Patients with macerated stillbirths or who were delivered before the 28th week of gestation had been excluded. Of the 226 patients, 101 received extradural analgesia, 79 received parenteral analgesia and 46 underwent elective Caesarean section. There was no difference in the incidence of breech extraction or emergency Caesarean section in the first two groups of patients. The length of both first and second stages of labour in multiparae was prolonged in the extradural group, but not markedly so. The Apgar scores of the infants delivered vaginally were not significantly different at one minute in both groups but the five minute Apgar score in the infants of primiparae was significantly higher in the extradural group. The Apgar score at one minute in the group delivered by emergency Caesarean section was significantly lower after extradural block but the difference was not significant at five minutes. This study suggests that the management and outcome of labour when the breech presents is not adversely affected by the provision of extradural analgesia.Type of Medium: Electronic ResourceURL: -

The karyopyknotic index (KI) was elevated for the same duration, and levels of luteinizing hormone (LH) and follicle stimulating hormone (FSH) were depressed. Factors VII, X and VII X complex rose significantly during treatment but only factor X levels were significantly elevated at the end of the four-month treatment period. There was no change in factor II or anti-thrombin III levels. These effects on coagulation factors during ethinyl oestradiol therapy are, in general, similar to those found during treatment with conjugated equine oestrogens and oestradiol valerate in a previous study from the same laboratory. Following an initial fall at the beginning of pregnancy there was a significant rise in the mean level of DPG during pregnancy with an abrupt fal in the puerperium. The mean red cell DPG level in the third trimester of pregnancies complicated by pre-eclampsia and diabetes was not statistically different from the normal but the mean value of all pregnancies in which the fetus was stillborn or growth retarded was significantly lower (p〈0.001). The possible mechanism of the changes in normal and abnormal pregnancy is discussed and it is suggested that the measurement of red cell DPG in the third trimester of pregnancy may prove to be a useful parameter of placental oxygenation.Type of Medium: Electronic ResourceURL: -

All but 5 of these patients suffered an attack during pregnancy. The mortality rate amongst primigravidae was 52 per cent, and in all cases 27 per cent.The severity of the disease during pregnancy is probably due to combination of the effect of pregnancy and injudicious administration of barbiturates. The soil was amended with coral lime at 0, 5, and 10Mgha–1 and with cowpea GM at 0, 7.5, and 15Mgha–1. Commercial NPK fertilizers at 50kgha–1 each of N, P, and K were included for comparison. The amendments were applied only once prior to planting of the first crop. Response parameters measured included nutrient composition of leaves at tasseling and grain yield of each crop, and selected soil chemical properties at each planting. Yields of the first crop were nearly tripled with GM additions and doubled with lime additions. Such yield increases were caused mainly by better K nutrition and to a lesser extent by enhanced P nutrition. Yields of subsequent crops were much lower than those of the first, and the declines were much steeper for the GM treatments than for the lime treatments. Thus, the enhancement effect on K nutrition did not last beyond one crop. Poor growth of the second and third crops was caused by K deficiency; probably coupled with Mn toxicity. Significant yield reductions were found when Mn-to-K ratios in leaves exceeded 0.010. As for effects on soil, soil pH was increased significantly by lime but only slightly by GM. Given the variable charge property of this Oxisol, each unit pH increase corresponds to a cation exchange capacity (CEC) increase of 5cmolckg–1. Having greater CEC, the amended soil retained K more effectively, thereby causing yield increases, especially of the first corn crop, which required at least 0.75cmolckg–1 of exchangeable soil K or 7% of CEC for adequate growth.Type of Medium: Electronic ResourceURL: -

A cross-sectional population survey using postal questionnaires was conducted in the North West Thames health region, followed by a prospective cohort study of men undergoing prostatectomy (North West Thames and Oxford regions). The subjects in the first survey were 221 men aged 55 and over with previously reported mild, moderate or severe urinary symptoms; subjects in the second study were 388 men undergoing prostatectomy. Main outcome measures were selfreported symptom severity, bothersomeness and general health status (Nottingham Health Profilie, Part 1). The response rate among eligible responders in the population survey was 85.7%. Increasing symptom severity was associated with worsening NHP scores for energy, emotional reactions, sleep and physical mobility (p〈0.01). Increasing bother-someness of symptoms was associated with emotional reactions, sleep and pain (p〈0.05). Men undergoing surgery reported worse health status than men in the population with the same severity of symptoms as regards emotional reactions, energy and pain. For a given level of symptom severity, the impact of those symptoms on aspects of a man's general health status may be the determinant of seeking and undergoing surgery. Greater understanding of the factors that affect a man's response to his symptoms is needed in interpreting the decision to seek and accept treatment.Type of Medium: Electronic ResourceURL:
