    Abstract Early allograft vascular wall degeneration has emerged as a major important complication in young patients. To explain this mechanism, we reviewed studies on explants of allograft valved conduits implanted heterotopically into the infrarenal aorta in inbred rats (LEW;RT1, and CAP-RT1°C). The following strain combinations (isografts and allografts) were used: syngeneic, LEW-〉 LEW, strongly allogeneic, and CAP 〉 LEW (RT1- and non-RT1-incompatible). Second-set skin grafting was performed 3 weeks after the heterotrophic implant to test for immunogenicity and presensitization. The animals (LEW) were sacrificed serially on days 20, 30, 50, and 100 for immunofluorescence and SEM studies. Endothelial disruption was observed on day 30, while valve leaflets appeared normal. Humoral allograft rejection was demonstrated and associated with production of antibodies (IgG) against the endothelial cells and around the smooth muscle cells, and in areas of smooth cell necrosis, through 100 days. Neointimal repopulation by host cells and migrated smooth muscle cells was also observed in both viable and allovital grafts. Allovital grafts demonstrated more disorganized collagen and elastic fibers, as well as calcific degeneration in the media and neointima on day 50; the viable conduits showed such structural changes on day 100. In conclusion, vascular walls of allovital conduits calcified earlier than the viable conduits without discernible calcification of the valves. There is therefore evidence to prove causative relationships between cellular viability, immune response, and fibroproliferative calcific degeneration in allograft vascular conduits.

    The relevant medical literature has been reviewed objectively and the conclusion reached that the Wilson-Smith modification was first in the field.The importance of adequate air-passages within anæsthetic machines is stressed and attention drawn to the fact that the “ wide-bore principle,” introduced as long ago as 1899, has since become the sine qua non for all modern apparatus involving the closed method.The various wide-bore modifications are briefly described, compared and contrasted, and the various devices employed for maintaining the wide-bore whilst allowing for variation in ether vapour strength are illustrated.Quotations are given which show exactly how the benefits of wide-bore inhalers appeared to those who first used them.

    Abstract High-frequency ventilation (HFV) has not one but several histories. Proceeding along largely independent pathways are techniques using frequencies of 1–5 Hz and techniques using 10–40 Hz, the former being introduced by Sjostrand in 1971, the latter by Lunkenheimer in 1972. It was nearly ten years before it was recognized that these techniques must radically alter our concepts of gas transport within the lung. There has also been an unfortunate series of clinical studies purporting to show that HFV is superior to conventional ventilation in patients with lung disease. There is no doubt that nearly all reports show that HFV controls PaCO2 very easily, and that the mechanisms by which this is achieved is the primary topic of the symposium. What has not been proved is that HFV is better than conventional ventilation in increasing PaO2. The mechanisms of oxygen exchange when the lung has extensive shunts are quite different from those for CO2 exchange, and this problem has not been rigorously addressed.

    Linear polymers were prepared by the condensation of bis(dimethylamino)dimethylsilane and 1,4-bis(dimethylaminodimethylsily)benzene with fluorocarbon diols. 1,5-Dihydroxy-3-methyl-1,1,5,5-tetrakis(trifluoromethyl)-2-pentene, the cis addition product of hexafluoroacetone and isobutylene, with the silylbenzene monomer gave a polymer that cured at room temperature to a rubber exhibiting a glass transition temperature of 0°C, low swelling in hydrocarbons, and excellent resistance to hydrolytic, oxidative, and thermal degradation, retaining its flexibility after exposure to air for 3 hr at 305°C. The polymers obtained by condensing 1,5-dihydroxy-1,1,5,5-tetrakis(trifluoromethyl)-2-pentene, the trans addition product of hexafluoroacetone and propylene, with the silylbenzene and the silane monomers had glass transition temperatures of -12 and -50°C respectively, and greater resistance to swelling in hydrocarbons.
