The rotational anisotropy in the nonlinear response is found to persist for both metals in the absence of surface resonances and to show convergent behavior in both the relative phase and intensity as the incident energy is lowered from resonant to nonresonant conditions. The phase difference between the in-plane and out-of-plane responses in the long wavelength limit can be described by linear Fresnel theory. Whereas the tensor elements corresponding to the out-of-plane response are found to dominate, the in-plane contributions are found to be non-negligible.Type of Medium: Electronic ResourceURL: -

However, the combined use of these methods has revealed deficiencies in the available methods, and the lack of useful analytical frameworks currently constrains the utility of comparative risk and policy analyses. A principal issue in integrating risk and economic analysis is the lack of common performance metrics, particularly when conducting comparative analyses of regulations with disparate health endpoints (e.g., cancer and noncancer effects or risk-benefit analysis) and quantitative estimation of cumulative risk, whether from exposure to single agents with multiple health impacts or from exposure to mixtures. We propose a general quantitative framework and examine assumptions required for performing analyses of health risks and policies. We review existing and proposed risk and health-impact metrics for evaluating policies designed to protect public health from environmental exposures, and identify their strengths and weaknesses with respect to their use in a general comparative risk and policy analysis framework. Case studies are presented to demonstrate applications of this framework with risk-benefit and air pollution risk analyses. Through this analysis, we hope to generate discussions regarding the data requirements, analytical approaches, and assumptions required for general models to be used in comparative risk and policy analysis.Type of Medium: Electronic ResourceURL: -

ISSN: 1365-2230Source: Blackwell Publishing Journal Backfiles 1879-2005Topics: MedicineNotes: Guidelines have proliferated in recent years in all areas of medicine, sometimes with little evaluation of their effect. Referral guidelines for dermatology were compiled by the dermatologist at the Royal Surrey County Hospital in consultation with local general practitioners. An audit was undertaken to assess how appropriate referrals were just before and after distribution of the guidelines and was repeated 2 years later to determine whether they had made any significant impact. The results revealed a 40% increase in the number of appropriate referrals immediately after introduction of the guidelines, but this was not sustained 2 years later. Five common conditions accounted for two-thirds of inappropriate referrals before and after the guidelines were sent. The need for continued general practitioner education in dermatology to reinforce referral guidelines is demonstrated.Type of Medium: Electronic ResourceURL: -

ISSN: 1471-4159Source: Blackwell Publishing Journal Backfiles 1879-2005Topics: MedicineNotes: —Various parameters of the in vitro incorporation of [14C]amino acids into protein by cell nuclei isolated and purified from rat brain and liver were investigated. Nuclei purified through 2.2 m sucrose solution were capable of amino acid incorporation in vitro; and washing procedure to eliminate hypertonic sucrose before incubation was essential since sucrose in high concentration was inhibitory. Microbial contamination was found to be a serious source of error and the use of sterile conditions for incubation were necessary to obtain reproducible and valid results. Using completely sterile conditions, Na +, K+, RNase, DNase, puromycin, cycloheximide and chloramphenicol were without any effect on the ability of brain and liver nuclei to incorporate labelled amino acids into protein. Results of time-course and preincubation experiments revealed that some factors essential for amino acid incorporation pass out of the nucleus into the medium. In addition, approximately 15 per cent of the labelled nuclear proteins with higher specific radioactivity was recovered in the incubation medium. Incorporation of [14C]leucine was proportional to the concentration of labelled amino acid and to the number of nuclei, and it is suggested that carefully controlled conditions of incubation are essential to obtain valid comparisons between different types of nuclei in terms of their relative abilities to incorporate amino acids in vitro. No evidence was obtained indicating isotope dilution phenomenon in these experiments. Whether or not in vitro incorporation of amino acid by nuclei represents protein synthesis is discussed.Type of Medium: Electronic ResourceURL: -

ISSN: 1365-2044Source: Blackwell Publishing Journal Backfiles 1879-2005Topics: MedicineNotes: In a randomised, double-blind study, we compared a combination of morphine and alfentanil with morphine alone for patient-controlled analgesia (PCA) after Caesarean section under spinal anaesthesia. After surgery, patients were randomly allocated to receive PCA with a bolus dose of either morphine 0.75 mg plus alfentanil 0.125 mg (Group MA, n = 40) or morphine 1.5 mg alone (Group M, n = 37) with a lockout interval of 8 min and no hourly dose limit. Clinical assessments were made in the first 24 h, after which patients completed a written questionnaire. There were no differences between groups in PCA usage or visual analogue scale pain scores measured at 2, 4, 6 and 24 h. There was a low incidence of side-effects in both groups. In the questionnaire, patients in Group MA scored higher compared with Group M when asked to grade speed of onset and effectiveness of analgesia after a PCA bolus; there were no differences in grading for duration of analgesia or overall patient satisfaction. Addition of alfentanil to morphine may have advantages for PCA.Type of Medium: Electronic ResourceURL:
