The purpose of this study was to investigate the influence of postnatal x-ray pelvimetry after caesarean section on the management of the subsequent pregnancy. The case records of 331 women delivered by casearean section in their first pregnancy were reviewed. By standard radiological criteria, the pelvis was considered to be inadequate in 248 (75%) of them and adequate in 83 (25%). Of the women with a radio-logically inadequate pelvis, 172 underwent an elective caesarean section. Seventy-six were allowed vaginal delivery: 51 of these women delivered vaginally and 25 required an emergency caesarean section. Of the women with a radiologically adequate pelvis, 61 achieved a vaginal delivery and 22 were delivered by caesarean section. All of the three cases of uterine rupture occurred in women with a radiologically adequate pelvis. This study suggests that x-ray pelvimetry is not a good predictor of the outcome of a trial of vaginal delivery. We conclude that the practice of routine postnatal pelvimetry should be abandoned.Type of Medium: Electronic ResourceURL: -

The system captures every rule as a rudimentary neural network, which is called a network element (netel). The aim is to preserve the semantic structure of the expert system rules, while incorporating the learning capability of neural networks into the inferencing mechanism. These netel rules are dynamically linked up to form the rule-tree during the inferencing process, just as a conventional expert system does. The system is also able to adjust its inference strategy according to different users and situations. A rule editor is provided to enable easy maintenance of the netel rules. A number of key optical and structural properties are studied within local density-functional theory. In1−xTlxP at x=0.67 and In1−xTlxAs at x=0.15 are estimated to have a gap of 0.1 eV. Their binding energies are larger than that of InSb, and they are found to form stable zinc-blende alloys for all x. Published 1996Staff ViewISSN: 1077-3118Source: AIP Digital ArchiveTopics: PhysicsNotes: In this letter, we demonstrate how the differential absorption technique can be applied to study band gap uniformity in HgCdTe samples. The transmission of HgCdTe wafers is measured at room temperature and data filtering is used to remove both the interference fringes and high-frequency noise. This data treatment approach produces reliable transmission data for HgCdTe layers as thin as 8 μm. In addition, the spectrum of the interference fringes is used to estimate the HgCdTe layer thickness with an accuracy of ±0.1 μm. The absorption coefficient is differentiated twice with respect to the photon energy and an approximate value of the band gap is estimated from extrema of the derivatives. By applying this procedure to different points on the same HgCdTe wafer, we can determine both lateral and transverse fluctuations of the semiconductor band gap. The initial results indicate that the accuracy of the differential absorption technique is around ±0.5 meV for HgCdTe wafers. The differential absorption technique appears suitable for routine characterization of HgCdTe wafers because it is simple, nondestructive, and applicable at room temperature. © 1996 American Institute of Physics.Type of Medium: Electronic ResourceURL: -

Total internal reflection of THz pulses in high-resistivity silicon prisms provides a nearly 180° phase shift of one arm relative to the other to achieve destructive interference. We show that due to automatic compensation for laser fluctuations by the interferometer, it is possible to measure the index and absorption of thin-film samples with more accuracy than is achievable with standard THz time-domain spectroscopy. We demonstrate characterization of the complex index of refraction of 2 μm thick Mylar (polyester) films. By measuring the signal amplitude directly in the time domain, the interferometer can be used for rapid measurements of film thickness with a resolution of better than 1 μm. © 2001 American Institute of Physics.Type of Medium: Electronic ResourceURL: -

ISSN: 1365-2044Source: Blackwell Publishing Journal Backfiles 1879-2005Topics: MedicineNotes: We present the results of a randomised, double-blind controlled trial to determine the effect of adding bupivacaine to intraperitoneal Hartmann's solution, used to reduce the incidence of postoperative adhesions, on postoperative pain and on analgesic consumption in patients presenting for major laparoscopic gynaecological procedures. Fifty-six women were studied and postoperative analgesic requirements and visual analogue scores were used to assess the pain experienced by the treatment group when compared with the control group. There was no statistical difference in the pain scores between the two groups at any time during the study period (Student's t-test; p = 0.29–0.74) nor was there any difference in analgesic consumption (Mann–Whitney U-test; p = 0.34–0.79).Type of Medium: Electronic ResourceURL: -
