured. In 123 patients the function of the papillary valve was examined 2 to 7 years after transduodenal sphincterotomy. Duodeno-biliary reflux was observed in 43% of cases. A correlation between reflux and post-operative complaints could not be established.

    Neuroendokrine Tumoren (NET) sind seltene Neoplasien mit einer geschätzten Inzidenz von 0,1–1/100.000 [31], wobei etwa 75% der NET im gastroenteropankreatischen System lokalisiert sind [25]. Diese Tumoren zeigen einen deutlich protrahierten Erkrankungsverlauf im Vergleich zu anderen gastrointestinalen Neoplasien; so liegt die Fünfjahresüberlebenszeit in Abhängigkeit von Primärlokalisation und Metastasierung bei ca. 50% [25].
