ISSN: 1365-2214Source: Blackwell Publishing Journal Backfiles 1879-2005Topics: MedicinePsychologyNotes: Background The aim of this study was to establish the medium-term (three-year) psychosocial outcome of children with behavioural problems and their parents, who had received an intervention from a family support service.Methods Forty families were traced at the three-year follow-up and agreed to participate. Pre- and post-intervention and follow-up measures were the Health of the Nation Outcome Scales for Children and Adolescents (HoNOSCA) and the Strengths and Difficulties Questionnaire (SDQ).Results The primary HoNOSCA outcome items (i.e. those initially targeted by the parenting intervention) of aggression/antisocial behaviour and family relationships were not found to have changed significantly from the baseline (but had not sustained the sort-term improvement following the intervention). Deterioration was found in other HoNOSCA items such as overactivity, self-harm, scholastic/language skills, emotional, and poor school attendance. When we compared pre-intervention with follow-up SDQ scores, there was no significant change on any scales, i.e. these had returned to the level reported at the time of the original referral to the family support service.Conclusions Following the intervention from a family support service, children and families reported a significant improvement in most outcome measures, predominantly child behaviour and family relationships. However, these improvements were either not sustained or there were additional difficulties at three-year follow-up. These could be related to various external and developmental factors. This lack of sustainable treatment effects for children with behavioural problems is consistent with previous research findings on parenting programmes.Type of Medium: Electronic ResourceURL: -

Published 1995Staff ViewISSN: 1468-3083Source: Blackwell Publishing Journal Backfiles 1879-2005Topics: MedicineNotes: Background There is a general perception that sexual interest and activity does not persist into later life, with consequential failure to appreciate the potential prevalence of sexually transmitted diseases in the elderly.Methods Retrospective analysis of case notes of all patients over the age of 65 years who attended a Genito-Urinary Medicine Clinic: 81 patients were identified over a 6 year period.Results Referral because of positive serology for syphilis accounted for 20% of patients and was the most common reason for presentation. Significantly, another 14% of patients attended for HIV testing. Of the 61 patients whose sexual history was available, 32 had sexual intercourse within the 6 months prior to attendance.Comment This review confirms the need for the medical profession to be more aware of the sexual activity and the possibility of sexually transmitted diseases in this patient group.Type of Medium: Electronic ResourceURL: -

ISSN: 1468-3083Source: Blackwell Publishing Journal Backfiles 1879-2005Topics: MedicineNotes: Background Patients who are HIV positive have on increased risk of developing a malignancy, most notably Kuposi's sarcoma (KS) and Non-Hodgkin's Lymphoma (NHL).Objective To assess the incidence of malignancy in a HIV positive cohort in Ireland.Methods We retrospectively reviewed all case notes of 784 HIV positive patients attending a GUM clinic of whom 340 had Stage IV disease.Results There were 43 diagnoses of malignancy in 41 patients, the majority of whom were homosexual. There were 25 cases of KS, seven cases, of primary cerebral lymphoma, seven cases of systemic NHL and four other malignancies. The overall incidence of malignancy was 12.1% of all patients with AIDS (i.e. 41/340 patients).Comment The incidence is lower than previously reported studies, but represents significant morbidity. The lower incidence may be explained by our patient demographic profile, In addition, malignancies may occur at a stage when patients are too ill to pursue investigation and post mortem examination may be refused. The need for close monitoring of high risk groups, such as HIV positive women with cervical dysplasia, has been confirmed.Type of Medium: Electronic ResourceURL: -

The deduced amino acid sequence of the coagulase protein is compared with previously reported sequences of coagulases from strains 213 and BB. The secreted mature forms of coagulase proteins are composed of three distinct segments: (i) the N-terminal 150–270 residues, which are c. 50% identical, (ii) a central region with high (〉90%) residue identities, and (iii) a C-terminal region composed of repeated 27-amino-acid residue sequences. The variable N-terminal sequences are probably responsible for antigenic differences among coagulases of different serotype. The region of coagulase which binds to prothrombin and activates it to form staphylothrombin is also located in the N-terminal half of the protein.A site-specific substitution mutation in the coa gene, which abolished plasma clotting activity, was isolated by recombinational allele-replacement in strains 8325-4 and M60. The Coa− mutants did not show diminished virulence in subcutaneous and intramammary infections of mice. No evidence for a role for coagulase in virulence of toxigenic or non-toxigenic strains was obtained. This contradicts findings of several groups using Coa− mutants generated by chemical mutagenesis and suggests that the earlier results were obtained with strains that had suffered additional mutations in virulence-related genes.Type of Medium: Electronic ResourceURL: -

The hla gene was cloned from an Hla- TSST-1+ strain, Todd 555, which had been isolated from a case of toxic shock syndrome in the USA. Of the 630 bases of the Todd 555 gene sequenced, 46 differed from the hla gene sequence of strain Wood 46. The defect in α-toxin expression was shown to be due to a nonsense mutation which converted a CAG glutamine codon in the equivalent position in the functional Wood 46 sequence to a TAG stop codon. The same mutation was present in the hla gene cloned from a human septicaemia strain (V37) isolated in Dublin. The nonsense mutation of Todd 555 was suppressed by the supE 44 mutation in Escherichia coli resulting in haemolytic activity in cell lysates. Hybrid hla genes were formed by splicing fragments of hla from Todd 555 and Wood 46. Expression of one such chimaeric hla gene in S. aureus demonstrated that the Todd 555 hla gene has a functional agr-regulated promoter. Elderly orthopaedic admissions were allocated alternately to the Beaufort system and to the usual trolley, table and bed surfaces, and followed from admission to hospital until separation. The incidence of pressure sores was 15-6% in the 32 ‘trial’ patients, which was significantly less than the 48-8% in the 43 ‘control’ patients, as was the mean maximum diameter of the pressure sores incurred: 6-4 mm for the ‘trials’ as against 29-5 mm for the ‘controls’. In particular the trial group were free from pressure lesions to the heel, which affected 32-6% of the control group. The groups were well matched on a variety of criteria on admission, and we conclude that the Beaufort system successfully reduces the incidence and severity of pressure sores for elderly orthopaedic patients. The system – renamed recently the ‘Neumark-Macclesfield Support System’– is now in regular and satisfactory use.Type of Medium: Electronic ResourceURL: -

Chacun des patients présentait une perte considérable de la masse osseuse du fémur proximal due à la précédente arthroplastic. Afin de renforcer et de compenser le fémur naturel on avait procédé à des allogreffes fémorales proximales et des allogreffes à étai cortical. Les apparences radiographiques ont été établis sur une période moyenne de suivi médical de deux ans et demi. Un schéma bien défini apparaît à partir du moment où la greffe commence à se souder à l'os récepteur. On observe d'abord l'apparition d'une tache trouble et voilée aux extrémités de l'allogreffe suivi plusieurs mois plus tard d'un effacement aux extrémités. Une fois la greffe prise, on remarque une sclérose et trabécule grossière du corteux. Dans tous les cas observés il s'était produit une résorption, en général au niveau des cerclages métalliques fixant la greffe à l'os récepteur. Parmi les autres complications il faut citer également l'infection, la fracture de l'allogreffe et du fémur récepteur et la résorption de la greffe elle-même Dans trois des cas la greffe ne s'était pas faite et dans onze des cas elle n'était que partielle. Il est ainsi possible d'observer au niveau radiographique le schéma évolutif d'une allogreffe tandis qu'elle se soude à l'os récepteur. Il est utile de savoir reconnaître cette évidence afin de pouvoir estimer la progression de la soudure des os.Notes: Summary The aim of this study was to describe the radiographic appearances of femoral allografts in 29 total hip arthroplasty (THA) revision procedures among 21 patients. Each patient had evidence of severe bone loss in the proximal femur related to previous hip arthroplasty. Proximal femoral allografts and cortical strut allografts were used to supplement and reinforce the native femur. The radiographic appearances were reviewed over a mean follow-up period of 2.5 years. A definite pattern emerged as the graft proceeded to unite with the host bone. The earliest feature was haziness and blurring of the ends of the allograft, followed by rounding and tapering of the ends, several months later. Eventually, sclerosis and coarse trabeculation of the cortex was seen when the graft had incorporated. A minor degree of resorption occurred in all cases, most commonly beneath the cerclage wires binding the grafts to the host bone. Complications included infection, fracture of the allograft and the host femur, and resorption of the graft. There were 3 cases of complete non-union and 11 cases of partial union. A definite radiographic pattern is evident as allograft bone unites with host bone. Production of alpha, beta and delta hemolysin, toxic shock syndrome toxin (TSST-1) and staphylokinase was depressed about fifty-fold while protein A production was elevated twenty-fold. Hybridization analysis showed that the defect in expression of TSST-1 and alpha hemolysin was at the transcriptional level. Inability of the mutant strain to express either a cloned TSST-1 gene or the chromosomal gene indicates that the transposon has inactivated a trans-active positive control element.
